Spinal cord stimulation (SCS) is an emerging technology to treat chronic pain from complex regional pain syndrome (CPRS) neuropathy and post-laminectomy syndrome. A rarely reported postoperative complication of SCS paddle implantation is abdominal pain that can result from thoracic radiculopathy. Ogilvie's syndrome (OS) is a disorder characterized by acute dilatation of the colon in the absence of an anatomic lesion that obstructs the flow of intestinal contents, which has seldom been observed after spine surgery. Here, we describe the case of a 70-year-old male who developed OS after SCS padd

OBJECTIVE: In 2020, Mekhail et al published a formula that predicted the likelihood of a successful outcome for those who undergo spinal cord stimulation (SCS) for long-term pain management, based on retrospectively collected clinical and demographic data from one major medical center. The aim of this study is to validate such a predictive formula, prospectively, in a cohort of patients from multiple medical practices that are more representative of real-life clinical practice.
MATERIALS AND METHODS: For the study, 939 patients who underwent successful SCS or targeted drug delivery (TDD) tria

Spinal cord stimulation is an increasingly used treatment for a number of chronic pain states. Dorsal column stimulation is historically and currently the anatomical target of choice for most chronic pain conditions, including postlaminectomy syndrome and radicular pain. However, early studies suggested that stimulation of an alternative target, the ventral columns that carry pain fibers in the anterior and lateral spinothalamic tracts, may offer comparable or superior pain relief. A patient undergoing standard-of-care spinal cord stimulation trial had an additional lead placed on the right ve

BACKGROUND: Peripheral nerve stimulation (PNS) may be an effective treatment for neuropathic pain syndromes of the extremities, even after conventional neuromodulation techniques have failed.
CASE REPORT: A 71-year-old man with a history of postlaminectomy syndrome presented with chronic left-sided leg pain after receiving no relief after undergoing multiple neuromodulation modalities. The patient subsequently underwent a successful sciatic nerve PNS trial, followed by implantation, achieving 90% pain relief after 6 weeks.
CONCLUSION: We describe a case where PNS was successful in treating c

BACKGROUND: Scar tissue can form within the epidural space following surgical trauma, infection, annular tearing, hematoma formation, and disc herniation. When epidural fibrosis (EF) is present, it can render nerve roots more susceptible to entrapment, compression, and tension-each of which can potentially contribute to low back and radicular pain in the lower extremities. In the past, several different epidural adhesiolysis protocols have been described to treat such pain.
CASE REPORT: Three patients with chronic low back and radicular pain, and evidence of EF underwent a modified epidural a

BACKGROUND: Novel externally powered spinal cord stimulation technology can be fully implanted when trialing the effectiveness of the therapy, since no percutaneous leads are needed, and the trial period lasted 30 days. Multiple tests of different stimulation modalities and parameters are possible, thus improving the chances that the therapy will lead to effective pain reduction.
OBJECTIVES: The objective of this study was to analyze the effectiveness of the Freedom Spinal Cord Stimulator System (Stimwave LLC, Pompano Beach, FL) for the treatment of failed back surgery syndrome due to postlam

IMPORTANCE: The results of studies evaluating spinal cord stimulation (SCS) for postlaminectomy syndrome (PLS) have yielded mixed results. This has led to an increased emphasis on objective outcome measures such as opioid prescribing.
OBJECTIVE: To determine the association between SCS and long-term opioid therapy (LOT) for PLS.
DESIGN, SETTING, AND PARTICIPANTS: In this cohort study, adults with PLS were identified using the TriNetx Diamond Network and separated based on whether they underwent SCS. Patients were stratified according to baseline opioid use (opioid-naive or receiving LOT) and

BACKGROUND: Epidural fibrosis (EF) is one of the leading causes of post lumbar surgery syndrome (PLSS). Although there are studies in the literature suggesting that lumbar epidural steroid injections are an effective method in the pain management of PLSS caused by EF, no study is available comparing the effectiveness and safety of caudal and transforaminal approaches.
OBJECTIVES: To investigate the efficacy of caudal epidural steroid injection (CESI) versus transforaminal epidural steroid injection (TFESI) in patients with PLSS.
STUDY DESIGN: A prospective, randomized, assessor-blind study.

