Impact of Nontobacco Nicotine Dependence on Outcomes Following Anterior Lumbar Interbody Fusion.

STUDY DESIGN: Retrospective cohort study. OBJECTIVE: To compare rates of health care utilization, complications, opioid use, and revision surgery following anterior lumbar interbody fusion (ALIF) in patients with versus without nontobacco nicotine dependence (NTND). SUMMARY OF BACKGROUND DATA: The prevalence of NTND is rising in the United States due to products such as nicotine e-cigarettes, pouches, gums, and lozenges. Studies have shown that tobacco use can contribute to adverse outcomes following spinal fusion surgery, including pseudarthrosis. However, literature focusing specifically o

Revision of an Eight-lead Boston Scientific® Spinal Cord Stimulator With Epidural Migration: A Case Report.

Spinal cord stimulation (SCS) is a minimally invasive neuromodulation technique utilized to manage chronic pain conditions such as complex regional pain syndrome (CRPS), post-laminectomy syndrome, and peripheral neuropathy. Despite its efficacy, complications, including lead migration, discomfort, and device malfunction, can occur. We present the case of a male patient in his 60s with a history of chronic left lower extremity pain who underwent SCS implantation. Initial management included a successful trial period followed by permanent implantation. However, postoperative imaging revealed lea

The Use of Semaglutide in Patients Undergoing Lumbar Fusion Does not Increase 90-Day Medical or 1-Year Implant Complications.

STUDY DESIGN: Retrospective Cohort Study of National Database. OBJECTIVE: This study examines their effect on medical and mechanical complications within 90 days postlumbar spine surgery. SUMMARY OF BACKGROUND DATA: Patients undergoing spinal procedures increasingly use glucagon-like peptide-1 receptor agonists (GLP-1 RAs), originally for type 2 diabetes and now popular for weight loss. The impact of GLP-1 RAs on spinal fusion outcomes is unknown. METHODS: This study used medical records from TriNetX, a national deidentified database, to examine diabetic patients undergoing lumbar spine pro

Factors Associated With Same Day Discharge Post-Spinal Cord Stimulator Placement.

BACKGROUND: Spinal cord stimulator (SCS) surgeries, whether performed using the open or percutaneous approach, are becoming increasingly common for a range of neuropathic pain conditions, including post-laminectomy syndrome and complex regional pain syndrome. However, there is limited knowledge regarding the factors linked to same-day discharge patterns following SCS. OBJECTIVE: The purpose of this study was to identify factors associated with same-day discharge after SCS placement. The primary outcome was same-day discharge. STUDY DESIGN: Retrospective, cohort study using a nationwide datab

Clinical and Radiographic Outcomes of Anterior Lumbar Interbody Fusions Using a Titanium Cage with a Biomimetic Surface.

BACKGROUND: We analyzed clinical and radiographic outcomes in patients undergoing anterior lumbar interbody fusions (ALIFs) using a new biomimetic titanium fusion cage (Titan nanoLOCK interbody, Medtronic, Minneapolis, Minnesota, United States). This specialized cage employs precise nanotechnology to stimulate inherent biochemical and cellular osteogenic reactions to the implant, aiming to amplify the rate of fusion. To our knowledge, this is the only study to assess early clinical and radiographic results in ALIFs. METHODS: We conducted a retrospective review of data for patients who underwe

Prior Hypermobile Spinal Cord Stimulator Removal With Difficult Reimplantation due to Epidural Scarring Provides Relief in Postlaminectomy Syndrome.

