Impact of potentially inappropriate medication on surgical outcomes in older patients with lumbar spinal stenosis.

BACKGROUND: The administration of potentially inappropriate medications (PIMs) as well as polypharmacy in older individuals has become a medical and socioeconomic issue. Meanwhile, given that lumbar spinal stenosis (LSS) is one of the most prevalent musculoskeletal conditions, significantly affecting mobility in older adults and becoming increasingly common in aging societies, its management in this population has emerged as a critical concern. We aimed to elucidate the prevalence of PIMs among older LSS patients and identify those potentially impacting surgical outcomes. METHODS: We retrospe

The hidden asymmetry: facet joint tropism as a clue to spinal malalignment and muscle degeneration in adult spinal deformity.

OBJECTIVES: The role of facet joint tropism (FJT) in degenerative spinal disorders such as disc herniation, spondylolisthesis, and lumbar canal stenosis is well-established. However, its association with adult spinal deformity (ASD) remains underexplored. Hence, we aim to study the correlation of FJT with spinopelvic parameters and lumbar paraspinal muscle morphology in ASD patients. MATERIALS AND METHODS: We analysed 117 patients with ASD from 2021 to 2024. An absolute value difference (ΔFJA) of more than 10 degrees between the right- and left-facet joint angle (FJA) was defined as FJT. We c

‌Spike-shaped ossification of the posterior longitudinal ligament combined with dural ossification: a case report and literature review on imaging diagnosis and pathogenesis.

BACKGROUND: Ossification of the posterior longitudinal ligament (OPLL) combined with dural ossification (DO) significantly increases the technical difficulty and risk of complications in spinal canal decompression surgery. The pathological mechanisms underlying DO remain incompletely elucidated, and standardized imaging diagnostic criteria are currently lacking. CASE PRESENTATION: This study reports a 59-year-old female diagnosed with mixed-type cervical spondylopathy, OPLL at C5/6 and C6/7 levels, and hypertension. Preoperative computed tomography (CT) revealed flat-type OPLL at C5/6 and spik

Risk factors for underestimating lumbar spinal canal stenosis severity using MRI: A retrospective analysis.

STUDY DESIGN: A retrospective analysis. PURPOSE: This study aimed to identify the risk factors for underestimating the severity of lumbar spinal canal stenosis (LCS) using magnetic resonance imaging (MRI). OVERVIEW OF LITERATURE: In recent years, the indications for surgery for LCS have been increasingly determined by MRI alone, without myelography. However, as most MRI scans are performed in the supine position, it is challenging to detect dynamic stenosis using MRI alone. Thus, if the diagnosis is based on MRI alone without myelography, the severity of LCS may be underestimated. METHODS:

Validity, reliability, and responsiveness of a low-cost activity monitor for assessing walking in patients with lumbar spinal stenosis.

BACKGROUND CONTEXT: Reduced walking capacity is the hallmark of lumbar spinal stenosis (LSS). Currently, walking capacity in LSS is assessed with self-reported questionnaires. The Self-Paced Walking Test (SPWT) has been recommended as an objective capture of walking capacity, but takes up to 30 minutes to complete, preventing its widespread use by clinicians. Consumer-based activity monitors and smartphones have been suggested as an alternative but have not been validated in the LSS population. PURPOSE: To determine the measurement properties of criterion validity, reliability and responsiven

Can we predict post-surgical improvement in functional stooping in lumbar spinal stenosis? Insights from oblique lumbar interbody fusion outcomes and radiologic predictors.

BACKGROUND: Functional stooping, characterized by a forward-flexed lumbar posture in patients with lumbar spinal stenosis (LSS), serves as a compensatory mechanism aimed at alleviating pain by expanding the constricted spinal canal. Surgeons widely use the oblique lateral interbody fusion (OLIF) to treat patients with LSS, restoring segmental lordosis in index surgical level. In some patients with LSS, improvement of global sagittal imbalance occurs after short-level OLIF. it remains unclear whether this is predominantly due to segmental correction or the resolution of functional stooping. A new surgery choice of bilateral laminoplasty for symptomatic three or more-level lumbar canal stenosis in patients over 60 years old: a two-year retrospective study.

