Histologically Confirmed Adult Tracheomalacia at Autopsy After Single-Lung Transplantation.

A man in his 50s developed persistent ventilator dependence, recurrent pneumonia, and lung abscess after right single-lung transplantation, and later died following complications from native lung pneumonectomy. Autopsy confirmed the diagnosis of tracheomalacia, allowing detailed histological examination that revealed extensive cartilage degeneration, disruption, and fibrosis of the trachea. Because histopathological studies of adult tracheomalacia are extremely rare, this case provides valuable insight into its pathology.

A novel mouse trauma model for osteonecrosis of the femoral head.

Osteonecrosis of the femoral head (ONFH) is a devastating condition characterized by the disruption of vascular supply, progressive bone degeneration, and eventual collapse of the femoral head. While corticosteroid- and alcohol-induced ONFH models are well established, a reliable trauma-induced ONFH model in mice has not been previously described. However, traumatic ONFH is commonly seen in clinical settings-such as following FN fracture, dislocation, or hip trauma-but remains poorly understood at the mechanistic level due to a lack of appropriate small-animal models. To address this gap, we e

Rhinologic Manifestations in Systemic Rheumatic Disease.

Sinonasal symptoms are a common early manifestation of several systemic inflammatory diseases and may precede multisystem involvement by days to years. Because nasal obstruction, drainage, epistaxis, crusting, and hyposmia are frequently attributed to chronic rhinosinusitis, diagnoses such as antineutrophil-cytoplasmic antibody-associated vasculitis, immunoglobulin G4-related disease, relapsing polychondritis, and sarcoidosis are often delayed. Recognition of atypical sinonasal symptoms and associated systemic findings is critical to distinguishing these entities from primary sinonasal inflamm

Sinensetin ameliorates osteoarthritis progression by modulating the NF-κB/NLRP3 signaling pathway and suppressing chondrocyte pyroptosis.

Sinensetin (SIN) is a natural flavonoid with potential anti‑inflammatory effects. This study investigated whether SIN ameliorates osteoarthritis (OA) and explored its underlying mechanisms. In vitro, SIN (0.05-1 μg/mL) protected human OA chondrocytes from IL‑1β‑induced viability loss. Transcriptome sequencing and molecular docking identified SERPINA3 as a candidate target of SIN. SIN reduced the expression of NLRP3, GSDMD, and ASC, inhibited pyroptotic morphology, decreased TUNEL‑positive cells and LDH release, and lowered IL‑1β and IL‑18 secretion in IL‑1β‑treated chondrocytes. Knockdown of S

Laryngeal Manifestations of Rheumatic Disease.

Rheumatologic diseases are systemic, immune-mediated disorders that frequently involve the larynx, producing manifestations that range from subtle dysphonia to life-threatening airway obstruction. Laryngeal involvement may precede systemic disease, progress despite adequate systemic control, or be the sole presenting feature of active disease. Diagnosis relies on flexible laryngoscopy with disease-specific serology and biopsy when indicated. Corticosteroids, immunosuppressants, and biologics such as rituximab form the foundation of medical therapy, while surgery ranges from serial endoscopic d

Multi-omics analysis unveils inflammatory and metabolic alterations between the synovium and cartilage in patients with knee osteoarthritis.

BACKGROUND: KOA (knee osteoarthritis) is a degenerative disease affecting the entire joint, involving synovial inflammation and cartilage degeneration, but their molecular interplay remains unclear. This study integrated proteomics and metabolomics to investigate cross-tissue alterations in KOA. METHODS: Paired synovial and cartilage tissues from 12 KOA patients and 12 controls were analyzed using DIA proteomics and untargeted metabolomics. The major molecular abnormalities in OA synovium and cartilage were evaluated through differential expression analysis, GO and KEGG functional enrichment

ARID5B drives an inflammatory-to-destructive shift in pathologic fibroblast behavior.

During inflammatory diseases such as rheumatoid arthritis, fibroblasts prominently drive chronic inflammation and the subsequent destruction of cartilage and bone. The mechanism by which an activated, inflammatory fibroblast acquires tissue destructive behaviors is unknown. Here, we describe ARID5B as a transcription factor that directs inflammatory fibroblasts to become migratory and invasive. Upon upregulation in inflammatory fibroblasts, ARID5B binds to histone editors and localizes to both inflammatory and invasive gene loci, epigenetically repressing pro-inflammatory genes while enhancing

From Sprain to End-Stage Osteoarthritis: Why Earlier Intervention in Ankle Cartilage Damage Deserves More Attention.

➢ Ankle cartilage damage ranges from osteochondral lesions of the talus to end-stage osteoarthritis. End-stage ankle osteoarthritis has been associated with a health burden comparable with that of major chronic diseases such as congestive heart failure and kidney disease, while predominantly affecting young and active patients.➢ Ankle osteoarthritis often has a posttraumatic nature, making it possible to study the long-term results of traumatic articular injuries and identify potential targets for early therapeutic interventions.➢ The scientific literature has been predominantly focused on out

Perlecan regulates exercise-induced PGC-1α isoform expression and angiogenic gene program in skeletal muscle.

Exercise induces diverse adaptations in skeletal muscle, and extracellular matrix remodeling plays a key role in these adaptations. Capillaries, which are essential for transporting oxygen and other molecules to maintain skeletal muscle function, are crucial for exercise adaptation. Perlecan, a heparan sulfate proteoglycan and a major basement membrane molecule in skeletal muscle, has been implicated in capillary formation and maintenance. However, its role in exercise adaptation remains unclear. Herein, we used male mice lacking perlecan expression outside of cartilage (Hspg2-/--Tg) as well a

Slipped Capital Femoral Epiphysis (SCFE).

