A unique case is described of subarachnoid hemorrhage from a ruptured berry aneurysm of the right anterior ethmoidal artery, and its pathogenesis is discussed. The literature suggests an increased incidence of posterior circulation aneurysms in moyamoya disease, of contralateral anterior circulation aneurysms in agenesis of the carotid artery, and of aneurysms at various sites in spontaneous and iatrogenically acquired obstructions of the internal carotid artery. In addition to congenital anomalies of the vessel wall and systemic hypertension, increased regional blood flow should be regarded a

Subglottic stenosis secondary to endotracheal intubation is a distinct clinical entity. Prolonged intubation is an important causative factor, but even short-term endotracheal intubation with too large a tube can damage the cricoid sufficient to cause delayed subglottic stenosis. After studying cricoid size and other factors in 130 adult laryngectomy and autopsy specimens, the following conclusions were reached: (1) The subglottic cricoid is the smallest, fixed cross section in the upper airway. (2) Even if height is comparable, women have smaller cricoid diameters than men. (3) Height is the

A series of computer programs were created to display digital data obtained by the computerized tomographic (CT) scanner in a format readily comprehensible to the nonradiologist physician. The software requires no hardware beyond the CT scanner itself, is efficient in computational time and storage, and requires no operator intervention. The program manipulates standard CT axial data into images that simulate photographs of skin, muscle, or bone surfaces from a variety of perspectives. While the images themselves are planar, the surfaces they display are three dimensional. Portions of the imag

Iatrogenic dorsal bunions in children, observed mostly after operations for post-poliomyelitic paralysis or club foot, have been successfully treated by an operation in which the tendons of the flexor brevis and the abductor and adductor muscles of the hallux are transferred to the neck of the first metatarsal. This is supplemented by removal of the sesamoids, capsulotomy of the metatarsophalangeal joint, and when necessary by arthrodesis or tenodesis of the metatarsophalangeal joint. In seventeen feet treated in this way only one failure, in a patient with arthrogryposis, was recorded.

Surgical reconstruction of the total rhinectomy defect presents unique difficulties for both the surgeon and the patient. The forehead flap provides an excellent color match and two-stage reconstruction. However, frequently, flap delay is needed, there may be insufficient cutaneous tissue available, and, most important, a secondary residual facial deformity is created. The tube abdominal flap technique of total nasal reconstruction provides adequate tissue and a hidden scar but gives a poor facial color match, offers multiple operative stages, and can severely debilitate the patient's shoulder

A one-stage operation is described for reconstruction of large defects of the columella nasi, the lower part of the nasal septum, and the vestibular floors. Nasolabial flaps are transported on a cranial, de-epithelialized pedicle, and passed deep to the nostrils into the vestibular cavity. The method is illustrated with four case reports.

The experience at Emory University Affiliated Hospitals with transplantation of the greater omentum as a free revascularized graft in 18 patients is presented. In each instance, there was realization of the therapeutic objective, either 1) the amelioration of congenital or acquired somatic deformity (14 patients) or 2) the control of infection (4 patients). Because the omentum is a syncytium of blood vessels and a variable amount of fat within redundant leaves of peritoneal membrane laden with macrophages, it is a tissue that serves admirably its extended role as an extracelomic free transplan

Two observations have led to the consideration of another possible factor in the etiology of rheumatoid swan-neck deformity. Distraction of the rheumatoid wrist with correctable carpal collapse and reversible swan-neck deformities resulted in easier initiation of proximal interphalangeal joint flexion, not found in hands with boutonniere deformity or lack of deformity. Review of x-rays of rheumatoid patients with swan-neck deformity indicates that most have evidence of carpal collapse. With carpal collapse and proximal migration of the carpal-hand unit, long or extrinsic motor systems are no l

The character and frequency of internal carotid artery deformities on the neck were examined on 1009 arteriograms taken from patients suffering from various cerebral pathologies. Prevalence of convolution and looping of the artery was noted in patients with hypertensive hemorrhages (37%), hemorrhages due to intracranial sacculated aneurysms (35%), and, to a lesser extent, in patients with spontaneous subarachnoidal hemorrhages (19%), intracranial tumours (25%) and traumatic subdural hematomas (22%). This was, probably, due to age-related peculiarities of the vessel structure. It is supposed th

