BACKGROUND: Cervical laminoplasty is a motion-preserving procedure that addresses spinal cord compression and avoids postlaminectomy kyphosis associated with cervical laminectomy. The most common complications include C5 nerve palsy, axial neck pain, hinge nonunion, and premature closure. Plating is a relatively newer method of laminoplasty fixation that may provide greater stabilization postoperatively and reduce the risk of laminoplasty closure compared with less rigid (e.g., suture) fixation techniques. Although prior studies have reported low rates of laminar/lateral mass screw back out, p

Stand-alone cervical laminectomy for degenerative cervical myelopathy (DCM) has become increasingly rare due to risk of postlaminectomy kyphosis. This article discusses the biomechanics of cervical degeneration and how laminectomy effects spine stability, and summarizes relevant clinical studies to help guide surgical decision-making for the posterior treatment of DCM. Laminectomy and fusion remains a safe and efficacious treatment. Stand-alone laminectomy should only be used for a highly selected patient population with relative stiff lordotic cervical spines, using care to not disrupt facets

OBJECTIVE: Osteotomy through anterior exposure is challenging with severe complications for upper cervical kyphosis (CK), especially for cases with previous anterior fusion. A novel technique comprising 1-stage osteotomy via a posterolateral-only approach is introduced for treatment of CK secondary to C2-4 laminectomy for neurofibroma removal and subsequent anterior fusion.
METHODS: A 42-year-old man presented with progressive numbness and weakness of upper and lower limbs. As an adolescent, he underwent posterior laminectomy and neurofibroma excision without effective fixation and anterior C

Patients with permanent pacemaker posted for cervical spine instrumentation pose special challenges for modern-day anesthesiologist since the field of surgery is in proximity to the pacing apparatus. The important considerations in this regard are pacemaker dependency, prior reprogramming to asynchronous mode, perioperative interference with pacemaker function due to electrolyte, acid-base disturbances, and electromagnetic interference leading to pacemaker failure and hemodynamic compromise. We report successful anesthetic management of a patient of postlaminectomy kyphosis with compressive my

OBJECTIVE Cervical open-door laminoplasty (ODL) is designed to decompress the spinal cord, maintain motion, and prevent postlaminectomy kyphosis. Many traditional laminoplasty techniques involve disruption of the posterior ligamentous structures, most notably the C7-T1 supraspinous ligament and interspinales muscle complex (intraspinous and supraspinous ligaments [ISLs]). METHODS Eight human cervical cadaveric specimens (C2-T1) were used to investigate the subaxial kinematics following ODL with varying degrees of posterior element disruption. Ligamentous integrity was preserved and specimens w

Spinal kyphotic deformity after spinal laminectomy or laminoplasty is more common in pediatric patients than adults. Laminectomy can lead to decreased cartilage growth, anterior wedging and posterior spinal muscle insufficiency which can result in kyphotic deformity. Herein we outline a case report of a child presenting with kyphotic deformity after receiving a spinal laminectomy to treat a penetrating spinal trauma. The 8-year-old male presented with penetrating spinal trauma following a gunshot wound and subsequently underwent L1 laminectomy and thecal sac decompression to remove the foreign

OBJECT: Although laminectomy is an effective surgical technique for the treatment of multilevel cervical stenotic lesions, postoperative kyphosis and neurological deterioration have been frequently reported after laminectomy. Hence, laminectomy without fusion is seldom performed nowadays. However, the clinical impression from the long-term follow-up of patients who had undergone laminectomy does not support that postoperative kyphosis is common in patients with ossification of the posterior longitudinal ligament (OPLL). In this paper, the authors assessed the long-term outcome of laminectomy f

This paper is a narrative review of normal cervical alignment, methods for quantifying alignment, and how alignment is associated with cervical deformity, myelopathy, and adjacent-segment disease (ASD), with discussions of health-related quality of life (HRQOL). Popular methods currently used to quantify cervical alignment are discussed including cervical lordosis, sagittal vertical axis, and horizontal gaze with the chin-brow to vertical angle. Cervical deformity is examined in detail as deformities localized to the cervical spine affect, and are affected by, other parameters of the spine in

OBJECT: Cervical spondylotic myelopathy (CSM) is a common cervical degenerative disease that affects the elderly population. Spinal cord decompression is achieved through various anterior and posterior approaches including anterior cervical decompression and fusion, laminectomy, laminoplasty, and combined approaches. The authors describe another option, minimally invasive endoscopically assisted decompression of stenosis (MEDS), which obviates the need for muscle dissection and disruption of the posterior tension band, a cause of postlaminectomy kyphosis.
METHODS: The authors conducted a retr

