OBJECTIVE: The accurate evaluation of patients with multiple injuries is logistically complex and time sensitive, and must be cost-effective. We hypothesize that computed tomographic (CT) scan of the upper cervical spine (occiput to C3 [Co-C3]) would add little to the initial evaluation of patients with multiple injuries who have altered mental status.
METHODS: The study consisted of a prospective, unblinded, consecutive series. Patients met entry criteria if they had sustained a blunt mechanism of injury and had an altered mental status requiring CT scan of two or more body systems. All pati

This report describes a complex syndrome of injuries occurring in a young female who was a back seat passenger wearing a lap-belt restraint in a high-speed road traffic accident. As a consequence of the forced flexion distraction injury of her lumbar spine, she sustained a fracture-subluxation of the first lumbar vertebra in association with a jejunal perforation and extensive small intestinal mesenteric laceration. She also had a large traumatic hernia of the anterior abdominal wall, which was overlooked at primary laparotomy. This report highlights collectively the classical combination of i

INTRODUCTION: Platelet adhesion and aggregation on exposed vascular extracellular matrix (ECM) is critical for haemostasis, with dysregulation and inappropriate thrombus formation associated with cardiovascular disease. While collagen is recognised as vital in these processes, the role of other ECM proteins is less understood. Platelet-derived microparticles (PDMPs), small vesicles released by activated platelets, similarly influence haemostasis, though their modulatory effect on ECM substrates is also unclear. We investigated platelet adhesion and aggregation on various ECM proteins-collagen,

OBJECTIVE: To report the sign of "spinolaminar breach" and its likely importance in fractures of the cervical spinous processes.
DESIGN: Six cases of spinous process fractures demonstrating disruption of the spinolaminar line or "spinolaminar breach" were analyzed. Lateral and anteroposterior radiographs (n=6), CT scans (n=3) and MRI scans (n=1) were reviewed together by the authors, with consensus being reached as to the radiographic findings. Clinical records were also reviewed.
RESULTS: The levels of injury were C6 (n=5) and C5 (n=2). Injuries were associated with delayed anterior subluxa

OBJECT: Injuries of the occipitoatlantoaxial (Oc-C2) region are the predominant form of cervical injury in children younger than 10 years of age. Magnetic resonance (MR) imaging can be used to visualize directly the traumatic ligamentous and soft-tissue abnormalities of the Oc-C2 region. A retrospective review was undertaken to examine the spectrum of pediatric Oc-C2 injuries seen on MR imaging, their correlation with plain x-ray film and computerized tomography findings, and their clinical course.
METHODS: Seventy-one consecutive children younger than 10 years of age underwent cervical MR im

The pelvic tilt is part of a complex deformity in which both the spine and the hip are involved. By viewing the different planes and analyzing the deforming forces, four different types of pelvic tilt in combination with spinal and hip deformities can be identified. In spina bifida patients congenital deformities of the spine may add to the progression of scoliosis. Analyzing our own patients we found that certain types of pelvic tilt can be related to certain neuromuscular diseases. Moreover our results show that the pelvic tilt can be effectively corrected by spinal surgery, whereas no effec

A case of primary dystonia with atlantoaxial subluxation is reported. This 46-year-old man who was suffering from primary generalized dystonia since childhood presented with clinical features suggestive of recent onset of cervical myelopathy. Imaging studies confirmed presence of severe spondylotic changes in the cervical spine with cord compression along with atlantoaxial subluxation. It is important to identify this potentially dangerous complication, which may be surgically managed. This is the first case report in the literature about the association of atlantoaxial subluxation and primary

BACKGROUND: For more than 100 years, chiropractors have asserted that overall health can be improved through the use of spinal manipulative therapy. The autonomic nervous system is known to control and regulate all involuntary physiologic activities by controlling the activities of the internal organs, glands, and circulation. Recent studies document a potential relationship between the vertebral subluxation complex, autonomic tone, and cardiac function.
OBJECTIVE: This discussion reviews how it is possible to use heart rate variability analysis to calculate a quantitative index of autonomic

The case of a 86-year-old female victim of cervical trauma with C5-C6 fracture-luxation and bilateral locked facets is hereinafter described. The case was reputed representative of the complexity in deciding between conservative and surgical treatment in spine surgery. Conservative management would have been dictated by the additional risks linked to the age of the patient, whereas the surgical treatment would have been favored by the easy access to the anterior cervical spine for reduction and fixation. On the way to plan the definitive treatment, the patient died by pulmonary embolism. The c

Lower cervical spine injuries with instability of the anterior and/or posterior column can be treated by anterior interbody fusion and plate fixation. Plates available for anterior instrumentation of the lower cervical spine can be divided into locking or non-locking systems with uni- or bicortical screw purchase. Our biomechanical comparative testing of different screw fixation systems demonstrates improved stability with the use of bicortical purchase. Clinical studies, however, have proven high fusion rates without loss of correction and a low implant related morbidity with the use of unico
