Cartilaginous Pushdown Nasal Hump Treatment.

The reduction of the nasal hump can be based on the partial resection of the cartilages and bones or on the preservation of these structures. The nasal dorsum preservation techniques, like the push down and let down, minimize problems like open roof and inverted V and lessens the necessity of grafts, the mainstays of the structured rhinoplasty. The cartilaginous pushdown preserves the middle third of the nose and let the bony portion to be treated independently. This approach expands greatly the indications of preservation techniques as in complex deviated noses, S shaped humps, and for patien

The Crooked Nose and Major Septal Deviations in Dorsal Preservation.

The crooked nose deformity (CND) has a multifactorial origin. Trauma, previous surgery, septal deviation, and maxillary and nasal bone hypertrophy contribute to asymmetry. The crooked nose has been deemed one of the most difficult to repair of all nasal deformities. Dorsal preservation (DP) rhinoplasty techniques have evolved to correct the boney vault, high septal deviations, and boney hypertrophy while limiting cartilage usage. A paucity of literature exists detailing functional and aesthetic outcome measures for DP. Herein the author's approach to improve the CND utilizing different DP tech

The Functional and Aesthetic Results of Fibrin Glue, Platelet-Rich Plasma (PRP), and Warm Blood as a Scaffold for Diced Cartilage in Saddle Nose Deformity: A Descriptive Study.

Irregularities on the nasal dorsum post rhinoplasty remain a prevalent concern for both surgeons and patients. Various techniques using cartilage are used to hide the nasal dorsum, such as crushed cartilage, free diced cartilage, surgical-wrapped diced cartilage, or diced cartilage mixed with blood glue. Diced cartilage can address issues that arise with solid materials. However, it may lead to undesirable outcomes such as graft deformation and migration. It is challenging to uniformly implant the loosely linked cartilage pieces. To prevent issues, cartilage is mixed with other materials to se

The Single Application of External V-Y Plasty in Correcting Alar Retraction.

With the rising incidence of basal cell carcinoma, postoperative deformities following Mohs surgery have become increasingly prevalent. Among these, alar retraction represents a particularly challenging complication. Current approaches to its correction have largely centered on grafting techniques, which are often complex and may necessitate multidisciplinary collaboration. External V-Y plasty, by facilitating soft tissue lengthening and tension release, presents a straightforward and effective alternatives for correcting alar retraction, offering favorable aesthetic outcomes.

Reconstruction of nasal tip using auricular cartilage to prevent depression following midline partial maxillectomy: A report of two cases.

Midline partial maxillectomy involving anterior nasal spine and septum in patients with maxillary carcinoma can result in postoperative depression of the nasal column and apex, even when a maxillary prosthesis is used. We encountered patients who developed significant nasal column and tip depression following surgery. To address this issue, we performed nasal septum reconstruction using auricular cartilage. Auricular cartilage was harvested bilaterally, and two grafts were stacked and transplanted into the nasal septal defect. After surgery, the transplanted cartilage became covered with mucos

Impact of Septal Extension Graft on Functional and Esthetic Outcomes in Patients with Severe Nasal Deviation: A Systematic Review and Meta-analysis.

BACKGROUND: Severe nasal deviation is a complex deformity affecting the esthetics and functionality of the nose, creating an asymmetrical appearance due to septal misalignment. Advances in nasal anatomy and biomechanics have led to the use of grafting materials, such as septal extension grafts. However, its efficacy remains inconclusive. This study investigated the impact of septal extension grafts on the functional and esthetic outcomes in severe nasal deviation. METHODS: A literature search was conducted using PubMed, Google Scholar, MEDLINE, Web of Science, and the Cochrane Central Registe

Idiopathic Subglottic Stenosis and the Epithelial Interface of Host and Environment.

BACKGROUND: Idiopathic subglottic stenosis (iSGS) is a rare fibrotic disease of the proximal airway affecting adult White women nearly exclusively. Life-threatening ventilatory obstruction occurs secondary to pernicious subglottic mucosal scar. Disease rarity and wide geographic patient distribution have previously limited substantive mechanistic investigation into iSGS pathogenesis. STUDY DESIGN: Harnessing pathogenic mucosa from an international iSGS patient cohort and single-cell RNA sequencing, we provide an unbiased characterization of the cell subsets present in the proximal airway scar

Cam Morphology and Sex-Based Differences in the Proximal Femur Anatomy of Collegiate Athletes Without Hip Pain: A 3-Dimensional Statistical Shape Modeling Analysis.

BACKGROUND: Radiographic studies have reported a high prevalence of cam morphology in athletes, especially in male athletes, suggesting these individuals are at an elevated risk of developing femoroacetabular impingement syndrome (FAIS). However, recent research has shown that 2-dimensional measurements do not accurately characterize cam deformities, motivating the need for 3-dimensional (3D) analyses. PURPOSE: To develop a 3D statistical shape model of the proximal femur to evaluate cam morphology in collegiate athletes through (1) quantifying shape variation, (2) establishing sex-based shap

Rotational Columellar Skin Flap for Nasal Alar Rim Reconstruction.

Nasal alar reconstruction is complex due to the region's anatomy and aesthetic importance. This report describes repairing a small, full-thickness alar rim defect in a 36-year-old man using a rotational columellar skin flap with septal cartilage grafting. This single-stage technique achieved good color match, symmetry, and minimal donor-site morbidity. Compared to multi-stage flaps or composite grafts, this method is less invasive and effective for small defects, offering reliable aesthetic and functional results in carefully selected cases.Level of Evidence V This journal requires that author

Extensor mechanism dysfunction and hand deformities caused by Dupuytren's disease: Surgical and rehabilitation perspectives.

Dupuytren's disease is a common fibroproliferative disorder that primarily affects the palm of the hand. While the disease is known for its characteristic palmar contractures, it also has a potential impact on the extensor mechanism of the hand, resulting in the development of boutonniere deformity, swan-neck deformity, and persistent metacarpophalangeal joint contracture due to extensor capsule stretching and tendon instability. These imbalances are challenging to correct under general or regional anesthesia. Wide awake local anesthesia without a tourniquet allows active range of motion and i
