This case illustrates the complex challenges of managing forearm trauma complicated by bone loss after implant removal. A long-standing radial shaft plate, integrated into cortical bone with overgrowth, had to be removed to treat a new peri-prosthetic fracture. Removal caused massive cortical loss, leaving poor bone stock for fixation. Dual plating was performed to bridge the defect and stabilize the distal radius fracture, but mechanical failure prompted a staged revision. A deliberately delayed second operation allowed time for biological preparation. Definitive reconstruction included corre

INTRODUCTION: Lumbar spinal fusion (LSF) is a common surgical treatment for chronic low back pain (CLBP), yet evidence of its effectiveness compared with conservative care remains inconclusive. While pain is the primary clinical indication, psychosocial factors, including hopes and expectations, may influence outcomes. However, little is known about how individuals experience life with CLBP before surgery or what they hope to regain after LSF.
OBJECTIVE: To explore patients' experiences with CLBP prior to LSF and their hopes and expectations for life after surgery.
METHODS: A qualitative, ex

OBJECTIVE: Endoscopic lumbar discectomy (ELD) has emerged as a minimally invasive alternative to open lumbar discectomy (OLD), with advantages including reduced tissue disruption and amenability to outpatient settings. Comparative data on clinical outcomes and costs across care settings remain limited. This study leveraged three national databases, TriNetX, National Inpatient Sample (NIS), and Nationwide Ambulatory Surgery Sample (NASS), to analyze short- and long-term outcomes and inflation-adjusted healthcare costs between ELD and OLD.
METHODS: A retrospective cohort study was performed usi

OBJECTIVE: Traditional management of symptomatic lumbar spondylolisthesis (SPL) involves interbody fusion (IBF). However, endoscopic unilateral laminectomy with bilateral decompression (E-ULBD) has emerged as a less invasive alternative with growing evidence of effectiveness. The authors aimed to comprehensively compare outcomes between E-ULBD and IBF in grade 1 lumbar degenerative SPL.
METHODS: The data of patients who underwent E-ULBD or IBF for symptomatic lumbar SPL by a single surgeon during the same period were retrospectively reviewed. Demographics, SPL characteristics, surgical profil

Introduction describes first metatarsophalangeal (MTP) joint arthrodesis as a historical and currently powerful option for severe forefoot deformities, including arthritis, hallux valgus, hallux varus, and trauma. Shortening of the first ray, particularly after MIS bunion procedures and revisional surgeries, creates biomechanics challenges that can compromise fusion. To address large segmental defects, the senior author advocates interpositional femoral head allograft to restore first-ray length and provide structural support for stable MTP joint arthrodesis. The approach avoids autograft draw

Isolated subtalar arthrodesis is a common surgical procedure used to treat subtalar joint pathologies including primary and post-traumatic arthritis, calcaneal fractures, tarsal coalitions, and posterior tibial tendon dysfunction. Its goals are pain relief, rearfoot realignment, and improved stability while preserving more motion than triple arthrodesis. This review covers the relevant anatomy of the subtalar joint and its facets, clinical examination findings, and imaging modalities. It details surgical techniques for native fusions, neglected calcaneal fractures, and rigid valgus deformities

The "Step Diet" is a physician-designed activity management protocol for patients with foot pathology or prior reconstructive surgery who experience persistent pain from overuse. Rather than pursuing the mythologized 10,000 step daily goal-rooted in 1960s Japanese marketing, not medical evidence-patients track daily steps and record pain levels on a 1 to 10 scale over 4 weeks. A personalized safe step threshold is then identified, with two permitted "cheat days" weekly. This structured approach enables patients to budget around priorities, predict discomfort, and plan recovery periods. Clinica

Post-vertebral augmentation infection is uncommon but clinically serious, especially in older patients with multiple comorbidities and fragile spinal bone stock. Revision surgery is challenging because surgeons must eradicate infection, decompress neural structures when needed, restore stability, and decide whether polymethylmethacrylate cement can be safely preserved. This article describes a posterior-only protocol for diagnosing and treating post-vertebral augmentation infection and illustrates the workflow with a retrospective representative cohort treated between January 2015 and December