Slipped capital femoral epiphysis (SCFE) is one of the most common pediatric hip disorders and a frequent cause of diagnostic delay due to its variable and nonspecific presentation. Children often present with groin, thigh, or knee pain, gait abnormalities, or minor trauma, which can obscure early recognition. This case report describes a 10-year-old female who presented to an urgent care setting with hip, groin, and lower extremity pain following minor trauma and was diagnosed with SCFE based on physical examination and radiographic findings. Despite conservative pain management, persistent g

Disease-first public-data integration with local virtual knockout prioritizes shared proteins linking osteoarthritis and osteoporosis.

Osteoarthritis and osteoporosis frequently coexist in older adults, but shared molecular programs remain unclear. We developed a disease-first multilayer public-data integration framework to identify shared protein candidates while preserving disease-specific structure before cross-disease comparison. Public bulk transcriptomic datasets from osteoarthritis synovium and cartilage and osteoporosis-related monocytes, femoral bone, and osteogenic stromal compartments were analyzed independently within each disease. Disease-level signatures were generated using differential expression analysis, rob

Dual-functional regulation of macrophage polarization by ROS-scavenging and chemokine-adsorbing heparin-based hydrogel microspheres for the treatment of chronic inflammatory diseases.

For chronic and progressive diseases such as osteoarthritis (OA) and diabetic wounds (DW), there are currently no effective drugs that can prevent or reverse their progression. Pathological overproduction of reactive oxygen species (ROS) and sustained inflammation are identified as key drivers of OA or DW progression. Herein, we developed a hydrogel microsphere system via cross-linking of star-shaped polyethylene glycol (star-PEG) with heparin, followed by surface functionalization with the radical scavenger Heparin @ PEG @ TEMPO (HPT) microspheres. The TEMPO-functionalized groups on the micro

An NIR-Triggered Self-Propelled Janus Nanomotor-Integrated Hydrogel with Enzymatic Cascade Reactions and Photothermal-Enhanced Drug Release for the Treatment of Gouty Arthritis.

Gouty arthritis is characterized by monosodium urate (MSU) crystal deposition, persistent joint inflammation, oxidative stress, and limited drug penetration. Here, we report an injectable hydrogel system embedding asymmetric Janus nanomotors (Col-PLGA@Au-CU) that integrates enzymatic cascade reaction, photothermal propulsion, and on-demand drug release for active gout therapy. The nanomotors consist of colchicine-loaded PLGA cores partially coated with gold nanoshells and functionalized with uricase (UOx) and catalase (CAT), which are encapsulated in a chitosan-based hydrogel for intra-articul

Stable Isotope Labeling to Track De Novo Fatty Acid Synthesis in Mouse Cartilage-Enriched Joint Tissue In Vivo: A Practical Workflow.

Cartilage is traditionally viewed as a metabolically quiescent tissue that relies primarily on exogenous lipid uptake to meet its energetic and structural demands. However, increasing evidence suggests that chondrocytes exhibit metabolically active behaviors, particularly under aging and pathological conditions. Here, we developed and applied an in vivo deuterium oxide (D2O)-based stable isotope labeling approach to directly quantify de novo fatty-acid synthesis in cartilage. Using gas chromatography-mass spectrometry (GC-MS), we tracked deuterium incorporation into palmitate in serum and arti

Multidimensional welfare profiling of broiler leg health under probiotic or antimicrobial growth-promoter supplementation and environmental enrichment.

Broiler leg health is a complex and multidimensional welfare concern that may be inadequately characterized using isolated indicators. This study developed an individual-level multidimensional welfare-response profiling framework to evaluate broilers subjected to three dietary strategies-negative control, zinc bacitracin as an antimicrobial growth promoter (AGP), or a multi-strain probiotic-combined with environmental enrichment in a 3 × 2 factorial experiment involving 1,440 male broilers housed in 48 pens. Welfare outcomes were organized into five biological domains: locomotor performance, s

Engineered Extracellular Vesicles for Cartilage Regeneration: Bridging Biological Complexity to Clinical Precision.

Osteoarthritis (OA) is a common degenerative joint disease characterized by pain, stiffness, progressive cartilage loss, and reduced mobility. Current treatments primarily aim to relieve symptoms rather than restore damaged cartilage, and durable regeneration of native hyaline cartilage remains a major clinical challenge. Extracellular Vesicles (EVs), particularly those derived from Mesenchymal Stem Cells (MSCs), have emerged as promising cell-free therapeutic platforms because of their ability to modulate inflammation, regulate chondrocyte activity, and influence extracellular matrix metaboli

Immunometabolic reprogramming in osteoporosis-osteoarthritis comorbidity: from inflammaging to osteochondral unit degeneration.

Osteoporosis (OP) and osteoarthritis (OA) are common and highly co-occurring degenerative bone diseases in the elderly population, and their comorbidity mechanisms cannot be fully explained by traditional factors such as mechanical load, changes in bone mass, or hormonal imbalance. OP is primarily characterized by systemic decrease in bone mass and reduced bone strength, while OA is mainly manifested by cartilage degeneration within specific joints, synovial inflammation, and abnormal subchondral bone remodeling. Although both are influenced by aging, inflammation, and metabolic abnormalities,
