Intrathoracic rib fixation in flail chest: first South American experience.

Intrathoracic rib fixation represents a technical innovation with advantages over traditional methods through minimally invasive approach. We present the first South American case in a 21-year-old patient with flail chest from polytrauma. Four intrathoracic fixation plates (two 90 mm anterior and two 60 mm posterior) were used through video-assisted anterolateral and posterior approaches. The technique facilitates reduction through guide cables, intrathoracic positioning with minimal muscular disruption and automatic adaptation to costal anatomy. Total surgical time was 155 minutes with 250 mL

Bridge over Troubled Tendon? A Novel Case of Heterotopic Ossification of Lister's Tubercle Causing Extensor Pollicis Longus Tenosynovitis.

INTRODUCTION: Distal radius fractures are a common injury in adults, and surgical treatment frequently consists of open reduction internal fixation (ORIF) with a volar plate and screws. Uncommonly, heterotopic ossification (HO) may form as a result of inflammation secondary to trauma in the wrist, and once formed, surgical excision is the only effective treatment. CASE REPORT: A healthy 57-year-old male presents with thumb pain and decreased range of motion 2 years postoperatively of a distal radius ORIF with notable dorsal comminution. Physical examination was significant for pain, weakness,

Immediate Unrestricted Assisted Full Weight-Bearing After Bicondylar Tibial Plateau Fixation: A Case Series.

High-energy bicondylar tibial plateau fractures of Schatzker types V and VI are conventionally managed, after open reduction and internal fixation, with 6-12 weeks of restricted weight-bearing, out of concern for loss of reduction and articular collapse. Emerging evidence questions whether this restriction is necessary. Reports of immediate weight-bearing specifically after fixation of these most unstable bicondylar patterns remain scarce, as most comparative studies pool this subtype within mixed cohorts. We present three consecutive patients (two men and one woman; mean age: 54 years; all of

Management of a Pilon Fracture Complicated by Drug-induced Hypersensitivity Syndrome, Fungal Infection and Limb Length Discrepancy: A Case Report.

AIM AND BACKGROUND: Pilon fractures are rare but severe injuries associated with high-energy trauma, often resulting in significant articular and soft tissue damage. Their management is challenging due to complications such as infection, nonunion, and post-traumatic arthritis, which often require complex staged surgical interventions. Drug-induced hypersensitivity syndrome (DIHS), a life-threatening reaction characterised by rash and systemic involvement, presents additional challenges in orthopaedic contexts due to its association with high-dose corticosteroid therapy and increased infection

Discordance among skeletal maturity indicators: sources, interpretation, and clinical implications.

Bone age and regional skeletal maturity staging are widely used to identify growth stage, estimate remaining growth, and support orthopedic treatment decisions in children and adolescents. However, skeletal maturity indicators differ in the regions examined, developmental features observed, scoring rules applied, and reference standards used. Consequently, the same child may receive different age estimates or maturity classifications when a different bone group, anatomical region, imaging modality, or assessment method is selected. This narrative review synthesizes evidence from within-image b

Postoperative infection following volar locking plate fixation of closed distal radius fractures: a retrospective cohort study of infection rates and antibiotic prophylaxis practice.

BACKGROUND: Distal radius fractures are among the most common surgically treated upper-extremity fractures, and volar locking plate fixation is the predominant operative technique. Although perioperative antibiotic prophylaxis is widely used in orthopaedic surgery, procedure-specific evidence for closed distal radius fractures remains limited. Contemporary data regarding postoperative infection rates and antibiotic prophylaxis practice following volar locking plate fixation are scarce. The aim of this study was to describe postoperative infection rates following volar locking plate fixation of

A novel stacked double-plate fixation with quadrilateral surface buttress screws for osteoporotic acetabular fractures in elderly patients: a prospective pilot case series study evaluating clinical and radiological outcomes.

BACKGROUND: Acetabular fractures in the elderly present unique challenges due to osteoporotic bone, comminution, and frequent involvement of the quadrilateral surface (QLS) and anterior column. Conventional fixation often fails in this population, leading to hardware complications and secondary displacement. This prospective pilot study evaluates the clinical and radiological outcomes of a novel stacked double-plate technique combined with QLS buttress screws in geriatric patients. METHODS: Twelve consecutive patients aged ≥ 70 years with displaced acetabular fractures involving the anterior

[Risk of Failure of Two Methods of Osteosynthesis of Fifth Metacarpal Neck Fracture].

PURPOSE OF THE STUDY: Fifth metacarpal neck fractures rank among the most common fractures of the hand skeleton. Only about 10% of these fractures are treated surgically. There are multiple surgical treatment options. This study aims to compare the outcomes of plate osteosynthesis with those of the "bouquet" technique. MATERIAL AND METHODS: Two cohorts of patients with fractures to the neck of fifth metacarpal treated at a district hospital and at a university hospital between 2017 and 2022 were evaluated, namely due to a different approach to fracture management. The district hospital cohort

Evaluation of stress distribution in different fixation techniques following bilateral sagittal split osteotomy: three-dimensional finite element analysis.

