OBJECTIVE: In patients with severe central canal stenosis or posterior vertebral body osteophytes, endoscopic techniques often face challenges such as limited decompression range, endoscopic visual blind spots, and inadequate decompression. To address these limitations, our team innovatively developed a modified posterior cervical endoscopic decompression technique termed "Cross-over top Decompression (COTD)", drawing on clinical experience in lumbar endoscopic decompression and biomechanical principles. This study aims to compare its biomechanical performance with endoscopic unilateral lamino

Degenerative diseases of the lumbar spine are a common cause of chronic low back pain and neurological dysfunction. Their pathological basis involves not only microscopic structural damage to the intervertebral discs but also microscopic injury to adjacent nerves and muscle tissues. Diffusion tensor imaging (DTI), which relies on the anisotropic properties of water molecule diffusion, enables non-invasive, quantitative assessment of microscopic structural changes in intervertebral discs, nerve fiber bundles, and muscle tissues. Studies have shown that DTI-related parameters-fractional anisotro

STUDY DESIGN: A descriptive study investigating ligamentum flavum (LF) hypertrophy in individuals with and without lumbar spinal stenosis (LSS).
OBJECTIVE: To elucidate the relationship between LF hypertrophy and spinal degeneration in the lumbar spine based on magnetic resonance imaging.
SUMMARY OF BACKGROUND DATA: LF thickening plays a major role in LSS pathogenesis, initiated by mechanical stress that induces chronic inflammation, tissue damage, scarring, and fibrosis. However, whether LF thickening arises solely from mechanical stress remains uncertain. The connection between spinal dege

Muscle herniation is an uncommon musculoskeletal condition that may present with non-specific lower limb symptoms and, in rare cases, result in peripheral nerve compression. We report the case of a 32-year-old woman who presented with a two-year history of progressive exertional bilateral lateral calf pain and dysesthesia initially suspected to represent neurogenic claudication. Lumbar spine magnetic resonance imaging (MRI) demonstrated no evidence of spinal canal stenosis, intervertebral disc herniation or nerve root compression. Subsequent MRI of both calves performed on a 1.5-T MRI system r

Spinal stenosis most commonly occurs at L4-5, with 91% of patients experiencing nerve compression at this location. It results from a degenerative process that begins in the intervertebral disc, as viable cell counts, water, and proteoglycan content decrease in the nucleus pulposus. The disc height decreases as the nucleolus pulposus degenerates and the annulus fibrosus bulges. Stenosis is generally classified by descriptive elements, such as etiology (i.e., degenerative or congenital), location (i.e., central, lateral recess, foraminal, or extraforaminal), and severity of narrowing on advance

INTRODUCTION: The high-intensity zone (HIZ) is a distinct finding on T2-weighted MRI (T2WI), clearly demarcated from the nucleus pulposus and localized within the posterior or posterolateral annulus fibrosus of the intervertebral disc. Histopathological studies suggest that this finding represents an inflammatory process occurring within the HIZ lesion. This paper describes a case of radiculopathy and testicular pain presumed to be caused by an inflammatory HIZ lesion.
CASE PRESENTATION: A 55-year-old man presented with severe pain and numbness extending from the left inguinal region to the a

Endoscopic spine surgery enables targeted neural decompression with the least amount of tissue disruption to paraspinal structures compared to other spine surgery techniques. Though with minimal visualization, endoscopic spine surgery can be challenging. This article presents a stepwise overview of three key uniportal lumbar endoscopic techniques: interlaminar microdiscectomy, interlaminar laminectomy, and transforaminal microdiscectomy. Through integrated operative footage, the videos demonstrate indications, portal trajectory, and critical anatomic landmarks unique to each approach. The int

BACKGROUND: Occipital cervical fusion (OCF) is an effective method for immobilizing the cranial cervical junction (CCJ). OCF failure is uncommon but can be associated with kyphosis after laminectomy. Revision OCF after long-segment cervical laminectomy is challenging, as grafting options are more limited.
OBSERVATIONS: The patient was a 53-year-old man with pseudarthrosis, posterior displacement of the occipital plate, a chronic type II odontoid fracture with anterior subluxation of the fractured dens, a fracture of the C1 vertebrae with anterior displacement, and a previous 6- to 7-cm lamine

Traumatic thoracic spondyloptosis is a rare, severe spinal injury that poses a major surgical challenge. Traditional approaches often involve complex mechanical maneuvers and increased patient risk. This case report describes a posterior-only reduction and stabilization technique using a distraction trauma system, monoaxial screws, and neuromonitoring, offering an alternative to more invasive strategies such as corpectomy or multi-rod constructs. A patient with traumatic lateral T6-T7 spondyloptosis underwent single-stage posterior reduction using a spine distraction trauma system. The techniq

Failed back surgery syndrome (FBSS), recently redesignated as persistent spinal pain syndrome type 2, is a common complication after lumbar spine surgery, yet population-level data on its incidence and preoperative determinants remain limited. This study aimed to quantify the incidence of FBSS under multiple definitions and to identify preoperative risk factors for its development. This retrospective cohort study analyzed claims data from the Korean National Health Insurance Service (2009-2023), including 1,029,500 adults who underwent lumbar spine surgery. FBSS was operationalized using two m

AIMS: While robotic-assisted total hip arthroplasty (THA) improves implant positioning accuracy, it often overlooks dynamic lumbopelvic parameters, such as pelvic tilt changes between sitting and standing. These factors are increasingly recognized as critical in preventing early complications, such as impingement and dislocation. The aim was to determine whether integrating pelvic tilt variations into functional 3D planning reduces complications and improves outcomes following THA.
METHODS: In this retrospective cohort study, 656 patients underwent robotic-assisted THA using either convention

In patients with congenital cervical stenosis, the addition of abnormal cervical facet orientation (horizontal or hypoplastic joints) may predispose to segmental instability, subluxation, and canal compromise. Full assessment of facet anatomy is essential pre-operatively, as selected patients (e.g., those with demonstrable subluxation, widened facet joint spaces, or high risk of adjacent-segment instability) may require instrumented fusion in addition to decompression to maintain stability.

OBJECTIVE: Symptomatic basilar invagination (BI) is a complex craniovertebral junction malformation often accompanied by atlantoaxial dislocation (AAD). While posterior reduction and fixation is widely used, anterior odontoidectomy remains essential for decompression. However, conventional odontoidectomy carries a high risk of cerebrospinal fluid (CSF) leakage. We present a novel technique-Transoral Odontoid Process Release, Partial Resection, and Antedisplacement (TOPRA)-designed to achieve effective decompression while reducing CSF leakage risk.
METHODS: Between April 2020 and September 202

Facet joint osteoarthritis (FJOA) represents an early and prevalent indicator of lumbar degeneration, potentially impacting the adjacent multifidus muscle through mechanical and inflammatory mechanisms. Nevertheless, the association between FJOA and multifidus fat infiltration (FI) in the context of spondylolisthesis remains inadequately understood. This study retrospectively analyzed 243 patients diagnosed with either isthmic or degenerative lumbar spondylolisthesis (L2-S1) from two specialized spine centers. The severity of FJOA was assessed using Weishaupt's criteria. Parameters of the mult
