Possibilities of intranasal reconstruction in complex nasal defects.

BACKGROUND: Complex nasal defects most often arise due to oncological resection or severe trauma. Traditional methods of two-stage nose reconstruction using a forehead flap with a skin graft have often resulted in collapse and deformity of the nose with a very compromised outcome over time. These techniques were gradually replaced by new procedures consistently reconstructing the intranasal lining, most often with flaps from the nasal septum. These methods reconstruct the cartilaginous and bony support of the nose as well, while the skin cover of the nose is, nowadays, in large defects, recons

Multimodal characterization of intracranial biomechanics in a 3D biofidelic head surrogate.

Rapid head motion causes the brain to deform, which may lead to acute and chronic consequences to normal brain health and function. Since the risk and severity of brain injury correlates with brain strain, computational and physical models of the brain response to impact are commonly used to assess risk and evaluate safety devices in silico. Most physical head surrogates that are used for equipment evaluation, however, are simplified and lack internal measures of brain deformation, relying instead on the interpretation of the resulting head kinematics to predict injury. Developing a more biofi

Finite element analysis of different cephalomedullary nail treatments for basicervical femoral neck fracture.

Cephalomedullary nail (CMN) is an ideal internal implant for the treatment of Basicervical femoral neck fracture (BFNF). This study uses finite element analysis techniques to compare the biomechanical characteristics of three types of CMN systems, namely proximal femoral nail antirotation (PFNA) InterTAN and proximal femur bionic nail (PFBN), in fixing BFNF using three cannulated screws (CSs) as controls, and analyzed their clinical significance. Based on femoral CT and internal implant data, a finite element analysis model for BFNF internal fixation was established: CSs, PFNA, InterTAN, and P

Posterior Shoulder Instability: Spectrum of Presentation and Treatment Outcomes in a Case Series.

Posterior shoulder dislocation is an uncommon condition, accounting for a minority of shoulder instability cases. It is more frequently encountered in high-level athletes and is often missed at initial presentation, which can lead to chronic instability and recurrent dislocations. Predisposing factors include congenital anatomical variations, capsular laxity, and acquired injuries such as trauma or tetanic muscle contractions during seizures or electrical shocks. Management strategies are broadly divided into conservative and surgical approaches. Nonoperative treatment focuses on strengthening

A Rare Case of Adult Acquired Flatfoot Deformity (AAFD) With Chronic Atraumatic Subtalar Dislocation: Successful Reduction and Fusion of the Talonavicular and Talocalcaneal Joints.

Chronic atraumatic subtalar dislocation is an exceptionally rare and severe manifestation of adult acquired flatfoot deformity (AAFD) secondary to posterior tibial tendon insufficiency (PTTI). Early recognition is critical, as progressive deformity leads to rigid valgus malalignment and functional decline. A 65-year-old male presented with a seven-year history of worsening left foot pain and deformity. Examination showed rigid hindfoot valgus, midfoot collapse, forefoot abduction, and taut skin over the medial talar head. Radiographs revealed chronic lateral subtalar dislocation with talonavic

Artificial intelligence-assisted detection of nasopharyngeal carcinoma on endoscopic images: a national, multicentre, model development and validation study.

BACKGROUND: Nasopharyngeal carcinoma is highly curable when diagnosed early. However, the nasopharynx's obscure anatomical position and the similarity of local imaging manifestations with those of other nasopharyngeal diseases often lead to diagnostic challenges, resulting in delayed or missed diagnoses. Our aim was to develop a deep learning algorithm to enhance an otolaryngologist's diagnostic capabilities by differentiating between nasopharyngeal carcinoma, benign hyperplasia, and normal nasopharynx during endoscopic examination. METHODS: In this national, multicentre, model development an

Tumor Treatment Response-Guided Adaptive Dose De-escalation/Escalation Strategy for Concurrent Chemoradiation Therapy of Nasopharyngeal Carcinoma.

PURPOSE: A novel tumor dose response-guided adaptive chemoradiation therapy process for nasopharyngeal carcinoma (NPC) was constructed and evaluated prospectively and retrospectively. In this process, tumor voxel dose response matrices (DRMs) quantified using 2-point fluorodeoxyglucose (FDG) positron emission tomography/magnetic resonance images acquired before and after an induction chemotherapy (IC) were used to guide quantitatively the individual patient dose de-escalation/escalation for patients with NPC in the post-IC concurrent chemoradiation therapy (CCRT). Characterizing the safety and efficacy of injection pharyngoplasty as a treatment for velopharyngeal insufficiency in adults.

PURPOSE: Although injection pharyngoplasty (IP) has been well described as a treatment for velopharyngeal insufficiency (VPI) in the pediatric population, there are far fewer studies on its safety and effectiveness in adults with acquired VPI. Our study aims to characterize the safety and efficacy of IP as a treatment for acquired VPI in adults while identifying underlying VPI etiologies and injection characteristics that are associated with positive outcomes. METHODS: We conducted a retrospective analysis of 30 adult patients with acquired VPI who were treated with IP. We collected data on p

Smartphone use-related acute accommodative esotropia : A possible new paradigm of visual disorders associated with the use of modern technologies.

