Impact of Septal Extension Graft on Functional and Esthetic Outcomes in Patients with Severe Nasal Deviation: A Systematic Review and Meta-analysis.

BACKGROUND: Severe nasal deviation is a complex deformity affecting the esthetics and functionality of the nose, creating an asymmetrical appearance due to septal misalignment. Advances in nasal anatomy and biomechanics have led to the use of grafting materials, such as septal extension grafts. However, its efficacy remains inconclusive. This study investigated the impact of septal extension grafts on the functional and esthetic outcomes in severe nasal deviation. METHODS: A literature search was conducted using PubMed, Google Scholar, MEDLINE, Web of Science, and the Cochrane Central Registe

Utilization of an Integrated Biomimetic Groove Graft for Moderate-to-Severe Saddle-Nose Deformity Correction.

BACKGROUND: The key to correcting moderate-to-severe saddle-nose deformities is restoring septal support. Owing to the heterogeneity in the extent and severity of septal defects, constructing a cartilage framework tailored to specific conditions is challenging. OBJECTIVES: The authors of this study aimed to introduce their procedure utilizing a novel integrated biomimetic groove graft (IBGG) technique for cartilage framework construction in the correction of moderate-to-severe saddle-nose deformities and to evaluate its clinical outcomes. METHODS: The authors of this retrospective study incl

Outcome of Absorbable Plate Insertion in Septoplasty for Nasal Bone Fracture Patients: Septal Straightening Effect.

BACKGROUND: Various synthetic materials, often used for intranasal packing rather than for structural struts, have been studied for use in septoplasty. The authors' method involved the use of an absorbable plate to support the corrected septum, aid immediate straightening, and provide structural support to withstand pressure during the healing period. METHODS: Between October 2010 and April 2024, 20 patients with nasal septum deviation underwent septoplasty with an intranasal absorbable plate. The surgical approach involved meticulous dissection of the mucoperichondrium, followed by insertion

Patient Satisfaction Level of the Functional and Aesthetic Outcome of Secondary Rhinoplasty in Patients With Secondary Deformity of the Nose After Lip Repair in Hospital Kuala Lumpur and Hospital Pakar Universiti Sains Malaysia.

BACKGROUND: Cleft lip and palate are common congenital conditions that often result in nasal deformities, affecting both aesthetics and functionality. Secondary rhinoplasty plays an important role in correcting these deformities, bringing about an improvement in the function of the nose, facial harmony, and quality of life. Surgical techniques have advanced with time, but limited research has highlighted the patient-reported satisfaction levels with the aesthetic and functional outcome of secondary rhinoplasty. This study evaluated the aesthetic, functional, and overall satisfaction among clef

Preservation Concepts in Secondary Rhinoplasty: Is There Any Role?

INTRODUCTION: Secondary rhinoplasty presents complex challenges due to anatomical distortions, scar tissue, and multiple iatrogenic deformities. OBJECTIVES & HYPOTHESES: Although a traditional structural approach is the time-honored benchmark against which any other technique should be measured, preservation concepts can be integrated together with structural techniques. STUDY DESIGN: This study is based on a retrospective review of cases. METHODS: Preservation concepts applicable to secondary rhinoplasty are outlined in detail. A paradigm case highlighting the strategic application of pres

Versatile Usability of Lateral Crural Strut Graft for Correction of Various Nasal Tip Deformities in Secondary Rhinoplasty in Asians.

BACKGROUND: The insertion of implants and tip plasties have been performed in Asian patients for an attractive and higher nasal profile, which can result in diverse tip deformations such as notching, pinching, alar retraction, incompetent external valve, deviated tip, cephalic rotation, and lateral crus malposition because of inappropriate manipulation of the lower lateral cartilage and/or silicone capsular contracture. This study aimed to investigate the effectiveness and utility of applying a lateral crural strut graft (LCSG) as a consistent and appropriate surgical method to correct various

Rotational Columellar Skin Flap for Nasal Alar Rim Reconstruction.

Nasal alar reconstruction is complex due to the region's anatomy and aesthetic importance. This report describes repairing a small, full-thickness alar rim defect in a 36-year-old man using a rotational columellar skin flap with septal cartilage grafting. This single-stage technique achieved good color match, symmetry, and minimal donor-site morbidity. Compared to multi-stage flaps or composite grafts, this method is less invasive and effective for small defects, offering reliable aesthetic and functional results in carefully selected cases.Level of Evidence V This journal requires that author

[Septal deviation].

