Occipitocervical fusion and decompression in Morquio syndrome: a case report and literature review.

Mucopolysaccharidosis IVA, also known as Morquio syndrome, is a rare autosomal recessive lysosomal storage disorder caused by a deficiency of N-acetylgalactosamine-6-sulfate sulfatase. Spinal involvement, particularly at the craniocervical junction, is a major contributor to morbidity and mortality due to atlantoaxial instability and cervical spinal stenosis. Early diagnosis and timely surgical intervention are essential to prevent irreversible neurological deterioration. In this report, we present the case of a 10-year-old girl with progressive quadriparesis due to severe cervical myelopathy

Proximal Interphalangeal Joint Surface Replacement Arthroplasty Using the WALANT Technique by Palmar Approach: A Case Report.

Osteoarthritis of the proximal interphalangeal joint (PIPJ) is a common condition characterized by pain, weakness, stiffness, malalignment, and loss of movement in the affected hand. Conservative treatments such as medication, physical therapy, and splinting, as well as surgical procedures like synovectomy and debridement, may occasionally help; however, arthrodesis and PIPJ arthroplasty are the definitive surgical options. The goal of replacement arthroplasty is to relieve pain, restore movement, and maintain the stability of the affected joint. Designing an effective replacement arthroplasty

Minimally Invasive Transforaminal Lumbar Interbody Fusion for Osteogenesis Imperfecta: A Case Report.

This case report describes the use of minimally invasive transforaminal lumbar interbody fusion (MIS-TLIF) in a patient with osteogenesis imperfecta (OI). A 59-year-old woman with significant spinal stenosis and anterolisthesis at L4-L5 underwent MIS-TLIF due to a lack of improvement with conservative measures. Preoperative planning was key, as the patient's dual-energy X-ray absorptiometry scan revealed osteopenia. The procedure was completed without intraoperative or postoperative complications; nerve root decompression resulted in significant symptomatic improvement. At 1-year follow-up, th

An Unexpected Journey: Migration of a Scoliosis Rod to the Groin.

Rod migration is a rare but potentially serious late complication of spinal instrumentation. A case of long-distance rod migration to the groin following anterior lumbar fusion for adolescent idiopathic scoliosis is presented. A 43-year-old female, who had undergone selective anterior lumbar fusion 20 years earlier, presented with progressive lumbar pain. Initial radiographs demonstrated screw loosening and rod dislodgment. Before the planned revision surgery, the patient re-presented with pain localized to the hip and groin. Imaging revealed distal migration of the rod toward the anterior asp

The CY 2027 Medicare Physician Fee Schedule and the Continuing Struggle to Balance Cost, Quality, Access, and the Survival of Interventional Pain Management.

BACKGROUND: On July 16, 2026, the Centers for Medicare and Medicaid Services (CMS) released the Calendar Year (CY) 2027 Medicare Physician Fee Schedule (PFS) Proposed Rule (CMS-1848-P). The rule arrives one year after the CY 2026 final rule institutionalized two structural changes of unusual consequence for procedural specialties: a recurring efficiency adjustment that reduces work relative value units (RVUs) for non-time-based services, and a redesign of the indirect practice expense (PE) methodology that shifts payment away from facility-based procedural care toward office-based evaluation a

Impact of diabetic kidney disease on perioperative outcomes after lumbar spinal fusion: a propensity score analysis of the Nationwide Inpatient Sample.

BACKGROUND: Diabetic kidney disease (DKD) is a common complication of diabetes mellitus and an important determinant of perioperative outcomes. However, its impact on lumbar spinal fusion (LSF) has not been well established. METHODS: We retrospectively analyzed 242,415 LSF patients from the National Inpatient Sample (2015-2019), identifying 5,278 with DKD. Propensity score matching (1:2) was performed, and multivariable logistic regression assessed the association between DKD and perioperative complications across six organ-system categories, as well as hospital outcomes. Subgroup analyses co

Three-dimensional printed artificial vertebral body versus titanium mesh cage and nanohydroxyapatite/polyamide-66 cage in anterior cervical corpectomy and fusion: a retrospective cohort study with a minimum 5-year follow-up.

