Scintigraphic and CT findings of Tietze's syndrome: report of a case and review of the literature.

A case of Tietze's syndrome is reported. A 43-year-old woman, who had experienced right anterior chest pain and tender swelling of the right first costosternal junction for seven months, showed increased accumulation of the right first, the right fourth, and the left first costochondral junction on bone imaging. Ga-67 imaging showed increased accumulation at the right first costosternal junction. CT showed sclerosis of the sternal manubrium, partial calcification of costal cartilage, and soft tissue swelling. Biopsy of the right first costal cartilage showed chronic inflammation with fibrosis

Non-cyclical mastalgia: an improved classification and treatment.

A prospective study of 72 patients with non-cyclical breast pain was carried out. Non-cyclical breast pain was subdivided into true non-cyclical mastalgia (35 patients) and musculoskeletal pain (37 patients), the latter including Tietze's syndrome and other causes of chest wall pain. Mean age at presentation was significantly greater in the musculoskeletal group (39.3 versus 33.9 years) with a shorter mean duration of pain (14.7 versus 35.4 months) compared with the non-cyclical mastalgia group. True non-cyclical mastalgia was commonly bilateral and located within the upper outer quadrant of t

Chest pain: a rheumatologist's perspective.

Chest pain is a frequent patient complaint that requires a careful history and physical examination to determine its cause. Cardiac and esophageal causes of chest pain are common, but musculoskeletal disorders such as Tietze's syndrome, chest wall pain syndromes, fibrositis, inflammatory arthritic conditions, cervical osteoarthritis, and disease of the thoracic spine may also result in chest pain. Musculoskeletal diseases must be differentiated from other causes of chest pain, since specific treatment of these rheumatic conditions often produces good results.

Sternocostal joint swelling--clinical Tietze's syndrome. Report of sixteen cases and review of the literature.

Sixteen patients with painful tender swelling in the region of the sternocostal joint (SCJ) are reported and analysed against the background of a review of 106 previously reported patients with Tietze's syndrome. Seven patients fulfilled all the diagnostic criteria for Tietze's syndrome. The radiographic findings and/or the history of these patients suggested that local strain generated by respiration is a pathogenetic factor. Nine patients had a systemic arthritic disorder, skin disease, or psoriasis in their family. They differed from the 7 patients with local involvement and from those revi

Costal chondritis in heroin addicts: a comparative study with postsurgical chondritis.

Infection of the chondrocostal junction occurs infrequently nowadays. However, with the increasing incidence in the last years of intravenous drug addiction, more cases have been reported recently. The authors studied two groups of patients with costal chondritis, one of heroin addicts and the other of patients who had undergone thoracic surgery previously. While in the postsurgical group the patients need some kind of resection for their treatment, in the heroin addicts an early drainage is usually enough.

Tietze's syndrome--a chameleon under the thoracic abdominal pain syndrome.

Tietze's syndrome is characterized by pain due to a self-limiting, localized, non-suppurative swelling of the costochondral or sternoclavicular junction of unknown etiology. The case of a woman is presented here who was submitted to the hospital under the suspicion of a pathological fracture. After extensive investigations the diagnosis of Tietze's syndrome was made by exclusion, and the patient was successfully treated with local injection of an anaesthetic. Possible differential diagnoses of Tietze's syndrome include myocardial infarction, pneumonia, and others. This report emphasizes the im

Radiographic findings in patients with clinical Tietze syndrome.

Thirteen patients with similar painful tender swelling in the region of the sternocostal joint (SCJ) are reported. X-ray tomography revealed changes which might explain the swelling in 11 patients. Three patients had anatomical variants of the sternum. One of these and a further two patients had marginal osteophytes at the affected SCJ, as signs of osteoarthrosis. Six patients with psoriasis, pustulosis palmoplantaris, or psoriasis in the family had past or present arthritis of the involved SCJ or the manubriosternal joint. Tomography was found to be a useful confirmatory examination in patien

Reliability of computed tomography in the initial diagnosis and follow-up evaluation of Tietze's syndrome: case report with review of the literature.

The clinical entity of Tietze's syndrome is well known. It is benign in nature, self-limiting, and of unknown etiology. Computed tomography has proved most helpful in the initial evaluation of this disorder, especially as roentgenographic plain films have been essentially noncontributory in the past. A case of this disorder was followed up over the course of 2 months, and the clinical and computed tomography findings were correlated. The authors believe that computed tomography is the most exact and reliable modality for the follow-up evaluation of patients with Tietze's syndrome.

The breast pain clinic: a rational approach to classification and treatment of breast pain.

Three hundred and fifty women complaining of breast pain symptoms of sufficient severity to interfere with their normal lifestyle were reviewed in a special breast pain clinic over a 5 year period. Seventy-two patients (21%) had spontaneous resolution of breast pain and they required reassurance only before discharge. Of the remaining 278 patients, accurate classification of breast pain syndromes was achieved in 89%, the commonest syndrome being cyclical breast pain which accounted for 54% of the women followed up. The remaining womens' breast pain was classified as trigger zone (14%), continu

CT observation of rib abnormalities: spectrum of findings.

The CT studies in 63 patients in which rib abnormality was identified or excluded were retrospectively analyzed. The CT features were detailed and correlated with other available radiographic findings as well as clinical data. Contiguous spread of tumor to rib or metastasis to rib characteristically showed subtle or complete segmental lytic rib destruction. An accompanying extrapleural soft tissue mass was frequently seen with metastatic disease and myeloma. In nine patients CT showed rib destruction that had been obscured on chest radiography by heart, diaphragm, mass, or pleural effusion. Ot

Effects of human calcitonin on pain in the treatment of Tietze's syndrome.

Tietze's syndrome consists of pain, tenderness, and swelling of the costosternal or sternoclavicular joints. The etiology is unknown, although microtrauma or rare infections have been though to be responsible for the lesion. Conventional therapy consists of anti-inflammatory drugs, sometimes combined with chemotherapeutic agents and antibiotics. Surgery is usually deemed necessary for cases that do not respond to conventional therapy. Experience with the use of human calcitonin in five patients suffering from severe Tietze's syndrome who did not respond to conventional therapy is reported. Sym

Clinical experience of drug treatments for mastalgia.

Results of randomised trials and open studies in 291 patients with severe persistent breast pain in whom breast cancer had been excluded showed that drug therapy produced a good or useful result in 77% of those with cyclical mastalgia and 44% of those with non-cyclical mastalgia. In patients with cyclical mastalgia good or useful responses were obtained with danazol in 70%, with bromocriptine in 47%, and with evening-primrose oil in 45%. The equivalent response rates in patients with non-cyclical mastalgia were 31%, 20%, and 27% respectively. Progestagens were not effective in either group. Fa

Musculoskeletal chest wall pain.

The musculoskeletal structures of the thoracic wall and the neck are a relatively common source of chest pain. Pain arising from these structures is often mistaken for angina pectoris, pleurisy or other serious disorders. In this article the clinical features, pathogenesis and management of the various musculoskeletal chest wall disorders are discussed. The more common causes are costochondritis, traumatic muscle pain, trauma to the chest wall, "fibrositis" syndrome, referred pain, psychogenic regional pain syndrome, and arthritis involving articulations of the sternum, ribs and thoracic spine
