Soft tissue sarcoma formation following partial tarsal arthrodesis in dogs: Report of 2 cases.

This case report describes the occurrence of soft tissue sarcoma at the surgical site of partial tarsal arthrodesis in 2 dogs within 8 mo after surgery. Surgical correction was uncomplicated in both cases, and the dogs did clinically well postoperatively, until masses were noted over the surgical sites. Each dog underwent amputation of the affected leg, as the masses were deemed unresectable. This report describes an uncommon potential complication of partial tarsal arthrodesis in dogs. Key clinical message: The cases reported herein are apparently the first descriptions of soft tissue sarcoma

Multisequence MRI and clinical data-based deep learning radiomics model for predicting adjacent segment degeneration post-lumbar fusion: a retrospective multicenter study.

BACKGROUND: While adjacent segment degeneration (ASDeg) is a major complication following lumbar fusion, objective tools for preoperative risk prediction remain lacking. This study developed a model integrating clinical data, deep learning, and radiomic features from multisequence magnetic resonance imaging (MRI) to predict high-risk patients for ASDeg after lumbar fusion. METHODS: This study included 305 patients who underwent lumbar MRI before surgery. The patients were randomly divided into a training cohort (n = 192) and an internal validation cohort (n = 83). An external test cohort (n =

The utility of the Risk Assessment and Prediction Tool in predicting discharge disposition, length of stay, and readmissions in patients undergoing transforaminal lumbar interbody fusion.

OBJECTIVE: The Risk Assessment and Prediction Tool (RAPT) has been utilized to anticipate discharge needs after procedures such as total joint arthroplasty. Its usefulness for spine patients, particularly those undergoing transforaminal lumbar interbody fusion (TLIF), has not been clearly established. This study evaluated the relationship between the preoperative RAPT score and 3 postoperative outcomes: discharge destination, hospital length of stay (LOS), and 30-day readmission. METHODS: A retrospective cohort study was conducted of adults who underwent elective TLIF with a recorded preopera

Acute intra-articular calcaneal fractures: currents concepts.

Calcaneal fractures are predominantly intra-articular and most commonly involve the subtalar joint, with an estimated incidence of 11,5 per 100,000 population and a male predominance. They occur mainly after high-energy trauma and are associated with substantial functional impairment and major socio-occupational consequences. Anatomical assessment (loss of height, posterior facet collapse, lateral wall widening, and varus displacement with superior migration of the tuberosity) determines prognosis and guides management. Plain radiographs (Böhler's and Gissane's angles) remain useful, but compu

Morphometric findings of the subaxial cervical spine among different geographic regions and the implications for cervical pedicle screw placement: a systematic review and meta-analysis.

OBJECTIVE: Although cervical pedicle screws (CPS) improve outcomes in trauma and deformity surgery, existing morphometric studies have not been systematically consolidated. This systematic review aims to pool available data to characterize subaxial cervical pedicle dimensions and insertion angles across geographically diverse populations and evaluate population-level anatomical variation relevant to cervical pedicle screw planning. METHODS: PubMed, Embase, and Google Scholar were searched to identify anatomical studies published between database inception and August 1, 2024 that reported comp

Outcomes after total first metatarsophalangeal joint arthroplasty for hallux rigidus: a systematic review and meta-analysis.

BACKGROUND: The role of first metatarsophalangeal (MTP) joint arthroplasty in hallux rigidus remains controversial. Although used as a motion-preserving alternative to arthrodesis, outcomes vary between studies and implant designs. This systematic review and meta-analysis evaluated clinical outcomes, range of motion (ROM), and implant survivorship following total first MTP arthroplasty. METHODS: A systematic review identified studies reporting pre- and postoperative outcomes following total first MTP arthroplasty for hallux rigidus. Pooled outcomes were the American Orthopaedic Foot and Ankle

Combined Minimally Invasive Transforaminal Lumbar Interbody Fusion and Minimally Invasive Laminectomy.

Transforaminal lumbar interbody fusion (TLIF) is a well-established intervention for patients with spondylolisthesis and instability with compression of neural structures. Minimally invasive (MIS) TLIF has been proposed as potentially superior to a standard midline approach. For patients with both bilateral lateral recess stenosis and central stenosis, combining MIS-TLIF with MIS laminectomy allows for bilateral decompression, which is not usually afforded by standard TLIF, in addition to arthrodesis. This paper describes our technique for combining MIS-TLIF with MIS laminectomy, preserving mi

A Comparative Effectiveness Study of Bipolar and Linked Quadripolar Techniques for Eliciting Transcranial Motor Evoked Potentials.

