The morphometry and pathological anatomy of the lumbar spine in South African negroes and caucasoids with specific reference to spinal stenosis.

Direct measurements were made on 2,166 lumbar vertebrae of 433 adult negro and caucasoid skeletons. On statistical analysis, forty-five vertebrae in twenty-seven skeletons were found to be stenotic, the mid-sagittal diameter being the significantly reduced dimension. Whereas spinal stenosis syndromes are rare in South African negroes, the lumbar canal is marginally narrower in the negro. There is a uniformity of configuration and capacity of the lumbar spinal canal, which transcends race and sex. By a new method of determining the dorsal limit of the lumbar canal on lateral plain radiography,

Anatomy of the achondroplastic lumbar canal.

The narrowed bony canal in the achondroplastic spine cases varied neurologic problems. The specific anatomic areas seem not to have been directly measured in an autopsy specimen. An achondroplastic spine was examined radiographically and found to show decreased interpediculate distance, thick pedicles, inferior facet thickness, and for animal stenosis in the lumbar canal. These findings have direct application to surgical treatment insofar as simple laminectomy for achondroplastic dwarfs may be insufficient to produce neural canal decompression. The problem is likely to require thorough exposu

Computed tomography of the lower lumbar vertebral column. Normal anatomy and the stenotic canal.

Computed tomography (CT) was used to obtain images of the vertebrae and spinal canal. The normal CT anatomy of the lower spinal canal is demonstrated. Examples of spinal stenosis caused by degenerative spondylosis, post-fusion hypertrophy, and pseudospondylolisthesis are presented.

Results of surgical treatment of idiopathic developmental stenosis of the lumbar vertebral canal. A review of twenty-seven years' experience.

Between 1948 and 1975 147 patients were treated surgically for developmental stenosis of the lumbar vertebral canal, measurement of the mid-sagittal diameters in the whole area of stenosis being performed in 116 patients. Ninety-two of these patients were followed up for periods varying between one and twenty years. About two-thirds were completely relieved fo symptoms and signs. Sciatica and intermittent claudication were more frequently cured than radicular deficit and lumbago, the latter being the most frequent persisting symptom. A permanent neural deficit as a result of the surgical proce

[Postoperative spinal canal stenosis (author's transl)].

Four cases of postoperative spinal canal stenosis were presented in which compression of cauda equina was observed. The lesion was found in 5.8% of operated spinal stenosis (four out of 69 cases). One must pay attention to differentiate the lesion from the recurrence of initial disorder, because the symptoms of the postoperative spinal stenosis is no always typical but sometimes resemble to the symptoms of the initial disorder which needed the operation. Myelography is the only reliable method to decide the extent of the spinal canal stenosis.

Lumbar spinal stenosis.

Recognition and management of problems inherent in spinal stenosis require a clear understanding of the diverse anatomic changes, their radiologic representations, and careful correlation with a wide spectrum of fluctuating clinical manifestations. Although a common basis might be postulated for narrowing of the spinal canal, emphasis should be placed on its various forms.

Spinal stenosis. The narrow lumbar spinal canal syndrome.

This is a report of 7 cases of narrow lumbar spinal canal syndrome encountered in 7 patients in a community hospital in one year. Nerve compression occurred from pressure from enlarged posterior elements. Although the clinical manifestations were unspecific, the lumbar spine radiographs and myelographic features are quite characteristic. Supported by myelography and/or operative findings, surgical management consisted of decompression laminectomy at multiple levels, together with diskectomy at the appropriate levels. The interim results observed at one year follow-up examination were excellent

[Myelopathies cervical arthroses. Apropos of 14 personal cases, 12 of which were operated on].

Reviewing fourteen cases, the authors show that cervical myelopathies present by spastic quadriplegia associated in half of the cases with sensory impairment and in a third of cases with radicular signs in the upper limbs. Tomography always shows posterior discal osteophytosis, sometimes gross, which reduces the lumen of the canal more particularly since it is associated (eleven times out of fourteen) with constitutional stenosis, of variable severity. Opaque or gaseous myelography makes it possible to exclude malformation or new formation and will demonstrate cord constriction within the cana

Axial transverse tomography of the cervical spine narrowed by ossification of the posterior longitudinal ligament.

Ossification of the posterior longitudinal ligament in the cervical region is generally regarded as a rare disease, though a Japanese series of 185 cases has been reported. The main symptoms arise from a myelopathy, the degree of which varies from moderate to severe, due to stenosis of the spinal canal. In order to clarify the relationship between the severity of symptoms and the cross-sectional area of ossification, axial transverse tomography of the cervical spine has been carried out on twenty-six patients at intervals of 5 millimetres over the full extent of the ossification. In this way t

Injuries of the hindfoot.

Although the long term prognosis for many injuries of the hind foot is poor, prompt diagnosis and treatment can lead to a satisfactory result. Failure to treat these injuries promptly can lead to circulatory problems and skin necrosis. Stiffness and post-traumatic arthritis may occur despite adequate treatment due to the complex anatomy of the region. With the exception of certain rare fractures of the os calcis, displaced fractures of the talar neck and body and fracture-dislocations, injuries in this region of the foot heal best after closed treatment and early mobilization of the ankle and

Jefferson fracture with disruption of the transverse ligament. A case report.

A 20-year-old male incurred a vertex injury of the skull, disruption of the transverse ligament of the atlas, and a Jefferson fracture--lateral displacement of the lateral masses of the atlas in relation to the lateral margins of the second cervical vertebra. The combination of Jefferson fracture with transverse ligament disruption was not appreciated initially (or even late) because of the head injury. The hospital course was complicated by the patient's initial unconsciousness and restlessness caused by associated intracranial injury. Roentgenograms in all views of the cervical spine were es

Cervical spinal stenosis.

Cervical spinal stenosis occurs at the craniovertebral junction, usually incident to a congenital malformation, or it appears as a developmental defect with diffuse narrowing of the cervical canal. In its acquired form the lesion may be limited to one or two levels, or it may be more extensive and affect three or more segments.

The narrow lumbar spinal canal or lumbar spinal stenosis.

Narrow lumbar spinal canal or lumbar spinal stenosis is a not uncommon problem. Thirty-two patients with this disorder have been treated surgically at the Ochsner Medical Center between 1963 and 1973. The best method of diagnosis is a careful history and second is roentgenographic evidence, particularly the myelogram. The surgical treatment consists of total laminectomy and foraminotomy to insure complete decompression of the nerve roots. Eighteen patients have been followed from 14 to 44 months. Sixteen of the 18 considered their operations a success.