BACKGROUND: Chronic spinal pain is the most prevalent chronic disease, with chronic persistent spinal pain lasting longer than one-year reported in 25% to 60% of the patients. Health care expenditures have been escalating and the financial impact on the US economy is growing. Among multiple modalities of treatments available, facet joint interventions and epidural interventions are the most common ones, in addition to surgical interventions and numerous other conservative modalities of treatments. Despite these increasing costs in the diagnosis and management, disability continues to increase.

INTRODUCTION: Postlaminectomy syndrome (PLS), also known as failed back surgery syndrome, is the persistence of radicular pain in the face of surgical intervention. Despite its prevalence in 10 to 40% of spine surgery patients, outpatient pharmacologic and interventional management remains poorly characterized.
METHODS: The 2007 to 2016 National Ambulatory Medical Care Survey (NAMCS) was utilized to include all outpatients diagnosed with PLS. For each visit, documented pain medications (opioids, nonsteroidal anti-inflammatory drugs [NSAIDs], neuropathic agents, etc) as well as patient demogra

OBJECTIVE: Spinal cord stimulation is a proven, evidence-based therapy for persistent spinal pain syndrome. While some patients with this disease are managed by chronic pain physicians, many are managed in primary care offices. Despite mounting evidence, dissemination of this research outside of neuromodulation related fields and implementation of this treatment in common practice has not yet occurred. We hypothesize that family medicine residents in rural training programs will have little knowledge of neuromodulation despite it being an evidence-based and common treatment of post-laminectomy

Spinal cord stimulation (SCS) is a commonly utilized therapy for the treatment of neuropathic pain conditions. The primary indications for SCS therapy are post-laminectomy syndrome as well as complex regional pain syndrome. SCS therapy is minimally invasive and reversible. It involves the implantation of percutaneous or paddle leads along with a surgically implanted pulse generator (IPG). Severe pocket pain from the IPG is a well-known complication following SCS implants that can be challenging to treat and can lead to an explant of the SCS system. We present the case of a patient with post-la

BACKGROUND: Failed back surgery syndrome (FBSS) is a significant cause of lumbar disability and is associated with severe patient morbidity. As the etiology of FBSS is not completely elucidated, the risk factors and evaluation of patients with FBSS remains challenging. Our analysis of a wide variety of operation types, clinical setting, and their correlation to FBSS seeks to allow fellow clinicians to be aware of the potential risk factors that leads to this devastating diagnosis.
METHODS: Data were obtained for patients undergoing anterior lumbar fusion, posterior lumbar fusion, or decompres

Adhesiolysis is minimally invasive and commonly used for pain associated with adhesion after lumbar spine surgery. Caudal epidural block may be used for radiating pain due to failed back surgery syndrome. We evaluated the predictive value of response to caudal block performed prior to adhesiolysis in failed back surgery syndrome. Between January 1, 2013 and June 30, 2020, 150 patients with failed back surgery syndrome were treated with adhesiolysis using a steerable catheter at the pain clinic of a tertiary hospital after failed conservative treatment (including caudal block). Patient demograp

Introduction Bertolotti's syndrome (BS) describes the relationship between low back pain (LBP) and lumbosacral transitional vertebra (LSTV). It is a factor that is sometimes overlooked when it comes to evaluating and treating LBP.Because of the different diagnostic modalities and criteria used in the research, the LSTV incidence in the general population varies greatly, and hence the link between LSTV and LBP remains contentious. Some researchers found no link between low back pain and LSTV. As a result, the management of BS remains controversial and multiple treatments have been suggested, in
