Standalone interspinous process devices for lumbar spinal stenosis: a critical literature review.

BACKGROUND: Interspinous process devices (IPDs) have been widely promoted as a minimally invasive alternative to traditional decompressive surgery for lumbar spinal stenosis (LSS). However, their clinical durability, safety, and comparative effectiveness have remained controversial. This review aims to review and synthesize the biomechanical rationale, clinical outcomes, safety profile, and limitations of IPDs for the treatment of LSS. METHODS: A literature review was conducted using PubMed for clinical studies published between 2000 and 2025 on IPDs. These studies were further categorized by

Unilateral open-door laminoplasty involving C1 for developmental atlantal canal stenosis combined with multilevel cervical stenosis: a case report.

BACKGROUND: Developmental atlantal canal stenosis is a rare cause of high cervical myelopathy. When combined with multilevel subaxial cervical stenosis, even minor trauma may result in severe neurological deficits. Surgical management remains challenging, particularly when C1 is involved, as laminoplasty is conventionally limited to C3-C7. CASE PRESENTATION: A 55-year-old male presented with acute cervical pain accompanied by quadriparesis following a low-energy fall. Neurological examination revealed severe cervical myelopathy with an ASIA grade C injury. Preoperative imaging demonstrated dev

Efficacy and Safety of the Use of Interspinous Spacers in the Treatment of Lumbar Spinal Stenosis: Mapping Gap and Scoping Review.

OBJECTIVES: Low back pain is a common pathology in the general population. In people over 60 years of age, it is associated with degenerative changes that cause narrowing of the vertebral canal. Its treatment includes conservative measures and even surgery with decompression and fusion. In those patients with moderate symptoms, the use of interspinous spacers emerged as a minimally invasive therapeutic option. This scoping review seeks to map the existing literature on the use of interspinous spacers and identify knowledge gaps to clarify their real position in the stepwise approach to low bac

High-Resolution 2D versus 3D Lumbar Spine MRI Optimized with a Deep Learning Reconstruction Algorithm and Prototype Conformal Coil.

BACKGROUND AND PURPOSE: Lumbar spine MRI is predominantly performed using 2D FSE sequences. 3D FSE sequences offer potential advantages over 2D, especially with recent advances in deep-learning reconstruction (DLR) and the use of conformal high-density coil arrays to improve the SNR. This study aimed to compare image quality and diagnostically relevant differences between 2D and 3D lumbar spine protocols, both using a prototype conformal coil and DLR. MATERIALS AND METHODS: Subjects with lumbar curvature referred for routine lumbar spine MRI were prospectively recruited (n=31). A prototype, c

Initial follow-up results in patients undergoing uniportal full endoscopic cervical unilateral laminotomy for bilateral decompression.

OBJECTIVE: Cervical spondylosis affects 5%-20% of the population and is commonly managed with posterior decompression. However, early studies of laminectomy without fusion revealed a significant risk of postoperative kyphosis, leading to the adoption of laminectomy with posterior fixation or laminoplasty as the standard of care. The aim of this study was to assess whether posterior decompression performed via a uniportal full endoscopic approach can achieve adequate decompression of the cervical spinal cord while preserving postoperative alignment. METHODS: This single-surgeon single-institut

Comparative Study of Posterior Lumbar Interbody Fusion Using Expandable Cages versus Lateral Lumbar Interbody Fusion for Lumbar Canal Stenosis Associated with Osteoporotic Vertebral Fractures: A Multicenter Retrospective Cohort Study.

OBJECTIVE: Lumbar canal stenosis associated with osteoporotic vertebral fractures is rare, and optimal surgical strategies remain controversial. While anterior reconstruction is considered beneficial, no studies have compared lateral lumbar interbody fusion (LLIF) and extensive posterior lumbar interbody fusion (Ex-PLIF). This study compared the efficacy and safety of Ex-PLIF and LLIF for lumbar canal stenosis associated with osteoporotic vertebral fractures. METHODS: We retrospectively reviewed 40 patients from 5 hospitals treated between 2015 and 2022. Patients underwent 1-level or 2-level

Unilateral biportal endoscopic decompression for stable adjacent segment disease after lumbar fusion: Clinical and radiological outcomes.

PurposeAdjacent segment disease (ASD) is a recognized complication following lumbar fusion, characterized by new symptomatic and radiological degeneration at levels adjacent to the fused segment. While revision decompression with extended fusion has traditionally been performed, minimally invasive fusion-sparing strategies such as unilateral biportal endoscopic (UBE) decompression have gained attention. High-level evidence regarding their effectiveness in carefully selected stable ASD remains limited.MethodsThis retrospective cohort study included 17 consecutive patients with clinically and ra

Establishing navigated UBE in Europe - technical note.

INTRODUCTION: Unilateral biportal endoscopic (UBE) lumbar decompression has emerged as a minimally invasive technique for the treatment of degenerative lumbar pathologies, including disc herniation and spinal canal stenosis. The reproducibility of this technique depends on standardized positioning, imaging, portal placement and stepwise decompression under continuous endoscopic visualization. We present a navigation-assisted workflow that integrates intraoperative three-dimensional imaging (Loop-X, Brainlab AG, Munich, Germany). TECHNIQUE: Patients are positioned prone on a radiolucent table.

