Capsulectomy at Explant: Rethinking Spinal Cord Stimulator Infected Generator Pocket Management in the Immunocompromised-A Case Series.

BACKGROUND: Surgical site infections (SSI) associated with spinal cord stimulation (SCS) implants pose risks of morbidity, mortality, and increased healthcare costs. Immunocompromised patients are especially vulnerable due to impaired immune responses, delaying diagnosis and increasing complications. Current guidelines recommend SCS explantation for deep or inadequately treated superficial SSI, though the role of capsulectomy remains underexplored despite its use in other fields like plastic surgery and cardiology. CASE PRESENTATION: We present two cases of immunocompromised patients with SSI

Association of postlaminectomy syndrome with cardiovascular events in patients undergoing posterior lumbar fusion.

BACKGROUND: Postlaminectomy syndrome (PLS) is a chronic pain condition characterized by persistent or recurrent symptoms after lumbar spine surgery. Although chronic pain states have been linked to autonomic dysregulation, inflammation, and increased cardiometabolic burden, the long-term cardiovascular implications of PLS remain unclear. Posterior lumbar fusion (PLF) is one of the most common lumbar procedures, providing a clinically relevant population in which to evaluate whether PLS signals broader cardiovascular vulnerability. METHODS: We conducted a retrospective cohort study using the T

Non-tobacco nicotine dependence and incidence of pseudarthrosis and other postoperative complications in lumbar fusion surgery: a retrospective analysis.

INTRODUCTION: Lumbar fusion is a common procedure to help treat trauma, deformities, tumors, infections, and degenerative diseases of the spine by improving mechanical stability. The rate of non-tobacco nicotine, also known as e-cigarette, use among US adults has recently been increasing. This study seeks to take a collective look at fusion outcomes to address this problem by quantitatively evaluating the effects of non-tobacco nicotine dependence (NTND) on postoperative outcomes following lumbar fusion surgery. METHODS: A retrospective cohort study using the TriNetX database identified patie

Ventral Column Spinal Cord Stimulation for Pain Management: Mechanisms, Clinical Evidence, and Future Directions.

Spinal cord stimulation (SCS) is a form of neuromodulation that delivers electrical impulses to the spinal cord, altering pain signals to provide relief from chronic pain. Since its introduction in the 1960s, SCS has primarily targeted the dorsal columns of the spinal cord. While SCS has proven effective for many neuropathic pain conditions, its limitations have driven exploration of alternative anatomical targets for electrical stimulation. Work in the 1970's suggested that ventral column spinal cord stimulation (VC-SCS) might offer superior pain relief by targeting the spinothalamic tracts (

Structural and Functional Treatment of Persistent Spinal Pain Syndrome Type 2: A Fibrosis-Stratified Network Meta-Analysis of Epidural Adhesiolysis and Neuromodulation.

Background/Objectives: Persistent spinal pain syndrome type 2 (PSPS-T2) is treated with two mechanistically distinct interventional families: epidural adhesiolysis (EA), directed at structural epidural pathology, and neuromodulation (NM), directed at modulation of nociceptive signaling. Epidural fibrosis is a biologically plausible candidate for treatment stratification, but whether it independently modifies treatment response remains unproven. This study reviewed the available evidence to determine both the comparative findings and the limitations imposed by the current evidence architecture.

Uncommon indications in chronic pain for intrathecal drug delivery systems: a narrative review of the literature.

Intrathecal drug delivery systems (IDDS) have been used successfully for a number of refractory chronic pain conditions. The most common pain indications for the use of IDDS include cancer-related pain and postlaminectomy syndrome. IDDS can also be used to treat a variety of other less common refractory pain conditions, including central neuropathic pain, peripheral neuropathic pain and craniofacial pain. The aim of this article was to review the existing literature on the efficacy and clinical utility of IDDS for the management of uncommon pain indications, including spinal cord injury-relate

Structural basis for Mg²⁺-glycine synergy in the tandem glycine riboswitch revealed by cryo-EM.

Part I of this article discussed the potential functional importance of genetic mutations and alleles of the human cytochrome P450 2D6 (CYP2D6) gene. The impact of CYP2D6 polymorphisms on the clearance of and response to a series of cardiovascular drugs was addressed. Since CYP2D6 plays a major role in the metabolism of a large number of other drugs, Part II of the article highlights the impact of CYP2D6 polymorphisms on the response to other groups of clinically used drugs. Although clinical studies have observed a gene-dose effect for some tricyclic antidepressants, it is difficult to establ

The efficacy of pulsed radiofrequency ablation of the dorsal root ganglion in the setting of failed spinal neuromodulation: A case report.

Radiofrequency ablation (RFA) is an interventional procedure that has been used to treat chronic back pain for over 50 years; this unique case report demonstrates the effectiveness of pulsed radiofrequency ablation (PRFA) on the dorsal root ganglion (DRG) in the treatment of chronic radicular pain (Russo et al., 2021, J Pain Res, 14, 3897). The RFA provides pain relief by using thermal energy to disrupt peripheral nerves carrying nociceptive signals back to the central nervous system (Abd-Elsayed et al. 2020, Pain Ther, 9, 709). However, there is a lack of literature about the safety and effic

[Epidurolysis - an old-new procedure].

Epidural adhesions may be caused by inflammation of the surrounding spinal canal, disc degeneration (nucleus pulposus entering the epidural space as a result of damage to the disc), or scarring following spinal surgery (failed back surgery). The roots become stuck due to adhesions, which hinder their mobility. Furthermore, the richly innervated posterior longitudinal ligament and the dura mater are inhibited from “sliding” on each other. These patients develop chronic radicular and/or low back pain. The range of conservative treatments available is often limited, and the quality of life of pat

GLP-1 receptor agonist use in elective lumbar spine surgery: Reduced pseudarthrosis rates and favorable safety profile.

BACKGROUND: Obesity and diabetes mellitus are well documented to to elevate perioperative risks in spine surgery; hence, glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have gained popularity due to their efficacy in improving glycemic control and weight loss. However, their impact on lumbar spine-specific outcomes remains underexplored. This study evaluates associations between preoperative GLP-1 RA use and postoperative outcomes in patients undergoing elective lumbar spine surgery. METHODS: A retrospective cohort analysis of 5722 patients undergoing elective lumbar spine surgery was c