INTRODUCTION: Linked Quadripolar Stimulation (LQP) is a recent adaptation of transcranial electric motor evoked potential stimulation, with proposed advantages over traditional Bipolar (BP) stimulation. This study aims to comprehensively compare BP and LQP stimulation to validate the efficacy of LQP. METHODS: BP and LQP stimulation were performed on 30 patients undergoing anterior cervical discectomy and fusion. A comprehensive assessment involved conducting four trials for each technique on each patient. An Accelerometer placed over the right masseter region recorded movement. A Mann-Whitney

Living with pain: Experiences, hopes and expectations when going through lumbar spinal fusion - A qualitative study.

INTRODUCTION: Lumbar spinal fusion (LSF) is a common surgical treatment for chronic low back pain (CLBP), yet evidence of its effectiveness compared with conservative care remains inconclusive. While pain is the primary clinical indication, psychosocial factors, including hopes and expectations, may influence outcomes. However, little is known about how individuals experience life with CLBP before surgery or what they hope to regain after LSF. OBJECTIVE: To explore patients' experiences with CLBP prior to LSF and their hopes and expectations for life after surgery. METHODS: A qualitative, ex

Comparative and qualitative analysis of endoscopic unilateral laminectomy with bilateral decompression versus interbody fusion in the treatment of grade 1 degenerative lumbar spondylolisthesis.

OBJECTIVE: Traditional management of symptomatic lumbar spondylolisthesis (SPL) involves interbody fusion (IBF). However, endoscopic unilateral laminectomy with bilateral decompression (E-ULBD) has emerged as a less invasive alternative with growing evidence of effectiveness. The authors aimed to comprehensively compare outcomes between E-ULBD and IBF in grade 1 lumbar degenerative SPL. METHODS: The data of patients who underwent E-ULBD or IBF for symptomatic lumbar SPL by a single surgeon during the same period were retrospectively reviewed. Demographics, SPL characteristics, surgical profil

Use of Femoral Head Allograft with Segmental Defects of the First Ray.

Introduction describes first metatarsophalangeal (MTP) joint arthrodesis as a historical and currently powerful option for severe forefoot deformities, including arthritis, hallux valgus, hallux varus, and trauma. Shortening of the first ray, particularly after MIS bunion procedures and revisional surgeries, creates biomechanics challenges that can compromise fusion. To address large segmental defects, the senior author advocates interpositional femoral head allograft to restore first-ray length and provide structural support for stable MTP joint arthrodesis. The approach avoids autograft draw

Isolated Subtalar Arthrodesis.

Isolated subtalar arthrodesis is a common surgical procedure used to treat subtalar joint pathologies including primary and post-traumatic arthritis, calcaneal fractures, tarsal coalitions, and posterior tibial tendon dysfunction. Its goals are pain relief, rearfoot realignment, and improved stability while preserving more motion than triple arthrodesis. This review covers the relevant anatomy of the subtalar joint and its facets, clinical examination findings, and imaging modalities. It details surgical techniques for native fusions, neglected calcaneal fractures, and rigid valgus deformities

Managing Long-Term Ambulation After Midfoot or Rearfoot Arthrodesis.

The "Step Diet" is a physician-designed activity management protocol for patients with foot pathology or prior reconstructive surgery who experience persistent pain from overuse. Rather than pursuing the mythologized 10,000 step daily goal-rooted in 1960s Japanese marketing, not medical evidence-patients track daily steps and record pain levels on a 1 to 10 scale over 4 weeks. A personalized safe step threshold is then identified, with two permitted "cheat days" weekly. This structured approach enables patients to budget around priorities, predict discomfort, and plan recovery periods. Clinica

A Posterior-Only Revision Surgery Protocol with Selective Cement Preservation for Post-Vertebral Augmentation Infection.

Post-vertebral augmentation infection is uncommon but clinically serious, especially in older patients with multiple comorbidities and fragile spinal bone stock. Revision surgery is challenging because surgeons must eradicate infection, decompress neural structures when needed, restore stability, and decide whether polymethylmethacrylate cement can be safely preserved. This article describes a posterior-only protocol for diagnosing and treating post-vertebral augmentation infection and illustrates the workflow with a retrospective representative cohort treated between January 2015 and December

Muscle-Sparing Wiltse Transforaminal Lumbar Interbody Fusion: A Technical Guide for the Modified Paraspinal Approach.

The Wiltse transforaminal lumbar interbody fusion (W-TLIF) modifies the traditional open posterior lumbar fusion to treat the same degenerative pathologies through a smaller, muscle-sparing incision that accesses the lumbar spine through the paramedian corridor between the multifidus and longissimus muscles, preserving the posterior musculature and limiting iatrogenic soft tissue trauma. This protocol provides a reproducible, step-by-step guide to performing the muscle-sparing W-TLIF for lumbar decompression and interbody fusion.
