Otorhinolaryngological manifestations of systemic autoimmune and autoinflammatory diseases: a comprehensive review.

In many autoimmune and autoinflammatory diseases, the head and neck are the first sites of symptom manifestation. Recognizing these early signs is crucial to prevent diagnostic delays and fatal consequences. This review addresses: (1) specific otorhinolaryngological (ENT) manifestations indicating systemic diseases (2), evidence for differentiating these from isolated ENT disorders, and (3) the multidisciplinary role in patient management. A comprehensive narrative literature review was conducted utilizing established scientific databases, including PubMed/Medline, Scopus, Web of Science, and

Relapsing Polychondritis With Severe Airway Involvement Mimicking Refractory Asthma: A Diagnostic Challenge.

Relapsing polychondritis (RP) is a rare immune-mediated disorder that can cause severe tracheobronchial involvement and life-threatening airway compromise. We present a diagnostically challenging case of a 30-year-old non-binary patient whose progressive respiratory symptoms were initially attributed to severe refractory asthma. Despite repeated courses of systemic corticosteroids and escalation of asthma-directed therapy, they developed recurrent breathlessness, wheeze, and inspiratory stridor. Pulmonary function testing demonstrated severe fixed airflow obstruction with minimal bronchodilato

Imaging in Rheumatic Diseases of the Head and Neck.

Multimodality imaging is essential for evaluating head and neck rheumatologic disease. Computed tomography (CT) can demonstrate osseous erosions, mineralization, and structural complications, including temporomandibular joint and cervical spine involvement in JIA/RA, crowned dens in pseudogout, and sinonasal or airway disease in giant cell arteritis (GCA) and relapsing polychondritis (RP). Ultrasound is useful for detecting synovitis in JIA/RA, auricular chondritis in RP, salivary gland abnormalities in Sjögren's syndrome, and vascular changes in GCA. FDG-PET/CT can identify metabolically acti

Rhinologic Manifestations in Systemic Rheumatic Disease.

Sinonasal symptoms are a common early manifestation of several systemic inflammatory diseases and may precede multisystem involvement by days to years. Because nasal obstruction, drainage, epistaxis, crusting, and hyposmia are frequently attributed to chronic rhinosinusitis, diagnoses such as antineutrophil-cytoplasmic antibody-associated vasculitis, immunoglobulin G4-related disease, relapsing polychondritis, and sarcoidosis are often delayed. Recognition of atypical sinonasal symptoms and associated systemic findings is critical to distinguishing these entities from primary sinonasal inflamm

Laryngeal Manifestations of Rheumatic Disease.

Rheumatologic diseases are systemic, immune-mediated disorders that frequently involve the larynx, producing manifestations that range from subtle dysphonia to life-threatening airway obstruction. Laryngeal involvement may precede systemic disease, progress despite adequate systemic control, or be the sole presenting feature of active disease. Diagnosis relies on flexible laryngoscopy with disease-specific serology and biopsy when indicated. Corticosteroids, immunosuppressants, and biologics such as rituximab form the foundation of medical therapy, while surgery ranges from serial endoscopic d

Performance Characteristics of Anti-Collagen II Antibodies in Relapsing Polychondritis and Related Diseases: Prospective Analysis, Systematic Review, and Meta-Analysis.

OBJECTIVE: Relapsing polychondritis (RP) is a rare disease defined by recurrent cartilaginous inflammation. Anti-collagen II (Col2) antibodies have been proposed as a diagnostic biomarker for RP, but their performance characteristics are not well defined. METHODS: In an observational cohort, anti-Col2 antibody levels were measured in patients with RP compared to other inflammatory diseases that mimic RP. In parallel, a systematic review and meta-analysis was performed to assess anti-Col2 antibody prevalence across a broad spectrum of diseases. Individual study risk ratios (RRs) and pooled dis

Patient partnership model in rare and complex rheumatological conditions: research and beyond in European Reference Network ReCONNET.

In rare and complex connective tissue diseases, patient partnership is essential to address diagnostic delays, fragmented care, unmet needs, and the research agenda. European Reference Network (ERN) ReCONNET, the network dedicated to rare and complex connective tissue diseases, has implemented a structured and transferable model of patient partnership. Patients contribute to every phase of research and care development: from identifying unmet needs to co-authoring scientific publications. Patient input also shapes educational initiatives and strategic planning. By institutionalising partnershi

VEXAS syndrome and cancer: Insights about a possible "Tip of the Iceberg". Ambidirectional data from the international AIDA network registries.

BACKGROUND: VEXAS (Vacuoles, E1 enzyme, X-linked, Autoinflammatory, Somatic) syndrome is an acquired autoinflammatory disorder characterized by severe chronic inflammation and an increased occurrence of hematologic neoplasms. Although chronic inflammation is a well-established risk factor for cancer, the specific contribution of UBA1 gene mutations to tumorigenesis remains unclear. Therefore, this study aimed to evaluate the overall cancer risk in patients with VEXAS syndrome, including both hematologic and non-hematologic neoplasms. METHODS: The relative risk (RR) of cancer was compared betw

miR-34c-3p promotes osteogenic differentiation of BMSCs by inhibiting DCBLD2 activation of the PI3K/AKT signaling pathway.

In the process of bone metabolism, osteogenic differentiation of bone marrow mesenchymal stem cells (BMSCs) is closely related to bone formation, remodeling and repair. As a type I transmembrane protein, DCBLD2 regulates relapsing polychondritis, but its function in the osteogenic differentiation of BMSCs remains unclear. This study aimed to investigate the effects of DCBLD2 on osteogenic differentiation in BMSCs and the underlying regulatory mechanism. Human mesenchymal stem cell osteogenic differentiation medium was used to induce the differentiation of BMSCs into osteoblasts. Osteogenic dif

Dapsone Toxicity in Relapsing Polychondritis: Methemoglobinemia and Concomitant Hemolytic Anemia.

Relapsing polychondritis (RP) is a rare immune-mediated disorder characterized by episodic inflammation of cartilage and other proteoglycan-rich tissues. Dapsone is sometimes used as a steroid-sparing agent in selected phenotypes, but its oxidative metabolites can cause clinically significant methemoglobinemia and hemolysis. We describe a 70-year-old man with auricular chondritis and biopsy-supported RP who developed progressive dyspnea and cyanosis after three months of dapsone 100 mg daily. Oxygen saturation remained low despite supplemental oxygen, while arterial oxygen tension was preserve

Identifying quality of life domains and facets affected in relapsing polychondritis: a qualitative analysis for the development of a disease-specific health-related quality of life instrument.

INTRODUCTION: Relapsing polychondritis (RP) is a rare inflammatory disease characterized by recurrent inflammation of cartilaginous structures and a broad spectrum of systemic features. To date, no Health-related Quality of life (QoL) data are available in RP, and generic measures may not fully capture the unique burden faced by patients, given the specific nature of RP symptoms. This study aimed to identify QoL facets relevant for further inclusion in a disease-specific QoL instrument, the RP-QoL. METHODS: An international study was conducted within the framework of the European Reference Net