Septal deviation is one of the most frequent causes of nasal obstruction. Septal deviation may result from trauma, congenital malformations, or age-related changes, and symptoms include impaired nasal airflow, snoring, and poor sleep quality. Septoplasty is the standard treatment. The procedure is generally safe and improves quality of life for most patients, though recurrence of symptoms is not uncommon. Careful patient selection and preoperative evaluation are key to optimizing surgical outcomes as argued in this review.

Total Anatomical Nasal Skeletal Reconstruction Using a New Autologous Graft: The Tent Shaped Fibular Contoured Bone Graft.

BACKGROUND: Reconstruction of the bony nasal dorsum is a challenge due to the unique anatomy of the nasal bones. The nasal bones by virtue of their shape, angulation and size provide an aesthetic and pleasing contour of the upper nose. The anatomy of the nasal bones can get deformed in many situations like congenital anomalies, iatrogenic and post-traumatic situations leading to nasal deformity. Many options for reconstruction of nasal dorsum are available including soft tissue and bony material. It is always desirable to reconstruct like tissue with like tissue. This paper describes the use o

Even Unmanipulated Eyebrows Are Unreliable Landmarks for Evaluating Deviated Noses in Rhinoplasty.

OBJECTIVES: The eyebrow is a frequently used landmark in rhinoplasty. While caution is advisable when relying on manipulated eyebrows as a reference, there is a lack of evidence regarding the use of unmanipulated eyebrows as a landmark. This study aimed to assess the reliability of unmanipulated eyebrows as landmarks for evaluating deviated nose. METHODS: We analyzed preoperative photographs of patients who underwent rhinoplasty. We established mid-canthal and mid-eyebrow lines by bisecting the intercanthal distance and the most medial points of the eyebrows, respectively. Measurements were t

Comparison of Unilateral versus Bilateral Spreader Grafts in Crooked Indian Noses.

BACKGROUND: Although bilateral spreader grafts play a crucial role in rhinoplasty of crooked noses, adequacy of the cartilage often remains the key limiting factor, especially in Indian noses. Instead, a unilateral spreader graft can be used along with the neoseptum to ensure good functional and aesthetic outcomes. The present study compares the functional and aesthetic outcomes of unilateral vs. bilateral spreader grafts in patients with crooked noses. MATERIALS AND METHODS: It is a retrospective analysis of cases who underwent septorhinoplasty in a tertiary care referral hospital from March

Possibilities of intranasal reconstruction in complex nasal defects.

BACKGROUND: Complex nasal defects most often arise due to oncological resection or severe trauma. Traditional methods of two-stage nose reconstruction using a forehead flap with a skin graft have often resulted in collapse and deformity of the nose with a very compromised outcome over time. These techniques were gradually replaced by new procedures consistently reconstructing the intranasal lining, most often with flaps from the nasal septum. These methods reconstruct the cartilaginous and bony support of the nose as well, while the skin cover of the nose is, nowadays, in large defects, recons

Functional and Cosmetic Considerations in Saddle Nose Deformity Repair.

The saddle nose deformity is associated with dorsal collapse and can have both function and cosmetic problems. The saddle nose can cause nasal obstruction by narrowing the nasal cavities, eliciting dynamic internal and external nasal valve narrowing, and abnormally widening the internal and external nasal valves altering airflow dynamics, sinonasal passageways, and olfaction. The saddle nose is challenging to treat due to skin contracture, lack of donor tissue, and difficulties in esthetic integration. A focus on balancing the functional and esthetic concerns during treatment is paramount.

Low-Strip Dorsal Preservation Rhinoplasty Opens the Internal Nasal Valve in Tension Nose Deformity.

BACKGROUND: Dorsal preservation is a recently popularized technique to lower the nasal dorsum without opening the cartilaginous vault. Improved nasal breathing has been reported after lowering an intact dorsum using preservation techniques, suggesting that septal deprojection opens the internal nasal valves. The goal of this study was to evaluate the effect of dorsal preservation on internal nasal valve dimensions in noses with an overprojected cartilaginous septum. METHODS: Ten postmortem human specimen heads with a tension nose deformity were imaged using ultra-high-resolution photon-counti
