Biomechanical and finite element analysis of a novel annular multi-axial locking plate for patellar fracture fixation.

BACKGROUND: Traditional tension band structures for patellar fractures have multiple complications such as wires lossening and soft tissue irritation. In response to the above problems, a new type of annular multi-axis locking plate has been designed for the treatment of patellar fractures. This study aims to compare the biomechanical performance of two methods for stabilizing patellar fractures. METHODS: A three-dimensional finite element model was established to evaluate the biomechanical performance of annular multi-axis locking steel plate fixation for patellar fractures at knee flexion a

Anterior Spinal Artery Aneurysms Associated with Coarctation of the Aorta: A Focused Narrative Review with an Illustrative Case.

Anterior spinal artery (ASA) aneurysms are rare causes of subarachnoid hemorrhage (SAH), and coarctation of the aorta (CoA) may recruit the spinal arterial circulation into a high-flow collateral network. We report the case of a 58-year-old woman who was resuscitated after pulseless electrical activity arrest. Noncontrast CT demonstrated SAH concentrated in the cervical spinal canal with limited intracranial extension. Cervically extended CT angiography identified a C5-6 ventral spinal aneurysm, severe thoracic CoA with extensive collateralization, and diffuse left subclavian artery hypoplasia

Synthetic MRI from radiotherapy planning CT images for Head-and-Neck Cancer.

BACKGROUND: Computed Tomography (CT) provides geometric accuracy and electron density information essential for radiotherapy planning; however, it offers limited soft-tissue contrast. Magnetic Resonance Imaging (MRI) improves tumor visualization but introduces registration uncertainties when acquired separately from the radiotherapy planning CT (pCT). Cross-modality image synthesis enables the generation of synthetic MRI (sMRI) directly from pCT and may combine the strengths of both modalities, reducing reliance on MRI access. PURPOSE: We propose a two-stage deep-learning framework for genera

Achondroplasia: Current Disease-Modifying Therapies and Genome Editing Strategies.

Achondroplasia (ACH) is the most common non-lethal skeletal dysplasia and is caused in the vast majority of cases by a heterozygous gain-of-function variant in fibroblast growth factor receptor 3 (FGFR3, p.Gly380Arg). Hyperactive FGFR3 signaling constrains endochondral bone growth by suppressing growth-plate chondrocyte proliferation and hypertrophic differentiation. In the past decade, management has expanded beyond supportive care, and several mechanism-based interventions have shown disease-modifying effects in clinical studies. These include the activation of the NPR2/CNP axis with vosorit

Integrated stress signaling pathways promote adaptive reprogramming for chondrocyte survival in Metaphyseal Chondrodysplasia Type Schmid.

Collagen X, encoded by the Col10a1 gene, is expressed specifically by hypertrophic chondrocytes (HCs) in the cartilage. Mutations of the Col10a1 gene in humans, which increase the endoplasmic reticulum (ER) stress in HCs, lead to metaphyseal chondrodysplasia type Schmid (MCDS). However, the mechanism of how HCs respond to ER stress to affect bone development and growth is not entirely understood. In this study, we generated a new mouse model harboring a 13 bp deletion (13del) in the endogenous Col10a1 gene. These 13del-MCDS mice display all the known clinical characteristics of human MCDS, as

Efficient Generation of Definitive-like Hematopoietic Stem and Progenitor Cells from Human Pluripotent Stem Cells through Optimized Lateral Plate Mesoderm Induction and a Supportive Endothelial-Stromal Microenvironment.

The generation of hematopoietic stem and progenitor cells (HSPCs) with definitive features from human pluripotent stem cells (hPSCs) requires coordinated lineage specification together with a supportive hematopoietic microenvironment. Although 3D and aggregate-based systems can recapitulate endothelial and stromal niche features, their self-organizing architecture can introduce structural heterogeneity and batch-to-batch variability, complicating experimental standardization and process control. Here, we developed a chemically defined, feeder-free, lateral plate mesoderm (LPM)-directed 2D mult

Simple Bone Cyst: A Report of a Rare Case.

A simple bone cyst is an uncommon non-neoplastic intraosseous pseudocyst in the human jawbone. Because it lacks an epithelial lining, we refer to such a cyst as a pseudocyst. For simple bone cysts, several other names exist, such as traumatic bone cyst, solitary bone cyst, progressive bone cavity, hemorrhagic bone cyst, unicameral bone cyst, and extravasation cyst. Usually, routine radiographic examinations discover these cysts, which otherwise are asymptomatic. Unicameral bone cysts tend to gradually increase in size and enlarge along the long axis of the mandibular alveolar bone, causing no

Proximal Scaphoid Pole Reconstruction Utilizing Ipsilateral Proximal Hamate Autograft.

BACKGROUND: Proximal pole scaphoid fractures complicated by nonunion and osteonecrosis present a challenging scenario with few good reconstructive options, especially when the proximal fragment is small or fragmented1-4. The use of an ipsilateral proximal hamate osteochondral autograft has emerged as a promising technique for reconstructing the proximal scaphoid pole, with the potential to restore carpal mechanics while avoiding the morbidity associated with free vascularized bone grafts or distant autograft sites. DESCRIPTION: The procedure involves replacing the diseased proximal scaphoid w

Thoracolumbar Fascia Remodeling in Lumbar Spinal Stenosis: Translational Orthopedic Perspectives.

Lumbar spinal stenosis (LSS) is a major cause of pain and disability, yet its pathophysiology remains incompletely understood. Although spinal decompression is the standard surgical treatment for LSS with disabling pain or with progressive neurological symptoms, clinical outcomes are often suboptimal, with reoperation required in 5-23% of cases. While degenerative changes in the vertebral canal have been extensively investigated, the potential contribution of the thoracolumbar fascia (TLF) has never been explored at the molecular level. We hypothesized that TLF undergoes extracellular matrix r

Expert Consensus on Robot-Assisted Vascularized Bone Grafting for the Treatment of Early-Stage Osteonecrosis of the Femoral Head.

Osteonecrosis of the femoral head (ONFH) remains a progressive clinical challenge that predominantly affects young and active adults between 20 and 50 years of age. Without timely and effective surgical intervention, natural disease progression leads to structural subchondral fracture, articular surface collapse, and secondary hip osteoarthritis, ultimately necessitating total hip arthroplasty (THA). Given the elevated lifetime revision rates of prosthetic replacements in young patients, joint-preserving strategies are prioritized during the precollapse window [primarily Association Research C