BACKGROUND: Anterior cervical corpectomy and fusion (ACCF) is a commonly performed procedure for cervical spondylotic myelopathy (CSM) and ossification of the posterior longitudinal ligament (OPLL), but the choice of reconstructive device after corpectomy remains unsettled. Titanium mesh cages (TMCs) and nanohydroxyapatite/polyamide-66 (n-HA/PA66) cages are both widely used in clinical practice, although subsidence and postoperative loss of cervical alignment remain important concerns. Three-dimensional printed artificial vertebral bodies (3DP-AVBs) have been introduced as an alternative recon

Functional neurological disorder is associated with postoperative complications after ACDF: a retrospective propensity-matched analysis.

INTRODUCTION: Psychiatric comorbidities are associated with worse postoperative outcomes after cervical spine surgery, yet the impact of Functional Neurological Disorder (FND), a neuropsychiatric condition characterized by involuntary neurologic symptoms, has not been investigated. The objective of this study was to evaluate the association between FND and perioperative and long-term complications following anterior cervical discectomy and fusion (ACDF). METHODS: The TriNetX United States Collaborative Network was used to examine adults (≥18 years) undergoing primary ACDF between 2004 and 202

Posterior Facet Distraction and Fusion Technique: Driving a Paradigm Shift in the Surgical Algorithm for Atlantoaxial Dislocation.

BACKGROUND AND OBJECTIVES: Atlantoaxial dislocation (AAD) remains one of the most technically challenging disorders of the craniovertebral junction. Traditional combined anterior-posterior approaches, although effective, carry substantial morbidity. The posterior facet distraction and fusion (PFDF) technique achieves reduction through sequential facet release and distraction, offering a safer posterior-only alternative. This single-center retrospective study aimed to evaluate the reduction efficacy, clinical outcomes, and safety of a PFDF-centered surgical strategy, and to develop and validate

Tibiotalocalcaneal Arthrodesis: Deformity Correction in Medically Comorbid Patients.

BACKGROUND: Tibiotalocalcaneal (TTC) arthrodesis is a common orthopaedic procedure used to manage end-stage pathologies of the ankle and hindfoot. A variety of surgical techniques exist to accomplish TTC arthrodesis. We retrospectively reviewed and compared postoperative outcomes of 2 different TTC arthrodesis constructs, including intramedullary nail (IMN) and open plating (OP) with a plate/screw construct, in high-risk patients with a wide range of deformity and comorbidities. METHODS: A retrospective review was conducted on 73 ankles of 69 consecutive patients who underwent TTC arthrodesis

Full Tibialis Anterior Muscle Turnover Flap for Distal Tibial Coverage in a High-Risk Patient With Ankle Arthrodesis: A Case Report.

Distal tibial wounds with exposed bone or hardware pose a reconstructive challenge, particularly in medically high-risk patients unsuitable for free flap reconstruction. The tibialis anterior muscle is rarely used for reconstruction because sacrifice of the muscle may result in loss of ankle dorsiflexion. However, in selected patients with ankle arthrodesis, complete tibialis anterior sacrifice may be more acceptable because active ankle dorsiflexion is no longer essential. This case report describes a 66-year-old woman with significant cardiopulmonary comorbidity who developed anterior distal

Meyer Type III Vertical Atlantoaxial Distraction with Complete Ligamentous Disruption: A Case Report and Surgical Management.

BACKGROUND: Vertical atlantoaxial distraction (AAD) represents a rare and severe form of craniocervical injury caused by high-energy trauma. It involves vertical separation of the atlas and axis due to complete rupture of the ligamentous stabilizers and poses an immediate risk of neurological or vascular compromise. CASE DESCRIPTION: A 33-year-old male sustained multiple injuries following a suicidal jump from 10 m, including a Type III vertical AAD with 6 mm distraction between the C1 and C2 lateral masses. Imaging revealed complete rupture of the alar, apical, and posterior longitudinal lig

Optimized Graph Structures for Calibrating Graph Neural Networks with Out-of-Distribution Nodes.

BACKGROUND: Bone autograft options may be limited in revision spinal fusion cases. Reconstruction of the iliac bone graft (IBG) donor site with cancellous bone allograft allows for reharvest for patients who subsequently have another fusion. This study examined viability of the reconstructed IBG (RIBG) donor sites. Secondarily, we assessed the feasibility of whether the RIBG sites could be reharvested for obtaining a successful arthrodesis for a secondary fusion. METHODS: Prospectively collected data of 154 consecutive lumbar revision fusions were reviewed, of which 17 cases had their IBG don
