Fractures of the phalanges of the hand were studied in 203 children. Fingertip fractures caused disability for three weeks but usually had good results. Comminuted fractures often took the form of longitudinal splitting. Epiphyseal fractures were mostly of the Salter type 2 and metaphyseal types and usually had good results: remodelling occurred in both planes at the base of the proximal phalanx but only in the sagittal plane at the base of the middle phalanx. This review of 29 patients with relapsing polychondritis seen at the Mayo Clinic between 1962 and 1976 emphasizes the head and neck manifestations of the disease and the role of the otolaryngologist in its diagnosis and treatment. The major clinical features included inflammation of the pinna, eye involvement, nasal cartilage involvement, laryngotracheal involvement, arthropathy, hearing loss, costal chondritis, and fever. The erythrocyte sedimentation rate was often elevated, and mild anemia was not uncommon.

Composite autografts of skin and cartilage from the auricule are useful in the reconstruction of defects in many areas of the head and neck. However, their use has certain limitations if minimal loss of the graft is to be achieved. A variety of examples are presented by the authors and the limitations are summarized.

A procedure is described for reconstruction of the oblique retinacular ligament using a small tendon graft in a spiral fashion to act as a dynamic tenodesis to restore distal interphalangeal extension and to restrain proximal interphalangeal hyperextension. The method has been uniformly successful in the treatment of post-traumatic "swan-neck" or "mallet" deformity. Although the concept is simple, the procedure demands thorough understanding of digital anatomy for successful completion.

Xeroradiography of the neck was performed on 36 children with suggested or known acquired tracheal abnormalities. The information obtained from the lateral projection xeroradiograph was superior in selected cases to the conventional soft tissue roentgenogram. In these cases, the information obtained justified the increased (12-fold) radiation exposure and obviated the need for positive contrast examination. This procedure should be considered in any case in which a fluoroscopic contrast examination is indicated.

Cervical herniation of the lung is a rare condition that results from congenital agenesis of Sibson Fascia. Occasionally, this may result from acquired causes like trauma. We report a rare case of a patient who was recently encountered in our Otorhinolaryngological Clinic and who posed an interesting problem of differential diagnosis. The management of such patients requires frequent checkups to safeguard against spontaneous pneumothorax. An otolaryngologist should not be caught unaware as the rhythmic "ballooning" of the supraclavicular area is quite characteristic with forced expiration.
