The Impact of an Anterior Lumbar Interbody Fusion on Coronal Alignment in Adult Spinal Deformity Surgery.

BACKGROUND AND OBJECTIVES: This study examined the impact of anterior lumbar interbody fusion (ALIF) on coronal alignment, before and after the placement of posterior instrumentation, and the changes in coronal alignment based on patient positions in adult spinal deformity surgery. METHODS: A total of 65 consecutive adult spinal deformity patients with scoliosis presenting with a main curve cobb angle ≥20° undergoing thoraco-lumbar fusion with 1-2 levels of ALIF performed at L5-S1 or L4-S1 were retrospectively reviewed. L4 tilt, L5 tilt, and fractional curve (FC) cobb angle were measured to a

Sex-related differences in pain following posterior spinal fusion for adolescent idiopathic scoliosis: a retrospective cohort study.

PURPOSE: While opioids are commonly prescribed for postoperative pain management, their use is recommended with caution in pediatric populations. In adult populations, patient sex has been identified as a predictor for increased opioid use, and understanding sex-related differences in postoperative pain is important for optimizing analgesia safety in adolescents undergoing posterior spinal fusion for idiopathic scoliosis. METHODS: A single-institution retrospective cohort study was conducted on patients < 18 years who underwent corrective surgery for adolescent idiopathic scoliosis (AIS). Var

Treatment of iatrogenic vertebral artery injury during C1-C2 arthrodesis with a covered stent.

Iatrogenic vertebral artery injury (VAI) has been described in the context of both anterior and posterior approaches to the cervical spine.1 2 Endovascular treatment of VAI primarily entails vessel sacrifice or vascular reconstruction. The latter option offers the advantage of achieving hemostasis while maintaining parent vessel patency.3 In this video (video 1) we describe the case of a patient in their 20s undergoing C1-C2 arthrodesis with Harms technique. A V3 segment VAI was successfully treated with two overlapping covered stents which provided hemostasis and parent vessel reconstruction.

Lumbar, thoracic, and bilateral vertebral body tethering: do surgical outcomes differ?

PURPOSE: VBT is increasingly being used in the lumbar spine to preserve mobility. Surgeons have noted differences in complications and outcomes among single thoracic (T), single lumbar (TL/L), and bilateral (B) VBT, but evidence has been limited to small, separate case series. We aim to comprehensively assess complications and outcomes of vertebral body tethering (VBT) for adolescent idiopathic scoliosis (AIS) in the pediatric population. We hypothesize that the three approaches will have similar curve correction and surgical outcomes, and rates of major complications and reoperations. METHOD

Vertebral bone quality in patients with adolescent idiopathic scoliosis.

INTRODUCTION: In adolescent idiopathic scoliosis (AIS), the choice of upper and lowest instrumented vertebrae and related complications remain debated. Bone quality is a relevant factor in this context. The vertebral bone quality (VBQ) score has recently been proposed as a radiation-free magnetic resonance imaging (MRI)-based measure of bone quality. However, the role of the VBQ score in AIS and its relationship with the severity of scoliosis characteristics has not yet been explored. We hypothesize that the VBQ score is associated with key features of AIS, including curve type, Cobb angle, lu

Beyond clinical outcomes: a Reddit analysis of patient sentiment and experiences with sacroiliac joint dysfunction and fusion.

BACKGROUND CONTEXT: Sacroiliac (SI) joint dysfunction is a common cause of chronic low back and buttock pain, with SI joint fusion serving as a surgical intervention for patients who fail conservative management. While clinical studies provide patient-reported outcome measures (PROMs), real-world patient experiences, satisfaction, and complications remain underreported. Social media platforms are a vast database of patient narratives, and Reddit offers patients an anonymous support network. PURPOSE: To analyze sentiment and thematic trends in Reddit posts related to SI joint dysfunction and h

A Synergistic Patient-Specific Approach for Enhanced Spinal Fixation Using a Novel Flexible Pedicle Screw and a Complementary Steerable Drilling Robotic System.

OBJECTIVE: Current spinal fixation (SF) techniques face screw loosening and pullout challenges in osteoporotic patients. This can be attributed to conventional rigid pedicle screws (RPS) being forced to fixate along a constrained linear trajectory into low bone mineral density (BMD) areas of the vertebral body. This study proposes a synergistic patient-specific approach that integrates a steerable drilling robotic system with a novel Flexible Pedicle Screw (FPS) to enhance SF procedures by enabling curved screw fixation. METHODS: A patient-specific framework and synergistic design flowchart w