OBJECT: Although rarely encountered, pediatric patients with severe cervical spine deformities and instability may occasionally require occipitocervicothoracic instrumentation and fusion. This case series reports the experience of 4 pediatric centers in managing this condition. Occipitocervical fixation is the treatment of choice for craniocervical instability that is symptomatic or threatens neurological function. In children, the most common distal fixation level with modern techniques is C-2. Treated patients maintain a significant amount of neck motion due to the flexibility of the subaxia

BACKGROUND: Benign intradural extramedullary (IDEM) tumours in children are usually neural sheath derivatives or are dura based. Total resection of these benign tumours is the primary surgical goal. A management issue related to their occurrence in the growing paediatric spine is a higher propensity for the development of postlaminectomy kyphosis, especially in the upper cervical spine, than what is seen in adults.
METHODS: A retrospective study was performed on 22 patients to analyse the incidence, presentation and management of postoperative kyphotic deformities in children who underwent la

BACKGROUND CONTEXT: Postlaminectomy kyphosis of the cervical spine is a challenging condition to treat because it has a combination of an exposed cord, progressive kyphosis, segmental instability, and anterior neural compression. The ideal mode of surgical correction remains controversial. In terms of surgical strategy, there are few large series that have reported the long-term results of anterior surgical treatment of this condition.
PURPOSE: This study was designed to determine the long-term results and outcomes of anterior surgical treatment alone for the patients of postlaminectomy cervi

Cervical laminectomy has been known to cause postlaminectomy kyphosis and instability, often necessitating extensive anterior and posterior procedures with instrumentation for the stabilisation and correction of the deformity. We report a case of spontaneous stabilisation of postlaminectomy cervical kyphosis by anterior longitudinal ossification in the absence of an ossified posterior longitudinal ligament, thus circumventing the need for surgical intervention.

OBJECT: Sagittal plane deformities can be subdivided into kyphotic and lordotic forms and further characterized according to their global or regional (focal) presentation. Regional deformities of a significant magnitude constitute a gibbous deformity. Pedicle subtraction osteotomy (PSO) and interlaminar Smith-Petersen osteotomies have been used to correct sagittal plane deformities in the cervical, thoracic, and lumbar spine. By resecting a portion of the vertebral body and closing in the gap of this vertebra, the spine is placed in local lordosis and kyphosis is corrected. These osteotomies h

STUDY DESIGN: A case report and clinical discussion.
OBJECTIVE: To describe an unique case of purulent discitis at C3-C4 and an anterior C1-C4 epidural abscess secondary to oxygen-ozone therapy for the herniated cervical discs.
SUMMARY OF BACKGROUND DATA: Previously reported complications secondary to ozone therapy are rarely documented. No cases of pyogenic discitis with associated epidural abscess after oxygen-ozone therapy have been reported to date. To our knowledge, this is the first such case reported in literature.
METHODS: A 57-year-old female patient with nuchal stiffness was previ

STUDY DESIGN: Retrospective study.
OBJECTIVE: To determine the appropriate distal fusion level in posterior instrumentation and fusion for thoracic hyperkyphosis by investigating the relationship between the sagittal stable vertebra ([SSV]-the most proximal lumbar vertebral body touched by the vertical line from the posterior-superior corner of the sacrum), first lordotic vertebra (just caudal to the first lordotic disc), and selected lowest instrumented vertebra (LIV).
SUMMARY OF BACKGROUND DATA: It has been recommended that the distal end vertebra and the first lordotic disc beyond the tra

Two common complications associated with cervical surgery are malalignment, particularly after multilevel laminectomies, and nonunion. In both situations, prevention and early recognition are critical in clinical management. In postlaminectomy kyphosis, the prevailing cause is an alteration in the normal spinal biomechanics. The exact incidence is unknown; however, this malalignment can cause significant morbidity. Surgical treatment is used once a patient becomes symptomatic. Anterior cervical procedures are frequently used to correct the deformity. Like postlaminectomy kyphosis, most nonunio

We report a case of a skeletally immature achondroplastic adolescent with significant thoracolumbar lordosis who presented with neurogenic claudication and urinary incontinence progressing over a 1-year period. She underwent decompressive lumbar laminectomy from T12 to L5 with preservation of the facet joints. Over the ensuing 4 years of follow-up, she developed a progressive thoracolumbar kyphosis that progressed to 105 degrees. She remained neurologically intact and was fully ambulatory, but had severe back pain in the area of the deformity. Because of the severity and progression of her def
