A patient with postlaminectomy kyphosis with a neurological deficit which developed following the initial surgical treatment is reported. A 49-year-old man, complaining of neck pain, sought treatment in 1995. An extramedullary cervical spinal tumor was diagnosed and C2-C4 laminectomy and resection of the tumor were performed. Recurrence of the tumor was seen 1 year later and a second tumor resection and radiation therapy were performed. One year after the second resection of the tumor, X-rays of the cervical spine revealed kyphosis. Anterior spinal fusion without instrumentation was performed

STUDY DESIGN: Retrospective review.
OBJECTIVES: To determine the risk of postlaminectomy thoracolumbar kyphosis in skeletally immature achondroplasts and evaluate the need for concurrent fusion at multilevel decompression.
SUMMARY OF BACKGROUND DATA: Spinal stenosis is a relatively common complication of achondroplasia. Although most achondroplasts do not develop symptomatic spinal stenosis until the third or fourth decades, some patients become symptomatic before skeletal maturity. While postlaminectomy kyphosis typically does not occur in the adult achondroplast, it is not known if it occu

Postoperative sagittal-plane cervical spine deformities are a concern when laminectomy is performed for tumor resection in the spinal cord. These deformities appear to occur more commonly after resection of intramedullary spinal cord lesions, compared with laminectomy for stenosis caused by degenerative spinal conditions. Postlaminectomy deformities are most common in pediatric patients with an immature skeletal system, but are also more common in young adults (< 25 years of age) in comparison with older adults. The extent of laminectomy and facetectomy, number of laminae removed, location of

The patient presented with neurofibromatosis and a dystrophic kyphoscoliosis around the cervico-thoracic junction. When the patient was 59 years old, he started to suffer from dyspnea caused by an intrathoracic meningocele in the upper left thoracic cavity. A wide laminectomy from T2 to T5 was performed and the meningocele was resected. Although the dyspnoea disappeared postoperatively, the patient started to neurologically deteriorate. Laminectomy alone caused instability around the apex of the kyphosoliosis and spinal cord compression. Halo cast was applied and brought remarkable recovery of

OBJECT: The technique of cervical laminoplasty was developed to decompress the spinal canal in patients with multi-level anterior compression caused by ossification of the posterior longitudinal ligament or cervical spondylosis. There is a paucity of data confirming its superiority to laminectomy with regard to neurological outcome, preserving spinal stability, preventing postlaminectomy kyphosis, and the development of the "postlaminectomy membrane."
METHODS: The authors conducted a metaanalysis of the English-language laminoplasty literature, assessing neurological outcome, change in range

Postlaminectomy cervical kyphosis is an important consideration when performing surgery. Identifying factors predisposing to postoperative deformity is essential. The goal is to prevent postlaminectomy cervical kyphosis while exposing the patient to minimal additional morbidity. When postlaminectomy kyphosis does occur, surgical correction is often required and performed via an anterior, posterior, or combined approach. The authors discuss the indications for surgical approaches as well as clinical results.

STUDY DESIGN: Reconstruction techniques after multilevel anterior cervical decompression were retrospectively compared.
OBJECTIVE: To compare radiographic and clinical outcomes of multiple interbody grafting with strut grafting.
SUMMARY OF BACKGROUND DATA: Previous studies have reported lower fusion rates for anterior cervical decompressions reconstructed with multiple interbody grafts as opposed to a single strut graft, although these techniques have never been directly compared in a consecutive series of patients who underwent surgery by a single surgeon.
METHODS: Over a 20-year period, 1

From 1992 to 1997 a series of 12 multiply operated (averaging 2.5 previous operations) patients with recurrent peridural fibrosis and postlaminectomy kyphosis underwent surgery at our clinic. The surgery was designed to restore the physiological lordosis and relax tethered cord and epidural veins by transpedicular decancellation osteotomy at a vertebra other than the vertebra with peridural fibrosis. This paper presents the long-term functional outcome of these 12 patients. Clinical assessments were conducted pre-operatively and at 3-month intervals postoperatively and included X-ray assessmen

Between 1986 and 1995 10 patients who were 9 to 18 years of age underwent posterior spinal fusion and instrumentation to the pelvis for correction of spinal deformity using the modified sacral bar technique at the authors' institution. Etiologies of the spinal deformity included congenital scoliosis, cerebral palsy, myelomeningocele, neurofibromatosis, and postlaminectomy kyphosis. Indications for pelvic instrumentation were progressive scoliosis of the lower lumbar spine, pelvic obliquity greater than 15 degrees, and dysraphic posterior elements. Five of the patients had prior spinal surgery.

Postlaminectomy kyphosis is a rare but potentially catastrophic complication of laminectomy performed to treat cervical myelopathy. The best strategy for prevention is awareness of the sagittal plane before surgery in patients with cervical spondylotic myelopathy. An understanding of the biomechanical forces placed on the cervical spine, the normal structures contributing to stability and the underlying pathologic process helps in the prevention and ultimate treatment of postlaminectomy kyphosis. This is a review of the risk factors, biomechanics, work-up, and surgical treatment of postlaminec

Authors report on the control examinations of 108 patients on a peduncular system, known since 1986 and applied successfully in the operative treatment of various diseases of the lumbosacral spine. The results of healing of 69 patients in whom the fusion of the lumbosacral spine was performed because of long standing low-back pains are analysed. They describe also other possibilities given by the peduncular system, among others the correction and stabilization of the lumbar kyphosis in Becterew's disease, the postlaminectomy kyphosis and the lumbar scoliosis of adults. Cases of trauma and tumo
