Operative management of an extensive spinal epidural abscess.

Spinal epidural abscess (SEA) is a serious infection of the central nervous system that typically presents with midline back pain, fever and neurological deficits. We report a rare case of an extensive SEA extending from C2 to S1. The patient presented with neck pain, fever and progressive neurological deficits. MRI revealed a long-segment epidural abscess spanning the cervical, thoracic, lumbar and sacral spine. Surgical intervention involved posterior spinal instrumentation and fusion from C6 to T8. Intraoperatively, solidified abscess material was found encasing the spinal canal between C6

Cervical disc arthroplasty has higher preoperative costs than anterior cervical discectomy and fusion: a retrospective analysis.

PURPOSE: To assess and compare the preoperative healthcare costs associated with single-level cervical disc arthroplasty (CDA) and anterior cervical discectomy and fusion (ACDF), with additional evaluation across cost categories and insurance types. METHODS: Patients who underwent CDA or ACDF between 2010 and 2022 were queried from the PearlDiver Mariner database. Preoperative expenditures - including office visits, imaging (X-ray, MRI, CT), opioid prescriptions, steroid injections, and physical therapy - were quantified. Patients were stratified by insurance type, and statistical comparisons

The prognostic value of preoperative pulmonary function tests in adolescent idiopathic scoliosis surgery.

PURPOSE: The purpose of this study is to evaluate whether preoperative pulmonary function tests (PFTs) can predict the need for postoperative intensive care following adolescent idiopathic scoliosis (AIS) surgery, and to explore their associations with postoperative respiratory support, transfusion requirement, inotropic support, duration of hospital stay and scoliosis severity. METHODS: In this retrospective cohort study, we included 202 patients who underwent posterior spinal fusion for AIS between 2019 and 2024. Patients were grouped by their preoperative PFT forced vital capacity (FVC) Z-s

Risk factors for cage subsidence in patients undergoing cervical fusion: a systematic review and meta-analysis.

We conducted a systematic search of the literature in PubMed, EMBASE and the Cochrane Library until February 27, 2024. We also performed a manual search, including screening reference lists of previous systematic reviews and meta-analyses, to retrieve additional relevant articles for analysis. A random effects model was used to estimate pooled effects. On the basis of sample size, the P value of the Egger test, and interstudy heterogeneity, observational study evidence was classified as high quality (class I), medium quality (class II or III), or low quality (class IV). Sensitivity analyses al

Pelvic fixation and spinopelvic realignment accelerate hip osteoarthritis following long-segment fusion: a 5-year retrospective study.

BACKGROUND CONTEXT: Spinopelvic fixation is commonly used to reduce the risk of distal adjacent segment disease in adult spinal deformity surgery. However, it may impose increased stress on the hip joint, potentially leading to hip osteoarthritis (OA). The concept of adjacent joint disease has been proposed to describe hip joint degeneration due to compensatory overload following rigid spinopelvic fixation, but longitudinal data supporting this are lacking. PURPOSE: To investigate the prevalence and risk factors for the progression of hip OA after spinal fusion surgery, and to evaluate adjace

Migration of Spinal Hardware Into the Popliteal Vein: A Case Report.

CASE: A 41-year-old woman with prior T12-L4 instrumentation for adolescent lumbar scoliosis presented with left posteromedial thigh pain initially diagnosed as an adductor strain. Two years later, she re-presented with knee stiffness, lower leg mottling, and paresthesia suspected to be reflex sympathetic dystrophy. Radiographs later revealed a 13.6-cm radiopaque object in the popliteal fossa and rod absence from the lumbar spine. The spinal rod was removed from the popliteal vein by orthopaedic surgery. CONCLUSION: Radiographs are an easily obtained, low-cost, and high-yield resource that are

Epidemiological Trends and Projections of Adolescent Idiopathic Scoliosis Patients Among 118 Thousand Patients.