The Wiltse transforaminal lumbar interbody fusion (W-TLIF) modifies the traditional open posterior lumbar fusion to treat the same degenerative pathologies through a smaller, muscle-sparing incision that accesses the lumbar spine through the paramedian corridor between the multifidus and longissimus muscles, preserving the posterior musculature and limiting iatrogenic soft tissue trauma. This protocol provides a reproducible, step-by-step guide to performing the muscle-sparing W-TLIF for lumbar decompression and interbody fusion. After prone positioning and fluoroscopic level localization, a p

AIM: Finite element analysis (FEA) is widely used in implant prosthodontics to assess biomechanical behavior and support treatment planning in complex edentulous cases. This work aimed to evaluate stress distribution patterns associated with three implant configurations (all-on-four, all-on-six, and quad-zygomatic) in the edentulous maxilla using a single patient-specific finite element model under immediate loading conditions.
SETTINGS AND DESIGN: Two CBCT scans from a single edentulous patient, acquired before and after full-arch bone grafting, were segmented to generate three digital twin

PURPOSE: Focal chondral lesions of the wrist are a frequent cause of pain and, despite their high incidence, there are no well-defined criteria for their treatment. For advanced focal chondral lesions (Outerbridge IV), treatment remains challenging. Although salvage procedures, such as arthrodesis provide reliable pain relief, joint-preserving techniques, have gained increasing interest in carefully selected patients because they may preserve wrist motion while delaying irreversible salvage surgery.
METHODS: We describe the surgical technique and the clinical and functional outcomes of a pros

RATIONALE: Adrenal insufficiency (AI) is a rare but potentially life-threatening cause of perioperative hemodynamic instability characterized by an inadequate cortisol response to physiological stress.
PATIENT CONCERNS: We report a case of a 77-year-old woman undergoing oblique lumbar interbody fusion who developed profound intraoperative hypotension refractory to fluid resuscitation and multiple vasopressors. Notably, she had a history of corticosteroid therapy for rheumatoid arthritis that had been discontinued nearly 3 decades earlier. She also exhibited extreme skin fragility.

Pathological fractures due to metastatic bone disease frequently require palliative stabilization. While polymethylmethacrylate (PMMA) provides immediate mechanical stabilization, its limited bending and multiaxial properties raise concerns. Reinforcement strategies such as plate fixation, intramedullary nails, and metallic augmentation have been proposed, yet their mechanical behavior under physiological loading remains insufficiently characterized. This study aims to evaluate the mechanical performance of three reconstruction strategies for a proximal radial defect: PMMA alone, PMMA combined

BACKGROUND: Sacroiliac (SI) joint dysfunction is a common source of chronic low back pain. Minimally invasive SI joint fusion techniques, including posterior interpositional and lateral transiliac approaches, have emerged as treatment options for patients who fail conservative management; however, comparative real-world outcome data, particularly from procedures performed by interventional pain physicians, are limited.
OBJECTIVES: We aimed to evaluate short-term outcomes, complication rates, and revision rates of posterior interpositional vs lateral transiliac SI joint fusion performed by int

BACKGROUND: Sexual dysfunction represents a meaningful yet understudied complication of spine surgery, with substantial implications for quality of life. Differences in surgical exposure between anterior and posterior approaches may influence these outcomes through variation in anatomic disruption, neural risk, and perioperative course. While existing studies have reported select postoperative sexual complications in broad patient populations, comparative data specifically assessing dysfunction by operative approach among men undergoing single-level lumbar fusion remain sparse. This study comp

BACKGROUND: Medial malleolar stress fractures (MMSFs) are uncommon but potentially high-risk injuries in athletes. In basketball players, repetitive jumping, landing, and cutting may expose the characteristic vertical fracture pattern to substantial shear, yet sport-specific operative data remain limited. This study reports the outcomes of plate-based fixation for MMSFs in high-level basketball players.
METHODS: A retrospective case series was performed of 10 high-level basketball players treated surgically for MMSF between 2007 and 2025. All patients underwent radiography, magnetic resonance
