Correction of swan-neck and boutonnière deformities.

Rheumatoid arthritis frequently affecting the hand is a progressive and disabling illness. The delicate and precis digital motor mechanism is frequently affected by this disease and the consequences are devastating, with marked functional impairment. This paper discusses two of the most frequent digital deformities: the swan-neck and boutonniere. The pathophysiology, and classification according to the evolution grades of these deformities are presented. Practical application of various surgical techniques, each worked out and proven over 30 years of experience and a great number of cases trea

Computerized imaging for soft tissue and osseous reconstruction in the head and neck.

Recent developments in computer-aided medical imaging coupled with the related emergence of computer-aided design and manufacturing technology have had a significant effect on our management of patients with congenital and acquired head and neck deformities. In our institution, plain film skull radiography, cephalometry, and pluridirectional tomography have been largely replaced by high-resolution CT scanning augmented by planar reformations and three-dimensional surface reconstructions. A sophisticated computer-assisted radiologic imaging unit has been established to assist the surgeon and re

Head and neck complications of epidermolysis bullosa.

Epidermolysis bullosa is a rare congenital skin disease which is characterized by easy formation of traumatic skin bullae. The disease is usually detected in early infancy due to the recurrent bullae. Prognosis is dependent on the histological variant which is present. A case of autosomal recessive epidermolysis bullosa is presented, which demonstrates many of the head and neck complications of the disease. Traditional skin care modalities as well as multiple surgical procedures such as skin grafts and the use of amniotic membrane were used without success. Limited natural healing, which was p

Correction of rheumatoid swan-neck deformity by lateral band mobilization.

Fifty-seven digits in 18 hands of 14 patients with rheumatoid disease were retrospectively evaluated after surgical correction of swan-neck deformity. Release of each hyperextension deformity was done via a previously described lateral band mobilization technique with temporary pin fixation of the proximal interphalangeal (PIP) joint and primary skin closure. Extension block splinting was used for 1 month after pin removal. Follow-up averaged 24 months. Twenty-two percent of the patients were enthusiastic about their results, 56% were satisfied, 22% were equivocal, and none were dissatisfied.

Flexible implant arthroplasty in the proximal interphalangeal joint of the hand.

Between 1966 and 1981, 812 proximal interphalangeal joints were reconstructed by the senior author using the flexible silicone rubber implant resection arthroplasty method. Within this series, a group of 182 patients with 424 proximal interphalangeal joint implants were available for a detailed clinical and roentgenographic review. Increased motion in a more functional arc with good pain relief and a favorable bone response pattern were noted in most cases after surgery. Complications were rare and included implant dislocation or fracture, recurrence of deformity, and infection. The highest co

Testicular cancer in homosexual men with cellular immune deficiency: report of 2 cases.

Embryonal cell carcinoma of the testis was seen in 2 homosexuals with peripheral lymphadenopathy of the head and neck, and abnormal cellular immunity measured by reduced T helper cells and increased T suppressor cells. One patient had no history of venereal disease but had taken marijuana, nitrites and methyl-dextroamphetamines regularly. The other patient had a history of syphilis, gonorrhea, hepatitis and venereal warts but rarely used inhalant recreational drugs. Both patients had smoked cigarettes. Neither patient had any known risk factors that predisposed to testicular cancer. Biopsy of

Roentgenographic and mathematical analysis of first metatarsal osteotomies for metatarsus primus varus: a comparative study.

The operative effectiveness of five different first metatarsal osteotomies for nonarthritic hallux valgus and metatarsus primus varus were objectively evaluated roentgenographically. The series consisted of 120 feet (75 patients) seen over a 5-year period. The osteotomies were biplanar neck, Chevron, biplanar basilar, basilar concentric, and basilar concentric combined with a lateral closing wedge. All the osteotomies except for the Chevron had varying degrees of plantar displacement of the distal fragment and crossed Kirschner wire fixation. The operative techniques and failures are discussed

[Bladder wall urethroplasty in the treatment of urinary incontinence caused by congenital cervico-urethral malformations].

Twelve patients with urinary incontinence from congenital cervico-urethral malformations were treated by bladder wall urethroplasty. Operation was that of Leadbetter (16 times) or Tanagho (twice) type. Results were satisfactory in 5 patients, poor in the other 7 cases: 6 children required endoscopic resection of the neck malformation to improve bladder retention, but this failed to provoke deterioration of the acquired continence. The relevant literature is reviewed, the commonest techniques mentioned, and factors affecting results analyzed. Essential factors appear to be the state of the blad

Dental and maxillofacial abnormalities in long-term survivors of childhood cancer: effects of treatment with chemotherapy and radiation to the head and neck.