INTRODUCTION: Multi-level lumbar canal stenosis (LCS) in patients over 60 years is common and frequently complicated by significant comorbidities, making surgical decisions complex. While posterior lumbar interbody fusion (PLIF) is often used, bilateral laminoplasty offers a potential motion-preserving alternative. This study compares outcomes of bilateral laminoplasty versus PLIF for multi-level LCS in this elderly population. METHODS: This retrospective study included consecutive patients >60 years with ≥3-level LCS undergoing bilateral laminoplasty or PLIF at our center. Patients were follo

Surgical Threshold Measurements for Cervical Spinal Stenosis: Post-Myelogram Computed Tomography Versus MRI.

INTRODUCTION: MRI and CT myelography (CTM) are common diagnostic tests used for preoperative assessment of patients with cervical spondylotic myelopathy. The purpose of this study is to determine if MRI and CTM result in the same quantification regarding the severity of canal stenosis and cord compression. METHODS: Fifty patients with cervical spondylotic myelopathy underwent MRI and CT myelogram within a 6-month time frame preoperatively. Each spine segment (C3-C7) was measured for AP diameter of the canal and spinal cord on MRI axial, CTM axial, MRI sagittal, and CTM sagittal images. Thresh

Neck posture-dependent variations in cervical central canal stenosis among ossification of the posterior longitudinal ligament patients: Follow-up neutral position magnetic resonance study.

In patients with cervical ossification of the posterior longitudinal ligament (OPLL), posture differences in neutral Magnetic Resonance Imaging (MRI) can affect central canal stenosis evaluation, but their impact on sagittal alignment parameters and central canal stenosis measurements remains unclear. This retrospective cohort study sought to evaluate neck posture-dependent variations in central canal stenosis in patients with OPLL and assess the influence of sagittal alignment parameters. Fifty-seven patients underwent two neutral-position MRI scans within six months. Occipito-C2 (O-C2) angle

Evaluating whether adding laminoplasty to ACDF provides superior neurological recovery compared to ACDF alone in High-Risk patients with multilevel cervical spondylotic myelopathy and cervical canal stenosis.

PURPOSE: Multilevel cervical spondylotic myelopathy (CSM) with severe stenosis and anterior compression is challenging in high-risk patients (e.g., positive ISI, low compression ratio) where ACDF alone may be inadequate. We evaluated if combined laminoplasty and ACDF (LP-A) improves neurological recovery over ACDF in this population, as comparative evidence is limited. METHODS: This retrospective study analyzed 82 patients (LP-A = 26, ACDF = 56) treated between January 2017 and June 2023 for multilevel CSM (≥ 3 levels) with canal stenosis (< 13 mm) and anterior-predominant compression (modifi

Predictors of failure in posterior short segment instrumentation for thoracolumbar burst fractures: a systematic review and meta-analysis.

BACKGROUND: Posterior short segment instrumentation (PSSI) aims to restore spinal stability following thoracolumbar burst fractures. Its advantages include involving fewer mobile segments and reduced operative time compared to posterior long-segment fixation. However, reports of PSSI failure raise concerns about the factors influencing treatment success. This systematic review and meta-analysis seek to determine the prevalence of PSSI failure and identify preoperative imaging characteristics associated with increased failure risk. METHOD: A comprehensive literature search was conducted across

Utility of Emergent Spine MRI in the Emergency Department.

INTRODUCTION: Prolonged emergency department (ED) waiting times for STAT spine magnetic resonance imaging (MRI) in the ED can expose patients to hospital-acquired infections and increase the workload in the ED, further impacting healthcare quality. In this study we aimed to characterize emergent spine MRI frequency and positivity in the ED, and its impact on ED length of stay (LOS), admission rates, and the necessity for surgical interventions. METHODS: We performed a retrospective chart review of a consecutive group of patients who had emergent spine MRI (cervical, thoracic, lumbar) ordered