BACKGROUND: The choice of fixation technique following bilateral sagittal split osteotomy (BSSO) plays a critical role in postoperative stability and biomechanical performance. Despite extensive research, the influence of varying mandibular advancement magnitudes and physiologic muscle forces on stress distribution remains insufficiently explored. PURPOSE: This study aimed to compare stress distribution patterns and mechanical behavior of two commonly used fixation systems-miniplates (MPs) and bicortical screws (BCSs)-under different mandibular advancement conditions using three-dimensional f

A hybrid simulation phase correction method for endoscopic trans-osseous ultrasound imaging.

Due to the high risk of neurological or vascular injuries related to cortical perforation during pedicle screw insertion, the endoscopic ultrasound imaging shows potential for radiation-free intraoperative navigation, capable of providing real-time guidance for the insertion. To ensure penetration depth, low-frequency ultrasound is often selected. However, the quality of trans-osseous ultrasound imaging is affected by the limited resolution as well as the bone-induced aberration. To improve the image quality, a virtual source synthetic aperture focusing and coherence factor weighting method (V

Delayed Structural Failure Following Medial Femoral Condyle Flap Reconstruction of the Distal Radius: A Report of Two Cases.

Vascularized medial femoral condyle (MFC) flap transfer is a technique that may be used to reconstruct recalcitrant distal radius nonunion. Although union rates are high, the flap provides thin cortical bone requiring rigid fixation. The long-term strengthening and structural integrity of the MFC flap bone remains understudied. We describe two cases of delayed structural failure following distal radius MFC reconstruction. Two patients underwent distal radius reconstruction using vascularized corticocancellous MFC flaps stabilized with spanning fixed-angle volar plates. Both achieved radiograph

Triphasic Evolution of Spontaneous Calvarial Neo-Ossification After Decompressive Craniectomy: A 15-Year Serial Imaging Study.

Spontaneous cranial bone regeneration after decompressive craniectomy (DC) is rare in adults and has uniformly been described as a unidirectional, progressive process. The authors report the first case of a triphasic, nonlinear evolution-formation, resorption, and reactivation-of spontaneous calvarial neo-ossification in an adult. A 24-year-old man underwent bilateral DC for severe traumatic brain injury; the native dura was preserved almost intact on the left side, whereas the right side underwent extensive durotomy with expansile duraplasty. Serial computed tomography at 8 time points over 1

Influence of adding a pericardium membrane to the bioactive shield technique in vestibular socket therapy for immediate implant placement in Type II alveolar sockets: a randomized clinical trial.

This investigation aimed to compare the efficacy of a slowly resorbable cortical laminar bone membrane (CLBM), used either in isolation or in combination with a pericardium membrane, alongside an enriched bone graft. The treatment was to ascertain improvements in buccal plate thickness, buccal plate height, and soft-tissue thickness after immediate dental implant placement into Type II alveolar sockets managed with VST. Thirty participants with type II alveolar sockets were randomly assigned to two experimental cohorts. They received immediate implants with CLBM after sticky bone application i

Volar Rim Morphology and Plate Congruence in Fragment-Specific Distal Radius Fixation.

BACKGROUND: Fragment-specific fixation of distal radius fractures often requires implants positioned at or beyond the watershed line, which may increase the risk of flexor ultrasound-detected flexor tendon abnormalities. However, tendon conflict may depend not only on plate position but also on how the implant adapts to individual volar rim morphology. This study assessed whether volar rim morphology, classified according to the Dolomites concept, was associated with plate application and ultrasound-detected flexor tendon abnormalities after fragment-specific fixation. We hypothesized that Dol

A rare traumatic triad: Open bicondylar non-conjoint Hoffa fracture with partial tibial tubercle and medial epicondyle avulsions - A case report and biomechanical analysis.

UNLABELLED: Coronal shear fractures of the distal femoral condyles are rare intra-articular injuries, and the simultaneous occurrence of a non-conjoint bicondylar Hoffa fracture with a partial anterior tibial tubercle avulsion and a medial epicondyle avulsion constitutes an exceptionally complex traumatic triad. We report the case of a 35-year-old male motorcyclist who sustained this "floating" articular block injury (AO/OTA 33-B3.3) following a high-velocity accident. Diagnostic assessment utilizing Computed Tomography with 3D reconstruction revealed a vertical Letenneur Type II fracture of t

The developmental remodeling front: resorptive cell-vascular coupling at the chondro-osseous junction.

The chondro-osseous junction is commonly described as a site where cartilage is removed and bone is deposited, but this sequence does not explain how matrix topology is relayed into primary trabecular architecture. This review frames the junction as a developmental remodeling front in which cartilage-derived signals, endothelial advance, matrix-processing cell states, temporary scaffold retention, and osteoid deposition overlap in space and time. Mouse perturbation studies support vascular endothelial growth factor A (VEGF-A)-dependent entry, endothelial matrix metalloproteinase 9 (MMP-9)-medi

Growth plate senescence, chondrocyte biology, and the growth hormone-insulin-like growth factor 1 axis in childhood growth.

Longitudinal bone growth in children depends on endochondral ossification within the epiphyseal growth plate, a process long viewed as a passive readout of systemic hormones but now recognized as an actively regulated, self-limiting process. This mini-review synthesizes recent evidence on chondrocyte biology, growth plate senescence, and the growth hormone (GH)-insulin-like growth factor 1 (IGF-1) axis. Local paracrine networks establish precise antagonistic and synergistic interactions: fibroblast growth factor receptor 3 (FGFR3) suppresses chondrocyte proliferation via MAPK/STAT signaling, w