Sixty-eight long-term survivors of childhood cancer were evaluated for dental and maxillofacial abnormalities. Forty-five patients had received maxillofacial radiation for lymphoma, leukemia, rhabdomyosarcoma, and miscellaneous tumors. Forty-three of the 45 patients and the remaining 23 who had not received maxillofacial radiation also received chemotherapy. Dental and maxillofacial abnormalities were detected in 37 of the 45 (82%) radiated patients. Dental abnormalities comprised foreshortening and blunting of roots, incomplete calcification, premature closure of apices, delayed or arrested t

Post-burn contractures in Ibadan.

A review of our experience with post-burn contractures over a ten-year-period at the University of Ibadan is reported. Fifty-two patients (32 males, 20 females) were treated; 52% were under the age of 5 years. In 25% the contractures were due to therapeutic burns. Sixty-two per cent of the contractures occurred in the upper limbs. Best results were obtained by release of contracture, application of split-thickness skin grafts, and splinting for up to six months.

The otolaryngologist's role in the craniofacial anomalies team.

The otolaryngologist has a multifaceted role on the craniofacial anomalies team. The diagnosis and treatment of hearing disorders, both congenital and acquired; maintenance of communication through adequate speech and hearing; management of the airway and deglutition; and reconstructive surgery of the head and neck are the primary responsibilities to children and adults with craniofacial anomalies. The complete evaluation and treatment of these disorders can be both very challenging and very rewarding.

Cosmetic rhinoplasty: radiological features.

The usual radiologic postoperative appearance of the bony nasal pyramid following a typical cosmetic rhinoplasty is demonstrated, and possible atypical sequelae are discussed. More specifically, the sites of the osteotomies performed as part of this surgical procedure are diagrammatically illustrated. It is believed that because of the increasing popularity of rhinoplastic operations, it is timely to acquaint radiologists with the postsurgical stigmata which, though minor, might be erroneously interpreted as resulting from accidental trauma or other pathologic processes. Beyond academic consid

Acquired obstructions of the lower urinary tract in children.

Acquired obstructions of the lower urinary tract in children are uncommon. They can be divided into intrinsic and extrinsic lesions, and have a very varied etiology. Several illustrative cases are reported, such as traumatic and infectious strictures, meatal stenosis, benign and malignant tumors, fused labia, and epidermolysis bullosa. Many of these lesions have distinct radiographical features, best demonstrated by micturating cystourethrograms.

Digital subtraction angiography: applications in otolaryngology--head and neck surgery.

Digital electronic technology has made its most notable contribution to radiology in the form of the CT scanners which have so greatly benefited medical practice. Experience with intravenous angiography using a digital fluoroscopy unit developed at the University of Arizona gives promise of a further major benefit in the area of angiography. The majority of examinations are conducted on an outpatient basis with a risk level comparable to that of an intravenous pyelogram. The authors review their experience with this unit, including it's diagnostic and therapeutic applications to congenital and

Acquired laryngeal deviation associated with cervical spine disease in erosive polyarticular arthritis. Use of the fiberoptic bronchoscope in rheumatoid disease.

Cervical spine disease in patients with erosive polyarticular arthritis often presents difficulties in endotracheal intubation at the time of surgery. After extensive experience with the use of the fiberoptic bronchoscope in such situations, the authors have identified a previously unrecognized and unanticipated tri-plane deviation in which the larynx is displaced caudally, deviated to the left, rotated to the right, and anteriorly angulated. A review of 710 consecutive fiberoptic intubations identified 15 arthritis patients with laryngeal deviation. Documentation was available in seven patien

Palmar arthroplasty for the treatment of the stiff swan-neck deformity.

Palmar arthroplasty for the treatment of the stiff swan-neck deformity in rheumatoid arthritis is designed to correct the mechanical block to flexion that is caused by palmar plate adhesions (which obliterate the retrocondylar recess) and by collateral ligament contracture and adhesions. This procedure can be performed at the same time as correction of the primary cause of proximal interphalangeal joint (PIP) hyperextension (e.g., intrinsic tightness or flexor tenosynovitis) and can also be supplemented with superficialis tenodesis to minimize recurrent hyperextension. Postoperative flexor dyn

Xanthoma disseminatum: an unusual cause of facial and limb deformity.

A rare case of xanthoma disseminatum producing deformity of the head, neck, and extremities is reported. This pathologically benign disease, considered to represent one of the histiocyte proliferative disorders, is manifested by numerous cutaneous xanthomas primarily in flexion areas. Progressive enlargement of these lesions accompanied by marked fibrosis has produced the severe cosmetic and functional deformities described in this patient. Treatment in this case involved excision of xanthomatous contraction bands with split-thickness skin grafting, followed by intensive physical therapy.