Pembrolizumab Failure in an Aggressive, Platinum-Resistant Primary Mediastinal Yolk Sac Tumor With Rapid Metastatic Dissemination, Spinal Cord Compression, and a Fatal Outcome.

Primary mediastinal yolk sac tumors are rare, aggressive extragonadal nonseminomatous germ cell tumors that carry a poorer prognosis than their gonadal counterparts. Outcomes are particularly dismal in platinum-resistant disease, and the role of immune checkpoint inhibitors in this setting remains poorly defined. We report the case of a 26-year-old man with a primary mediastinal yolk sac tumor who progressed despite multimodal therapy, including etoposide, ifosfamide, and cisplatin chemotherapy, high-dose carboplatin and etoposide with autologous stem cell transplantation, gemcitabine and pacl

Lumbar spinal stenosis in axial spondyloarthritis and chronic low back pain: a cross-sectional magnetic resonance imaging study with clinical dimensions.

BACKGROUND: Axial spondyloarthritis (axSpA) may be associated with degenerative spinal lesions that may contribute to lumbar spinal stenosis (LSS), and the overlap of degenerative and inflammatory imaging findings in axSpA complicates diagnosis and treatment. To quantitatively and qualitatively compare lumbar central and lateral stenosis findings on magnetic resonance imaging (MRI) in patients with axSpA to age- and gender-matched controls suffering from chronic low back pain (cLBP), as well as to evaluate associated clinical parameters. METHODS: This cross-sectional cohort study included pat

Effectiveness and Safety of the Minimally Invasive Lumbar Decompression (MILD) Procedure for Adults with Lumbar Spinal Stenosis: A Systematic Review.

OBJECTIVE: Lumbar spinal stenosis (LSS) is a common and disabling condition in older adults, and current treatment options leave a therapeutic gap between conservative management and open surgical decompression. Minimally invasive lumbar decompression (MILD) is a percutaneous, image-guided procedure that targets the hypertrophic ligamentum flavum as a primary driver of central canal narrowing. This systematic review evaluates the effectiveness and safety of MILD for the treatment of LSS. METHODS: A systematic search of MEDLINE, Embase, Cochrane CENTRAL, and Web of Science was conducted for st

[Spinal canal stenosis : Widespread disease or just a radiological diagnosis?].

Lumbar spinal canal stenosis primarily affects older individuals over 65 years old and is often associated with typical symptoms, such as spinal claudication. Aside from rare congenital causes, it usually develops secondarily due to age-related changes, such as disc protrusion, facet joint degeneration and ligament hypertrophy. Apart from the typical clinical symptoms, native upright X‑ray images in two planes and magnetic resonance imaging (MRI) are decisive. Patients without relevant neurological deficits should primarily undergo conservative treatment. In cases of pain that cannot be allevi

A novel bone screw implantation strategy in C1-2 instability: a framework for evolving high cervical spine surgery.

Fixation surgery for atlantoaxial instability (AAI) or atlantoaxial subluxation is recognized as a complex and high-risk procedure, prompting the development of various fixation techniques. In this preliminary study, we evaluated the safety and accuracy of C1-2 pedicle screw insertion using our developed planning software and 3D-printed guiding templates. We also analyzed feasibility with free-hand techniques to identify an optimal strategy for atlantoaxial fixation. We used a self-developed 3D planning method, along with guiding template techniques, for pedicle screws in high cervical vertebr

3D printing enabled biomechanical evaluation of a novel expandable wedge spacer for atlantoaxial reduction.

Atlantoaxial subluxation (AAS) is a severe instability of the craniovertebral junction that may cause progressive spinal cord compression and neurological impairment. Achieving complete reduction remains challenging when anterior inclination deformity of the atlas limits posterior traction. This study introduces a novel titanium alloy expandable wedge-shaped spacer designed to correct the anterior inclination of the atlantoaxial joint (AAJ) and facilitate controlled distraction and reduction. The implant integrates a two-stage expansion mechanism that enables sequential anterior elevation and

Robot-assisted unilateral biportal endoscopic lumbar interbody fusion for lumbar spondylolisthesis: ipsilateral direct with contralateral indirect decompression.

OBJECTIVE: Degenerative lumbar spondylolisthesis often leads to bilateral spinal canal and intervertebral foramen stenosis, yet symptoms frequently present unilaterally. The need for decompression on the asymptomatic or mildly symptomatic side remains a topic of debate. METHODS: There were 28 patients with single-level degenerative lumbar spondylolisthesis (Meyerding grades I/II) with bilateral symptoms were selected for this study. We measured preoperative and postoperative foramen height (FH), foraminal area (FA), disc height (DH), cross-sectional area of spinal canal(CASC), and degree of up

A Fixed Left-Sided Surgeon Position in Unilateral Biportal Endoscopic Surgery for Lumbar Disc Herniation: A Technical Note with Variations According to Herniation Type.

BACKGROUND: Unilateral biportal endoscopy (UBE) is increasingly used for the treatment of lumbar disc herniation. In this bimanual technique, adapting surgeon positioning according to herniation laterality may negatively affect ergonomics and operating room workflow. The feasibility and clinical impact of a fixed surgeon-positioning strategy independent of herniation laterality remain insufficiently documented. RESEARCH QUESTION: Can a fixed surgeon positioning strategy be safely and effectively applied across all lumbar disc herniation types in UBE? METHODS: A standardized left-sided surgeo
