Evaluation and management of neuropathic arthropathy of the shoulder: narrative review.

Neuropathic arthropathy of the shoulder, or Charcot shoulder, is a rare and destructive neuroarthropathy resulting from impaired protective sensation and altered joint mechanics that is most commonly associated with syringomyelia. The pathogenesis is believed to involve both neurotraumatic and neurovascular mechanisms, resulting in progressive osseous destruction, joint instability, and soft tissue compromise. Clinical presentation varies, with painless swelling, joint effusion, crepitus, and progressive limitation in shoulder function commonly reported. Radiographic findings frequently demons

Giant Cell Tumor of the Lateral Malleolus Treated by Tumor Excision and Reconstruction using A 180° Rotated Proximal Fibular Autograft - A Rare Case Report.

INTRODUCTION: Giant cell tumor (GCT) of bone was first described by Sir Astley Cooper in 1818. It is one of the most frequently encountered benign bone tumors in orthopedic practice and is characterized by a significant risk of local recurrence following surgical treatment. GCT involving the distal fibula, particularly the lateral malleolus, is extremely rare, accounting for <1% of all cases. Various treatment modalities have been described for lateral malleolus GCT, including tumor excision, ankle arthrodesis, reconstruction with ipsilateral proximal fibular graft, and autograft or allograft

What's New in Hand and Wrist Arthroscopy: Indications, Techniques, and Emerging Applications.

Wrist arthroscopy has undergone substantial evolution over the past 2 decades, transitioning from a primarily diagnostic procedure to a versatile therapeutic platform with applications across trauma, instability, degenerative disease, and reconstructive surgery. Advances in instrumentation, visualization, and surgical technique have expanded indications while facilitating less invasive management of complex wrist pathology. In acute fracture care, arthroscopy provides direct assessment of articular reduction and associated soft tissue injuries in scaphoid and distal radius fractures, with emer

To Walk or Not? A Prospective, Randomised Controlled Trial to Evaluate the Effect of Weightbearing on Patient Outcomes Following First Metatarsophalangeal Joint Fusion.

BACKGROUND: First metatarsophalangeal (MTP) joint arthrodesis is the gold standard surgical treatment of hallux rigidus and severe hallux valgus with osteoarthritis after failed nonoperative management. Despite its common use, postoperative weightbearing protocols vary considerably. To date, no prospective randomised controlled trials have directly compared outcomes between immediate weightbearing as tolerated (WBAT) and non-weightbearing (NWB) using modern fixation techniques. This study aimed to evaluate whether immediate WBAT following first metatarsophalangeal joint arthrodesis leads to di

Endoscopic Plantar Partial Sesamoidectomy for the Management of Metatarsalgia of the First Metatarsal Head After Arthrodesis of the First Metatarsophalangeal Joint.

First metatarsophalangeal arthrodesis has been used to treat various pathologies of the great toe with good clinical outcome. First metatarsal head metatarsalgia can occur after first metatarsophalangeal arthrodesis. Surgery is indicated if conservative measures cannot alleviate the symptoms. If there is isolated excessive plantar pressure of the first metatarsophalangeal joint without significant sagittal deformity of the first metatarsophalangeal joint, plantar partial ostectomy of the sesamoid bone(s) is indicated. The purpose of this technical note is to describe the technical details of e

Evolving Management Strategies for Osteochondral Lesions of the Talus: A Contemporary Treatment Algorithm.

➢ Osteochondral lesions of the talus (OLT) are common injuries that remain difficult to treat because of the challenge of restoring durable hyaline cartilage. Surgical outcomes are influenced by lesion size and morphology, cartilage integrity, subchondral bone involvement, and ankle stability, all of which should be considered when evaluating operative strategies.➢ Fragment fixation preserves the native osteochondral unit in lesions with a viable fragment and is primarily indicated in acute OLT. Lift-drill-fill-fix is utilized in selected chronic lesions with a salvageable fragment and comprom

Enhanced recovery after surgery for microsurgical oblique lateral interbody fusion: A retrospective before-and-after cohort study.