Intubation problems sometimes occur very suddenly and can be divided into two groups. The expected ones include the patients with a short neck and long teeth, cellulitis of the tongue, large oropharyngeal tumors, obstructing laryngeal tumors, congenital and acquired maxillofacial deformities, ankylosis of the temporomandibular joints, fractures or ankylosing spondylitis of the cervical spine, and all patients with a history of previous intubation problems. Unexpected problems can arise in patients who combine large incisors and canines with an inability to open the mouth wide, or when the glot

Congenital and acquired subglottic stenosis is a commonly encountered problem in the pediatric population. In acquired cases endotracheal intubation is responsible for its development in the great majority of cases, but high tracheotomy, laryngeal burns, external neck trauma, and tumors, both intrinsic and extrinsic, are occasionally seen. The management of mature subglottic stenosis in children remains a controversial issue. The prevailing attitude of otolaryngologists is to perform a tracheotomy and hope for decannulation after one or two years, due to the expected growth of the larynx. Unfo

A retrospective study was carried out by the analysis of 212 charts of leprosy patients attending the Schieffelin Leprosy Research and Training Centre for the period 1964 to 1974. The purpose was to determine what changes were evident in postoperative fingers after their sublimus tendons were surgically removed for some other purposes such as for use as a motor tendon for opponens replacement etc. At the conclusion of the analysis it was found that there were five negative and two positive effects for these fingers. The negative effects were post-operative development of (1) Swan Neck Deformit

This study investigates the relationship of hand use to the deforming hand joint changes of rheumatoid arthritis. Measurements of metacarpophalangeal lateral mobility, loss of metacarpophalangeal hyperextension, thumb metacarpophalangeal range of motion, and lateral pinch strength were statistically compared across dominant and nondominant hands of 51 adult subjects who had definite or classical rheumatoid arthritis. Incidence of boutonniere and swan neck deformities and distal radioulna laxity was also recorded. Results of a multivariate analysis of variance of the difference scores between d

Infantile coxa vara is an unusual localized dysplasia, not evident at birth, which is first noticed when the child begins walking. Twelve cases are presented with emphasis on the characteristic roentgen findings which include abnormal ossification of the femoral neck, a vertical physis, a characteristic triangular osseous fragment at the medial inferior corner of the metaphysis, and a straight femoral shaft. Prompt diagnosis and early management can reduce severe deformity and degenerative changes of the hip. For this reason, infantile coxa vara must be differentiated from other generalized an

Traumatic arteriovenous malformations (AVM) are formed by an abnormal and usually complex communication between arteries and veins. In contrast to congenital AVMs which occur most commonly in the head and neck, traumatic AVMs are usually reported in the extremities and are commonly the result of military conflicts. In 1757, William Hunter delivered his classical treatise on true and false aneurysms and described a traumatic fistula between the brachial artery and vein following phlebotomy. Approximately thirty cases of acquired traumatic AVMs of the facial region appear in the literature. Only

The factors contributing to stability in the proximal interphalangeal joints are described. As a result of damage to one or more links of the system, a typical abnormal position of the joints occurs, called swan-neck deformity. The possibilities for stabilization of the joints are discussed with emphasis on the technique of LITTLER which employs a distally based lateral band. This band prevents recurvatum deformity of the PIP joint and flexion deformity of the DIP joint when it is passed palmar to CLELAND's ligament, thereby simulating the oblique retinacular ligament of LANDSMEER.

The pathogenesis and the anatomic causes for the swan neck deformity are reviewed. Many different soft tissue operations can succeed in preventing catastrophic loss of function if the deformity is treated while still in stage 1 or 2. In the later stages (3 and 4) restoration of finger is often not possible as a result of joint stiffness and irreversible joint changes. In this case, arthroplasty and arthrodesis should be considered.

The authors have operated on forty instances of pes cavus in children. Ten were idiopathic, nine poliomyelitic, five hemiplegic and five as a sequel of club foot. Others cases were of various neurological cause. In all cases, the plantar fasciae and ligaments were released; the technique of this procedure is fully described. In a number of cases, soft tissue release was associated with an osteotomy of the first metatarsal, a transplant of the extensor hallucis longus to the neck of the first metatarsal, an osteotomy of the calcaneus or various muscle transplants depending on the etiology of th