We present a rare case of a 9-year-old child with acute accommodative comitant esotropia, a form of acquired strabismus caused by excessive smartphone use. Symptoms commonly include double vision, closing or covering one eye when working at close range and tilting the head or its unnatural position to correct visual abnormalities; however, there are also patients without any symptoms. As part of the differential diagnosis it is necessary to exclude any eye diseases or refractive errors and it is highly recommended to carefully exclude pathologies of the central nervous system and intracranial

Immune evasion through mitochondrial transfer in the tumour microenvironment.

Cancer cells in the tumour microenvironment use various mechanisms to evade the immune system, particularly T cell attack1. For example, metabolic reprogramming in the tumour microenvironment and mitochondrial dysfunction in tumour-infiltrating lymphocytes (TILs) impair antitumour immune responses2-4. However, detailed mechanisms of such processes remain unclear. Here we analyse clinical specimens and identify mitochondrial DNA (mtDNA) mutations in TILs that are shared with cancer cells. Moreover, mitochondria with mtDNA mutations from cancer cells are able to transfer to TILs. Typically, mito

Subglottic Stenosis After Radioactive Iodine Treatment for Graves' Disease: A Case Report.

BACKGROUND: Subglottic stenosis (SGS) is a narrowing of the airway below the glottis and may be congenital or acquired. While prolonged intubation is the most common cause of acquired SGS, other etiologies remain incompletely understood. METHODS: A 37-year-old woman presented with a four-year history of mild progressive stridor, dysphonia, and chronic cough. Her medical history was notable for Graves' disease, treated initially with long-term carbimazole followed by radioactive iodine (RAI). She had never been intubated and had no autoimmune or rheumatologic disease. Awake fiberoptic endoscop

Rabbit conformational predispositions to ear abnormalities: Field study of a pedigree population.

Lop-eared conformation has previously been associated with rabbit ear disease in retrospective studies of clinical populations. Here, we investigated conformational and signalment risk factors for ear abnormalities in a field study of pedigree rabbits, where ear and skull conformation could be directly observed. Ears of 435 breeder-volunteered rabbits were examined with an otoscope at British Rabbit Council shows and studs, and risk factor analysis was performed with linear and binary logistic modelling. Inter-observer reliability was Substantial-Excellent for most variables. Lop-eared rabbits

A rare case of unilateral parotid gland agenesis.

Parotid gland agenesis is a rare congenital anomaly with fewer than 30 cases reported in the literature. It can present with xerostomia, facial asymmetry, recurrent infections, or may be entirely asymptomatic. Due to its rarity and potential to mimic other pathological conditions, especially in cases of facial asymmetry, awareness is crucial for accurate diagnosis. We report the case of a 39-year-old man referred to our clinic for routine follow-up of a thyroid nodule. The patient had no history of systemic illness or symptoms related to salivary gland dysfunction. During neck ultrasonography,

Proteoglycans in Mechanobiology of Tissues and Organs: Normal Functions and Mechanopathology.

Proteoglycans (PGs) are a diverse class of glycoconjugates that serve critical functions in normal mechanobiology and mechanopathology. Both the protein cores and attached glycosaminoglycan (GAG) chains function in mechanically-sensitive processes, and loss of either can contribute to development of pathological conditions. PGs function as key components of the extracellular matrix (ECM) where they can serve as mechanosensors in mechanosensitive tissues including bone, cartilage, tendon, blood vessels and soft organs. The mechanical properties of these tissues depend on the presence and functi

'Choroid bar': easy-to-seek marker of normal posterior fossa at 12-14 weeks' gestation.

OBJECTIVES: Our objectives were: (1) to assess the visualization rate of the choroid bar in a consecutive series of 306 first-trimester scans; (2) to verify, in this cohort of fetuses, the normality of the posterior fossa later in pregnancy; and (3) to confirm the non-visualization of the choroid bar in a retrospective series of fetuses with posterior fossa malformations. METHODS: This study included a prospective and a retrospective series. The former comprised 306 fetuses undergoing routine obstetric ultrasound at our unit in both the first and second trimesters over a 6-month period, while

Chromosomal microarray testing yield in 829 cases of microcephaly: a clinical characteristics-based analysis for prenatal and postnatal cases.

INTRODUCTION: Microcephaly, characterized by abnormal head growth, can often serve as an initial indicator of congenital, genetic, or acquired disorders. In this study, we sought to evaluate the effectiveness of chromosomal microarray (CMA) testing in detecting abnormalities in both prenatal and postnatal cases of microcephaly. MATERIALS AND METHODS: CMA Testing: We conducted CMA testing on 87 prenatally-detected microcephaly cases and 742 postnatal cases at a single laboratory. We evaluated the CMA yield in relation to specific clinical characteristics. RESULTS: In prenatal cases, pathogeni

Craniofacial anomalies in schizophrenia-relevant GFAP.HMOX10-12m mice.

Subtle craniofacial dysmorphology has been reported in schizophrenia patients. This dysmorphology includes midline facial elongation, frontonasal anomalies and a sexually dimorphic deviation from normal directional asymmetry of the face, with male patients showing reduced and female patients showing enhanced facial asymmetry relative to healthy control subjects. GFAP.HMOX10-12m transgenic mice (Mus musculus) that overexpress heme oxygenase-1 in astrocytes recapitulate many schizophrenia-relevant neurochemical, neuropathological and behavioral features. As morphogenesis of the brain, skull and
