Mechanobiological Modeling of Lumbar Interbody Fusion: Sensitivity Analyses on Cell-Specific Parameters, Loading Conditions, and Cage Material.

Despite decades of clinical experience, lumbar interbody fusion (LIF) still leads to pseudoarthrosis in 5%-28% of patients. Although tissue-differentiation algorithms offer a powerful predictive framework, their application to spinal fusion modeling is currently limited. In this study, we present one of the first mechano-regulated tissue differentiation algorithms incorporating cell-phenotype-specific activities, with the goal to simulate the fusion process after posterior-LIF. The influence of the model input parameters (n = 23) was first assessed in 2D through fractional factorial analyses t

Bilateral lower limb dysfunction due to acute hematoma in the lumbar ligamentum flavum: A case report and literature review.

RATIONALE: There have been several reports of lumbar yellow ligament hematomas leading to cauda equina and/or nerve root compression. However, based on clinical history and histological findings, most of these cases involve chronic hematomas. Acute lumbar yellow ligament hematomas causing nerve compression are relatively rare. PATIENT CONCERNS: A 71-year-old male presented with an 8-day history of sudden-onset lower back pain without an identifiable trigger, which progressively worsened with new-onset pain and numbness in both lower extremities. Physical examination revealed muscle strength o

Clinical outcomes of tibiotalocalcaneal arthrodesis using a retrograde femoral nail versus a dedicated hindfoot fusion nail in charcot neuroarthropathy: A prospective observational study in a resource-limited setting.

BACKGROUND: Dedicated hindfoot fusion nail (HFN) systems for Charcot tibiotalocalcaneal (TTC) arthrodesis may be unavailable or unaffordable in resource-limited settings. A retrograde femoral nail (RFN) can serve as a pragmatic contingency implant when a dedicated HFN is unavailable. This study compared functional, radiographic, union, complication, and implant acquisition-cost outcomes between RFN and HFN. METHODS: A prospective, non-randomized, two-arm observational study was conducted in 60 patients undergoing TTC arthrodesis for Charcot neuroarthropathy, consecutively allocated to RFN (n

[Application of sample-efficient neural network in predicting postoperative facial profiles in skeletal class Ⅱ orthognathic patients].

OBJECTIVE: Soft tissue prediction is of great importance for the surgical design and patients ' satisfaction in orthognathic surgery. Previous studies either rely on elastic mechanical models, such as finite element which involves time-consuming computations, or using artificial intelligence method which require insatiable large patient data. Therefore, in this study, we proposed a more efficient and lightweight method for facial morphology prediction and validated its accuracy. METHODS: A total of 85 skeletal class Ⅱ patients who underwent double jaw surgery performed by the same senior surg

Successful Conservative Treatment for Vancouver Type B3 Periprosthetic Femoral Fracture after Total Hip Arthroplasty: A Case Report.

Periprosthetic femoral fractures after total hip arthroplasty (THA) are increasing, though Vancouver type B3 fractures-characterized by a loose femoral stem and poor bone stock-are generally considered unsuitable for conservative treatment. We report a rare case of successful conservative management in a 79‑year‑old woman who sustained a Vancouver type B3 fracture 12 years after conversion of hip arthrodesis to THA. Severe cardiac dysfunction (ejection fraction 30%) precluded surgical intervention, and long‑term non‑weight bearing with physiotherapy was adopted. After 20 months, the patient wi

Polydeoxyribonucleotide Promotes Longitudinal Bone Growth Through the Hepatic JAK2-STAT5/IGF-1 Axis Involving A2A Receptor Signaling.

Longitudinal bone growth is primarily regulated by the growth hormone (GH)-insulin-like growth factor-1 (IGF-1) axis. However, non-hormonal and orally administered strategies for promoting longitudinal growth remain limited. Here, we investigated the effects and mechanisms of polydeoxyribonucleotide (PDRN) on longitudinal bone growth in vitro and in vivo. In HepG2 cells, PDRN significantly increased IGF-1 mRNA expression and JAK2 and STAT5 phosphorylation without affecting cell viability. Pretreatment with ZM241385, a selective adenosine A2A receptor (A2AR) antagonist, significantly attenuated

Managing Severe Proximal Femoral Fibrous Dysplasia in a Young Adult Using Hemiarthroplasty in a Resource-Constrained Healthcare System: A Case Report.

Fibrous dysplasia of the proximal femur may lead to progressive deformity, pain, and pathological fractures, with shepherd's crook deformity representing one of its most severe manifestations. Although valgus osteotomy with intramedullary fixation is considered the preferred surgical treatment, extensive bone destruction and limited healthcare resources may preclude conventional reconstruction. We present a case of severe proximal femoral fibrous dysplasia successfully managed with bipolar cemented hemiarthroplasty in a resource-constrained healthcare setting. A 25-year-old man presented with

Determinants of Thoracic Reciprocal Change and Its Relationship to Proximal Junctional Kyphosis Following Lower Thoracic Fusion for Lumbar Flatback Deformity.

