Evaluation of IVIM-MRI biomarkers for microvascular dysfunction and prognosis in juvenile osteochondritis dissecans of the knee: a single-center study.

BACKGROUND: Microvascular dysfunction of the subchondral bone is thought to play a central role in juvenile osteochondritis dissecans (JOCD), but remains difficult to assess with conventional MRI. This study evaluated the diagnostic value of intravoxel incoherent motion MRI (IVIM-MRI) parameters in JOCD of the distal femoral condyle and explored their relationship with lesion morphology and stability. METHODS: In this single-center retrospective cross-sectional study, 57 skeletally immature patients (mean age 13.0 ± 2.0 years) with clinically and radiologically confirmed JOCD underwent bilate

Prevalence of Capitellar Osteochondral Abnormalities in Healthy Youth Gymnasts: An Ultrasound Study.

BACKGROUND: Gymnasts, due to repetitive upper extremity weightbearing, are prone to capitellar osteochondritis dissecans (OCD) lesions, which often require surgery and result in time away from sport. Ultrasound has shown promise for detecting early, often asymptomatic, capitellar osteochondral lesions in baseball players; however, it has not been applied to young, healthy gymnasts. PURPOSE: To determine the prevalence and characteristics of capitellar osteochondral abnormalities in young, high-level gymnasts. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: This instituti

Capitellar Osteochondritis Dissecans Lesion Treated With Bioabsorbable Pin, Extracellular Matrix Cartilage Allograft, and Intraosseous Bioplasty: A Case Report.

Capitellar osteochondritis dissecans (OCD) is a painful and debilitating condition, most often seen in adolescent overhead athletes and gymnasts. Treatment options for unstable capitellar OCD lesions include fragment fixation, autologous chondrocyte implantation, osteochondral autograft, fragment debridement, and drilling of the lesion. While many current surgical techniques yield suboptimal outcomes, this case demonstrates a novel technique for treating an unstable capitellar OCD lesion with associated subchondral cyst in a 14-year-old female patient. This technique combines bioabsorbable pin

Increased contact pressure at the radiocapitellar joint with anterior and posterior osteochondral defects in the sagittal section of the humeral capitellum: a cadaveric study.

BACKGROUND: In osteochondritis dissecans of the humeral capitellum, defect location and size were important in surgical planning. Recently, it was reported that posterior or large osteochondral defects of the humeral capitellum on pre-operative sagittal computed tomography affected outcomes of arthroscopic débridement for osteochondritis dissecans in adolescent baseball players. Here, we investigated radiocapitellar joint contact pressure for anterior and posterior osteochondral defect positions in the sagittal section of the humeral capitellum. METHODS: An anterior or posterior osteochondral

Juvenile osteochondritis dissecans of the knee in the general adolescent population: Prevalence and longitudinal trends.

OBJECTIVES: We established the prevalence of juvenile osteochondritis dissecans (JOCD) of the knee in a general adolescent population undergoing magnetic resonance imaging (MRI), irrespective of symptoms. METHODS: This study was part of an open population-based cohort study, the Generation R study. Between 2017 and 2020, adolescents aged 13 years underwent MRI of both knees. These MRIs were assessed for signs of JOCD. The participant characteristics and presence of symptoms of those with and without JOCD were compared. If an MRI at the age of 13 years showed a JOCD lesion, a follow-up MRI at

Osteochondritis Dissecans of the Capitellum: Arthroscopic Microfracture.

BACKGROUND: Osteochondritis dissecans of the capitellum is a focal injury to the articular cartilage and subchondral bone observed in young athletes. This can cause activity-limiting pain, intra-articular loose bodies, and limited range of motion. INDICATIONS: Stable lesions identified early with minimal symptoms can be managed nonoperatively. Indications for surgical management include unstable lesions, loose bodies, mechanical symptoms, and stable lesions that failed 6 months of nonoperative treatment. A range of surgical options have been advocated-including arthroscopic versus open loose

Concentrated Bone Marrow Aspirate With Extracellular Matrix Grafting for Osteochondral Lesions of the Talus in Pediatric and Adolescent Patients.

BACKGROUND: Treatment of osteochondral lesions of the talus (OLTs) can be challenging due to difficult access/visualization and limited healing capacity. While outcomes after concentrated extracellular matrix combined with bone marrow aspirate concentrate (ECM-BMAC) have been reported for adult populations, its use in pediatric/adolescent patients has not been thoroughly examined. This case series assesses clinical outcomes of ECM-BMAC grafting for young patients with OLTs. METHODS: All patients ≤18 years of age presenting to two surgeons between 2016 and 2024 who underwent primary surgery wi

Influence of repetitive batting on capitellar subchondral bone density distribution.

BACKGROUND: Osteochondritis dissecans of the capitellum occurs in adolescent baseball players, with repetitive throwing recognized as a major risk factor. Whether repetitive batting contributes to cumulative capitellar loading remains unclear. We hypothesized that repetitive batting would increase cumulative mechanical loading on the humeral capitellum, as assessed with computed tomography (CT) osteoabsorptiometry. METHODS: In this cross-sectional study, 32 healthy male collegiate baseball players (64 elbows), all right-handed throwers, underwent bilateral elbow CT during the off-season. Base

Knee Chondral Shear Injury Repair: A Biomechanical Laboratory Comparison of Suture Bridge to Chondral Pin Fixation.

BACKGROUND: Delamination of chondral fragments in pediatric patients is common and is usually caused by osteochondritis dissecans (OCD) or patellar dislocations. For fragments with minimal to no bone, fixation with screws may not be ideal due to hardware prominence on the cartilage or suboptimal screw purchase, which can result in hardware migration and loss of fixation. Lower-profile chondral fixation devices may be ideal in these circumstances, such as the suture bridge fixation construct and bioabsorbable chondral pin fixation. HYPOTHESIS: Suture bridge constructs would provide superior fi

Complete Trochlear Aplasia in the Knee-An Extreme Form of Trochlear Dysplasia: A Case Report.

CASE: A 39-year-old healthy woman without known genetic condition presented with bilateral patellar dislocations. Imaging demonstrated complete absence of the trochlear groove, defined as trochlear aplasia. Surgical treatment was initially performed on the left knee with refixation of an osteochondritis dissecans lesion, medial patellofemoral ligament reconstruction, and medializing tibial tubercle osteotomy. Intraoperative confirmation of the absence of trochlear groove made trochleoplasty unfeasible. At the 1-year follow-up, VAS was 0/10 without patellar dislocation. CONCLUSION: Trochlear a

Osteochondral autograft transfer for capitellar osteochondritis dissecans in high-level adolescent athletes.

BACKGROUND: Osteochondritis dissecans (OCD) of the capitellum is a condition that predominantly affects high-performing adolescent athletes engaged in repetitive overhead motion and can lead to pain and mechanical symptoms that can interfere with the ability to perform in sports. Optimal surgical management of large OCD lesions remains inadequately studied. Prior studies have shown good short-term outcomes with the use of osteochondral autograft transfer (OAT). This study's primary objective is to evaluate the long-term outcomes of adolescent athletes undergoing OAT procedures for capitellar O