BACKGROUND: When spinal cord stimulation (SCS) therapy fails for postlaminectomy syndrome (PLS), oftentimes the device is not removed or explanted, or rarely, it is reimplanted with the hopes of success with a new implant. CASE REPORT: Our patient is a 52-year-old man with a history of PLS treated with L5-S1 discectomy who presented with refractory chronic low back pain. He underwent an initial SCS implant with significant pain relief, but was limited due to unwanted abdominal stimulations with movement and explanted 8 months later. Despite his prior experience, a second SCS was implanted but

6. Persistent spinal pain syndrome type 2.

INTRODUCTION: Persistent Spinal Pain Syndrome (PSPS) refers to chronic axial pain and/or extremity pain. Two subtypes have been defined: PSPS-type 1 is chronic pain without previous spinal surgery and PSPS-type 2 is chronic pain, persisting after spine surgery, and is formerly known as Failed Back Surgery Syndrome (FBSS) or post-laminectomy syndrome. The etiology of PSPS-type 2 can be gleaned using elements from the patient history, physical examination, and additional medical imaging. Origins of persistent pain following spinal surgery may be categorized into an inappropriate procedure (eg a

External Oblique Intercostal Plane Block: A Case Report and Review of Literature.

Chronic low back pain (CLBP) and post-laminectomy syndrome (PLS) can pose significant therapeutic challenges, often refractory to conservative management. We present a case of a 52-year-old male with refractory CLBP and PLS who underwent spinal cord stimulation (SCS) lead placement, and subsequently developed chronic right anterior chest wall and upper abdominal pain. Despite using SCS and opioid therapy, the pain persisted until an ultrasound-guided external oblique intercostal plane block (EOIPB) was administered, resulting in complete pain relief. This case highlights the efficacy of EOIPB

Single Institutional Cross-Sectional Phone Survey Study: Evaluation of Causes for Loss to Follow-up After Spinal Cord Stimulator Implantation.

BACKGROUND: Spinal cord stimulation (SCS) is often an option of last resort for patients with post-laminectomy syndrome or an alternative option for patients with complex regional pain syndrome, chronic nonsurgical low back pain, or painful diabetic peripheral neuropathy when conservative management has failed. Although SCS is a helpful option, it is not without complications that can frequently lead to explantation of the SCS device and dissatisfaction with the treatment. Furthermore, as with any technology, SCS has potential issues that may lead to patient frustration and ultimately result i

Targeting IL-20 alleviates inflammatory mechanical allodynia and reduces epidural fibrosis in post-laminectomy syndrome rat model.

BACKGROUND: Post-laminectomy syndrome (PLS) manifests as recurrent chronic back pain, with or without radiating leg pain, affecting 10-40% of patients following laminectomy. While surgical interventions can alleviate recurrent disc herniation or joint instability, medical management of PLS remains challenging due to unsatisfactory outcomes. Epidural fibrosis is a frequent cause of PLS, leading to nerve root tethering and dural sac compression. This study aimed to investigate the role of interleukin-20 (IL-20) in the development of epidural fibrosis and the pathogenesis of PLS. METHODS: A rat

Intraoperative Findings of Pathologic Calcification Surrounding an Intrathecal Pain Pump: A Case Report.

BACKGROUND: Pathologic calcification is a common complication of surgically implanted devices, increasing the incidence of subsequent infection. Most literature describing this phenomenon occurs with cardiac implanted devices. Documented reports of this complication, specifically in the setting of intrathecal pain pump (ITP) and spinal cord stimulator (SCS) implants, are minimal, with presumably low incidence. CASE REPORT: We present a case of a 58-year-old healthy man with a history of motorcycle accident status post L4-S1 fusion complicated by post-laminectomy syndrome. In February 2023, th

The Association of Psychiatric Comorbidities With Short-Term and Long-Term Outcomes Following Spinal Cord Stimulator Placement.

BACKGROUND: Outcomes after spinal cord stimulator (SCS) placement are affected by psychologic comorbidities. It is part of routine practice to do psychologic assessments prior to SCS trials to assess for the presence of maladaptive behavioral patterns. However, few studies have sought to quantify the effect of psychiatric comorbidities on complications, reoperation, and readmission rates. The purpose of this study was to assess the association of psychiatric comorbidities with postprocedural outcomes after SCS implantation. MATERIALS AND METHODS: Inclusion criteria included SCS placement betw