BACKGROUND: Lumbar flatback deformity (LFD) is characterized by the loss of lumbar lordosis (LL) and global sagittal malalignment, often necessitating thoracolumbar corrective surgery. Lumbar correction induces adaptive changes in unfused thoracic segments, known as thoracic reciprocal change (RC); however, its independent clinical significance remains debatable. This study aims to identify factors associated with thoracic RC after lower thoracic fusion for LFD and to evaluate its independent effect on the development of proximal junctional kyphosis (PJK). METHODS: This retrospective study in

Emerging NPR-B agonists for cartilage and bone: from CNP biology to translational compound design.

Disorders affecting cartilage and bone development frequently originate from disrupted endochondral ossification within growth-plate cartilage. Nonetheless, few therapeutic interventions directly target the local signaling pathways governing this process. This review explores the C-type natriuretic peptide (CNP)/natriuretic peptide receptor B (NPR-B) axis as a mechanism-based therapeutic approach addressing issues in cartilage and bone, including skeletal dysplasias, genetic short-stature conditions, and acquired ailments such as bone fractures. It summarizes the production and processing of C

The Kerboull cross-plate: from the birth of a concept to the current indications.

PURPOSE: Reconstruction of major acetabular bone loss remains a demanding challenge during revision total hip arthroplasty. Since its original description, the Kerboull acetabular reinforcement device, also known as the Kerboull cross-plate or cage, has been developed to restore the native hip centre of rotation, to protect bone graft from early mechanical overload, and to allow cementation of a conventional full-polyethylene or a dual mobility cup. METHODS: This narrative review traces the evolution of the Kerboull cross-plate from its original conception to its current indications. RESULTS

GLP-1 receptor agonists in elective spine surgery: a systematic review of fusion outcomes, perioperative safety, and procedure-specific risk.

BACKGROUND: Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) are increasingly used in patients with obesity and type 2 diabetes mellitus undergoing elective spine surgery. Although these agents may improve metabolic risk profiles, their associations with spinal fusion, perioperative complications, and procedure-specific outcomes remain incompletely defined. METHODS: We conducted a systematic review and qualitative synthesis of comparative clinical studies evaluating adults undergoing spine surgery with documented preoperative or perioperative GLP-1 RA exposure. Rare Subungual Fibroepithelial Polyp With Keratosis: Differential Diagnosis.

Fibroepithelial polyps (FEPs), commonly known as acrochordons or skin tags, are benign cutaneous lesions typically found in intertriginous areas, such as the neck, axilla, and groin. Occurrence within the nail unit is exceptionally rare, presenting a diagnostic challenge as it mimics more common ungual tumors. We report a rare case of a 23-year-old woman who presented with a 4-year history of a painful, slow-growing lesion emerging from the proximal nail fold of the right fifth toe, causing nail plate deformity. Clinical differentials included acquired ungual fibrokeratoma and Koenen tumors. T

Case Series: MILD® Procedure as a Minimally Invasive Option for Lumbar Synovial Cyst Decompression.

BACKGROUND: Lumbar synovial cysts are benign, fluid-filled lesions arising from the facet joints, often associated with degenerative changes and segmental instability. These cysts can cause neurogenic claudication and radiculopathy, significantly limiting mobility and quality of life. While traditional treatments include conservative management, injections, aspiration, and open surgery, each has notable limitations. The Minimally Invasive Lumbar Decompression (MILD®) procedure, originally designed for lumbar stenosis, may offer a less invasive alternative for synovial cyst resection, especiall

Biomechanical evaluation of a medial-only fixation strategy for Takeuchi type 2 lateral hinge fractures in medial open wedge high tibial osteotomy.

BACKGROUND: Medial opening wedge high tibial osteotomy (MOWHTO) is a standard treatment for knee osteoarthritis. However, lateral hinge fractures occur in 19-25% of cases, with Type 2 fractures causing significant instability. While lateral plating is recommended, it requires an additional incision. This study investigated whether medial-only fixation using a 4.5 mm cortical screw inserted into the Tomofix plate's oval hole could provide sufficient stability for Type 2 hinge fractures. METHODS: Twelve fresh pig knees underwent MOWHTO with a 6 mm opening. They were divided into two groups: Gro

An NKX2-3/5/6 homolog is required downstream of BMP signaling to pattern the sensory-adhesive organ of a tunicate larva.

Tunicates are the sister group to vertebrates within the chordate phylum, yet unlike other chordate groups, they evolved a biphasic lifecycle alternating between motile larvae and sessile adults. The papillae of most tunicate larvae are the key sensory-adhesive organ regulating their settlement and metamorphosis. The papillae are nearly always arranged as a group of three morphologically identical organs that arise from an anterior neural plate border region nested between ventral epidermis and more dorsal/posterior neural tube progenitors. Due to their embryonic origin and molecular signature

Stepwise supine anterolateral approach for posterolateral tibial plateau fracture with intact posterior cortical rim: a case report.

Posterolateral tibial plateau fractures are technically demanding due to limited exposure imposed by the fibular head and posterolateral corner structures. The standard anterolateral approach visualizes only 36.6% of the posterolateral articular surface, contributing to malreduction rates reaching 32.3%. We report a 26-year-old female who sustained a Kfuri-Schatzker type 2 fracture with combined anterolateral and posterolateral involvement, characterized by depression of the posterior three-quarters of the lateral plateau with an intact posterior cortical rim. This morphology rendered posterio