This study evaluates the real-world effectiveness and safety of a standardized enhanced recovery after surgery (ERAS) pathway in patients undergoing single-level microsurgical oblique lateral interbody fusion (MIS-OLIF) for lumbar degenerative disease. This retrospective before-and-after cohort study included 89 patients who underwent single-level MIS-OLIF at L3-4 or L4-5 between January 2021 and December 2023. Patients were categorized according to the institutional perioperative pathway in effect at the time of surgery: conventional care (n = 44) or ERAS (n = 45). The primary outcome was pos

Outcomes after MI-TLIF without bone morphogenetic protein: a retrospective study.

OBJECTIVE: Minimally invasive transforaminal lumbar interbody fusion (MI-TLIF) is widely used to treat degenerative lumbar spine conditions, offering reduced tissue trauma, faster recovery, and lower complication rates compared to those with open TLIF. Recombinant human bone morphogenetic protein 2 (rhBMP-2) can enhance fusion and is often used in the interbody space in minimally invasive fusions because of the limited posterolateral surface area for arthrodesis, but it is associated with higher costs and an increased risk of complications. Authors of this study evaluated the outcomes of MI-TL

Outcomes and Complications of Elbow Arthrodesis.

BACKGROUND: Arthrodesis of the elbow is a rarely performed procedure sometimes considered when no other reliable surgical solutions exist. Most published reports are limited to single case reports or very small series. The purpose of this study was to evaluate the outcomes and complications of patients who underwent elbow arthrodesis and to determine whether this is a worthwhile salvage procedure for elbows not amendable to surgical reconstruction or arthroplasty. METHODS: Between 2000 and 2024, a retrospective review was performed of 13 elbows (13 patients) that underwent elbow arthrodesis a

BACKGROUND: Following radial-sided lateral exposure for interphalangeal silicone arthroplasty, the repaired radial collateral ligament must maintain coronal stability against postoperative ulnar drift. Prospective direction-specific alignment data are limited. METHODS: Consecutive patients undergoing proximal interphalangeal (PIP) or distal interphalangeal (DIP) silicone arthroplasty through a radial-sided lateral approach with structured collateral ligament repair were prospectively enrolled (minimum follow-up, 12 months). The primary outcome was signed coronal-plane deviation on standardize

Digit revascularization in patient with congenital monodactyly: expanding indications for salvage.

Management of partial and complete digit amputations is guided by injury characteristics, patient comorbidities, and functional prognosis; however, congenital hand differences and disability remain underexplored in current literature. We present a 47-year-old congenitally blind patient with tetramelic monodactyly who sustained a near-complete zone II amputation of his only left digit following a dog bite, with associated metacarpal head destruction. Despite relative contraindication conferred by the mechanism of injury, the patient had profound reliance on his digit for independence; as such,

Management of a Pilon Fracture Complicated by Drug-induced Hypersensitivity Syndrome, Fungal Infection and Limb Length Discrepancy: A Case Report.

AIM AND BACKGROUND: Pilon fractures are rare but severe injuries associated with high-energy trauma, often resulting in significant articular and soft tissue damage. Their management is challenging due to complications such as infection, nonunion, and post-traumatic arthritis, which often require complex staged surgical interventions. Drug-induced hypersensitivity syndrome (DIHS), a life-threatening reaction characterised by rash and systemic involvement, presents additional challenges in orthopaedic contexts due to its association with high-dose corticosteroid therapy and increased infection

Soft tissue sarcoma formation following partial tarsal arthrodesis in dogs: Report of 2 cases.

This case report describes the occurrence of soft tissue sarcoma at the surgical site of partial tarsal arthrodesis in 2 dogs within 8 mo after surgery. Surgical correction was uncomplicated in both cases, and the dogs did clinically well postoperatively, until masses were noted over the surgical sites. Each dog underwent amputation of the affected leg, as the masses were deemed unresectable. This report describes an uncommon potential complication of partial tarsal arthrodesis in dogs. Key clinical message: The cases reported herein are apparently the first descriptions of soft tissue sarcoma
