O-Arm CT-Guided Intercostal Nerve Radiofrequency Ablation for Refractory Tietze's Syndrome: A Case Report.

BACKGROUND: Tietze's syndrome is a rare inflammatory condition of the anterior chest wall that may cause chronic localized pain resistant to conservative treatment. Surgical cartilage resection often provides only temporary relief, and recurrence of pain presents a significant therapeutic challenge. CASE PRESENTATION: We report the case of a 49-year-old female with recurrent anterior chest wall pain due to refractory Tietze's syndrome following two costal cartilage resections. Despite corticosteroid injections and physiotherapy, pain persisted with a VAS score of 7-8/10. Considering multiple

Difficulty diagnosing breast cancer recurrence in the context of a radio-frequency-identification device-containing breast implant - A case report.

Concerns around the risk of prosthetic post-mastectomy breast reconstruction obscuring breast cancer recurrence existed in the early days of breast reconstruction but were laid to rest with cohort studies. We present an unusual case where MRI images were limited in diagnosing breast cancer recurrence, partly due to an artefact produced by a radio-frequency-identification device (RFID) contained within the implant. Co-existing Tietze's syndrome, an uncommon benign inflammatory condition effecting the chest wall, further confused the picture. This case highlights the diverse pathologies that can

Chest Pain in Children: Is It Another "Growing Pain"?

Chest pain is a common complaint among children that has a non-cardiac origin in 99% of pediatric cases. We conducted a literature review of the different proposed etiologies of pediatric chest pain, as well as the evidence base supporting current approaches. Among the non-cardiac causes of chest pain in children, musculoskeletal causes are reported to be the most prevalent. This includes precordial catch syndrome, Tietze's syndrome, and costochondritis. However, these origins of musculoskeletal chest pain were described historically, and their labels are likely applied too broadly. It is impo

Detection of epileptic spasms using foundational AI and smartphone videos.

Infantile epileptic spasm syndrome (IESS) is a severe neurological disorder characterized by epileptic spasms (ES). Timely diagnosis is crucial, but it is often delayed due to symptom misidentification. Smartphone videos can aid in diagnosis, but the availability of specialist review is limited. We fine-tuned a foundational video model for ES detection using social media videos, thus addressing this clinical need and the challenge of data scarcity in rare disorders. Our model, trained on 141 children with 991 ES and 127 children without seizures, achieved high performance (area under the recei

A pictorial essay of thoracic wall diseases: multiple pathologies in the same anatomical site.

OBJECTIVES: To describe diagnostic and radiological features of the main pathologies affecting the thoracic wall, providing a pictorial atlas based on several clinical cases extracted from our archive. MATERIALS AND METHODS: A wide variety of pathologies affect the tissues of the thoracic cage; these conditions are often encountered by radiologists during examinations performed for unrelated clinical questions. Modern imaging techniques enable the detection of these pathologies and allow definitive diagnoses to be achieved. RESULTS: Pathological processes that involve the chest wall may be c

Atypical chest wall pain: paravertebral tuberculosis mimicking costochondritis.

This case describes a woman in her 30s presenting with a 2 month history of thoracic pain, initially suspected to be costochondritis. The pain was unresponsive to conservative management, and imaging revealed a paravertebral mass at the T6-T7 level. The differential diagnosis included malignancy, sarcoma and tuberculosis (TB). A positive QuantiFERON-TB Gold test suggested TB exposure, before a biopsy subsequently confirmed spinal TB. She was started on quadruple anti-tuberculosis therapy and showed significant clinical improvement. This case emphasises the importance of considering TB in the d

Feasibility and Oncological Outcomes of Sacral Resection for Locally Advanced and Recurrent Rectal Cancer : A Single Institutional Experience.

OBJECTIVE: To detect the appearance and specify the types of adverse reactions to vaccines registered in a paediatric population. PATIENTS AND METHODS: A 6-month, prospective, observational, multicentre epidemiological vaccine safety study was undertaken in 2002 covering a paediatric population subject to vaccine administration. A two-phase telephone survey of all patients was conducted, comprising an initial call at 1 week and a follow-up call at 30 days after the vaccine administration date. A paediatrician was responsible for diagnosing the specific type of adverse reaction. RESULTS: Of a

Co-occurrence of breast cancer and malignant peripheral nerve sheath tumor in a patient with neurofibromatosis type 1.