INTRODUCTION: Suspected changes in adolescent idiopathic scoliosis (AIS) demographic distribution compared with historical cohorts emphasize the need for an updated understanding of epidemiological trends. We aimed to characterize AIS trends in the United States from 2016 to 2023, compare these findings with historical cohorts, and project trends through 2030. METHODS: AIS cases from 2016 to 2023 were identified using the TriNetX US Collaborative platform. A subanalysis examined patients who underwent posterior spinal fusion (PSF) and cases during the COVID-19 pandemic. Primary outcomes inclu

Cost-effectiveness of early surgical intervention for asymptomatic cervical spinal cord compression:a Markov model analysis.

BACKGROUND CONTEXT: Asymptomatic cervical spinal cord compression (ACSCC) is common in aging populations and can progress to degenerative cervical myelopathy (DCM). The management of ACSCC remains controversial, with uncertainty about whether early prophylactic surgery provides sufficient benefit to justify its risks and costs. PURPOSE: To evaluate the cost-effectiveness of early anterior cervical discectomy and fusion (ACDF) compared to observation in patients with ACSCC and to identify the progression rate threshold at which early surgery becomes economically favorable. STUDY DESIGN/SETTIN

A Novel Palmar Approach for Arthroplasty of the Distal Interphalangeal Joint.

Degenerative changes of the distal interphalangeal (DIP) joint are often managed surgically with arthrodesis or silicone implant arthroplasty when conservative treatments fail. Conventional approaches for silicone implant arthroplasty require division of the extensor tendon, leading to prolonged immobilisation and potential extension lag. We introduce a novel palmar flexor tendon splitting approach. This approach allows silicone implant arthroplasty while preserving the extensor mechanism, eliminating the need for post-surgical immobilisation and permitting early range of motion exercise. We p

Differential Risk Factors for Proximal Junctional Kyphosis Between T8-T10 and T11-L1 Upper Instrumented Vertebrae in Adult Spinal Deformity.

BACKGROUND AND OBJECTIVES: Proximal junctional kyphosis (PJK) remains a persistent mechanical complication after adult spinal deformity surgery. While lower thoracic upper instrumented vertebrae (UIV; T8-L1) are frequently selected, some studies have treated them as a single group, potentially obscuring sublevel-specific risk profiles. Therefore, this study aims to compare risk factors for PJK between patients undergoing lower thoracic fusion with UIV at T8-T10 and those with UIV at T11-L1. METHODS: A retrospective review was performed of 334 adult spinal deformity patients (mean age, 69.6 ye

Cadaveric evaluation of distal tibial autograft harvest: Can we optimize bone harvesting?

BACKGROUND: Autogenous bone grafting is a useful tool for the foot and ankle surgeon to augment their arthrodesis procedures. Distal tibial graft has been shown to be increasingly useful with the benefit to those with limited scope of practice while maintaining low morbidity. While techniques of harvest are surgeon dependent, no study has assessed optimal anatomic location of obtaining distal tibial graft. PURPOSE: The primary goal of this study was to evaluate zones of distal tibial graft harvest to assess the location of optimal graft volume while also identifying structures at risk through

Relationship between preoperative supine radiographs and intraoperative coronal spinal alignment in thoracic adolescent idiopathic scoliosis.

Significant variability exists among surgeons regarding planning methods for adolescent idiopathic scoliosis (AIS) surgery. Lenke et al. advocated the use of preoperative supine full-spine radiographs, but their accuracy in predicting intraoperative alignment remains debated. This study aimed to compare preoperative supine radiographs with intraoperative prone radiographs during AIS posterior fusion. A total of 139 patients undergoing AIS posterior fusion at a single center were prospectively studied. Frontal radiographic parameters from preoperative supine full-spine films were compared with

Leveraging automated machine learning to benchmark, deconstruct, and compare frailty indices for predicting adverse spinal surgery outcomes.

Frailty indices, composite summary scores of diagnostic codes, labs, or vitals, are widely used to assess patient vulnerability. However, their value in predictive modeling remains uncertain. In a cohort of 2,912 elective lumbar spinal fusions, we retrospectively evaluated two established indices, the Hospital Frailty Risk Score (HFRS) and the electronic Frailty Index (eFI), against a curated set of 63 clinical features from electronic medical records. Using automated machine learning optimized logistic regression models, we compared predictive performances and feature importances of frailty i