CpG 7909 [PF-3512676] is an immunomodulating synthetic oligonucleotide designed to specifically agonise the Toll-like receptor 9 (TLR9). It is being developed for the treatment of cancer [ProMune] as a monotherapy and in combination with chemotherapeutic agents, and it is also under development as an adjuvant [VaxImmune] for vaccines against cancer and infectious diseases. CpG 7909, acting through the TLR9 receptor present in B cells and plasmacytoid dendritic cells, stimulates human B-cell proliferation, enhances antigen-specific antibody production and induces interferon-alpha production, in

Spondyloarthritis and Tietze's syndrome: A re-evaluation.

Spondyloarthritis (SpA) comprises a number of related but different disorders with distinct phenotypes: psoriatic arthritis, reactive arthritis, arthritis related to inflammatory bowel disease, undifferentiated arthritis, and ankylosing spondylitis (the well-known prototypic subtype). Differentiating rheumatic diseases, such as rheumatoid arthritis, synovitis-acne-pustulosis-hyperostosis-osteitis syndrome, pustulotic arthro-osteitis, gout, and SpA, is difficult because they all may manifest swelling at the upper anterior chest wall, often involve the sternocostal and/or sternoclavicular joints

Tietze's syndrome presenting as enthesitis diagnosed by ultrasound: A case report.

Tietze's syndrome (TS) is an inflammatory disorder characterised by painful, non-suppurative swelling in the sternocostal or sternoclavicular joint. The aetiology of TS is unknown. Herein, we described a case of isolated enthesitis in the sternocostal joint in a 42-year-old male patient whose clinical course led to the diagnosis of TS. A 42-year-old male Japanese patient (HLA-B27 negative) presented with chronic anterior chest wall pain. Mild swelling and tenderness were found in the right, third sternocostal joint (rt.3STCJ). No other arthralgia was present. Ultrasonography (US) of the rt.3ST

Have female twisted-wing parasites (Insecta: Strepsiptera) evolved tolerance traits as response to traumatic penetration?

Traumatic insemination describes an unusual form of mating during which a male penetrates the body wall of its female partner to inject sperm. Females unable to prevent traumatic insemination have been predicted to develop either traits of tolerance or of resistance, both reducing the fitness costs associated with the male-inflicted injury. The evolution of tolerance traits has previously been suggested for the bed bug. Here we present data suggesting that tolerance traits also evolved in females of the twisted-wing parasite species Stylops ovinae and Xenos vesparum. Using micro-indentation ex

Slipping rib syndrome: A clinical and dynamic-sonographic entity. A serial cases report.

BACKGROUND: Slipping rib syndrome (SRS) consists of false or floating rib hypermobility, which can force the ribs to come into contact with each other. OBJECTIVE: We aimed to examine each case by dynamic ultrasound to determine their ultrasound characteristics and analyze the clinical features of patients with SRS in order to better manage and follow them up. METHODS: Retrospectively, we collected 14 case series presenting to SRS between June 2016 and September 2018. The diagnosis was clinical and confirmed by dynamic ultrasound maneuvers. RESULTS: The mean age was 35.00 ± 10.66 years and 6

Tietze's Syndrome Post-COVID-19 Infection in an Adult Patient.

Tietze's syndrome is a rare inflammatory disorder characterized by chest well swelling and inflammation of the costal cartilages. We describe a gentleman with repeated presentations to the emergency department (ED) with left-sided chest and sternoclavicular pain on a background of recent asymptomatic coronavirus disease 2019 (COVID-19) infection. He had elevated inflammatory markers and MRI subsequently confirmed the diagnosis of Tietze's syndrome. Anti-inflammatory medications and colchicine eventually led to a complete resolution of symptoms. This case highlights how Tietze's syndrome -- a d

The efficacy of oral corticosteroids for treatment of Tietze syndrome: A pragmatic randomized controlled trial.

WHAT IS KNOWN AND OBJECTIVE: Tietze syndrome is a rare form of chest wall costochondritis with joint swelling which can cause significant chest pain and decline in ability of daily activities. There is no standardized treatment protocol. The aim of this study was to assess the efficacy of adding oral steroids in addition to other non- steroidal treatment in improvement of pain and quality of life (QOL) in patients with Tietze syndrome. METHODS: Forty patients with Tietze syndrome were randomly divided into two treatment groups: (1) One week of prednisolone 40 mg daily followed by 1 week of pr
