Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association1997eng
This study was undertaken to review the results of an arthroscopic posterior capsular shift procedure. Twenty consecutive shoulders in 19 patients were treated with an arthroscopic posterior capsular shift for symptomatic posterior shoulder instability. Patients underwent the procedure if they exhibited a posterior Bankart lesion or had complaints of posterior instability and evidence of increased posterior joint laxity on physical examination and examination under anesthesia. Twelve of the 20 patients were injured during athletic activity. All surgeries were performed in an outpatient setting

STUDY DESIGN: A case of traumatic spondylopelvic dissociation and a method of fixation are described.
OBJECTIVES: The treatment of this rare injury pattern required differentiation from less severe lumbosacral fracture-dislocations or bilateral sacroiliac joint dislocations.
SUMMARY OF BACKGROUND DATA: Several case reports have described less severe fracture patterns, but only a handful have illustrated true spondylopelvic dissociation in which there is complete displacement of the lumbar spine into the pelvis.
METHODS: After initial stabilization at an outside hospital, the patient was tra

The correction of nasal deformities in cleft patients presents a challenge in the secondary treatment of this malformation. The complexity of the nasal deformity in cleft patients partly depends on the congenital anomaly itself and partly on the surgical procedures which were previously carried out. The nasal deformities in unilateral CLP patients are typical. The nasal floor and the nostril sill are mainly constructed during primary lip repair. While the ala on the cleft side is repositioned, the septum deformity and the anterior spine dislocation are not touched. If large circular incisions

Hip dysplasia is a common developmental problem affecting the canine population. Despite extensive research into the condition, many questions remain unanswered and numerous misconceptions are present among the general public. The purpose of this paper is to review the current knowledge on the development of hip dysplasia, factors modifying its development, and current diagnostic techniques.A computerized literature search was conducted for the period of January 1983 to April 1985 using the MEDLINE and CAB databases, and the keywords hip dysplasia, hip, dog, and canine. An unusual type of traumatic comminuted fracture/dislocation of C1 and C2 is presented. The fracture differs significantly from previously described types of C1-2 complex fracture. Anatomically contoured titanium loops were used for stabilization of the occipitocervical junction. To the best of the author's knowledge this is the first description of comminuted fracture/dislocation of the bodies and laminae of C1 and C2 in the English literature.

The authors report the rare and complex case of a girl who had been followed since the age of 3 years for hydrocephalus the cause of which was found only when she was 6-year old. The causative agent was a pilocytic astrocytoma of the cerebellum. On April 10, 1990, she underwent subtotal excision of the tumour, associated with radiotherapy. Four months later she developed an increasingly painful and irreducible torticollis which did not respond to tractions. Dynamic CT scans and 3-dimensional CT scans were performed in the fourth month and provided a diagnosis of right C1-C2 rotatory subluxatio

A 77-year-old man fell and sustained a fracture-dislocation of the atlantoaxial complex with 20 mm of posterior displacement. Initial and repeat examinations failed to diagnose the fracture-dislocation, which was first diagnosed by a magnetic resonance imaging study 16 weeks after the injury and after the patient had developed a profound myelopathy. The patient was treated by a C2 decompressive laminectomy and an in situ posterior fusion with iliac bone graft and internal fixation with gradual resolution of his myelopathy. Postoperatively he developed respiratory problems and persistent diffic

The availability of better and quicker resuscitation means that more patients with occipitocervical injuries survive. In view of the complex anatomy and functional capabilities of the upper part of the spine it makes sense to discuss injuries to this part separately from any to the lower cervical and thoracolumbar vertebral column. Computerized tomography with additional multiplanar reconstructions, digital radiography and magnetic resonance imaging early in the diagnostic workup improves the likelihood of a quick and satisfactory diagnosis. Our classification of injuries should enable the rad

The authors present their technique of occipital-axis posterior wiring and fusion for atlantoaxial dislocation associated with an occipitalized atlas. The technique consists of drilling a 3 x 1-cm horizontal groove in the occipital bone 1 cm posterior to the foramen magnum and building up a bony bridge along the posterior margin of the foramen magnum. This bony bridge is referred to as an "artificial atlas." Conventional wiring and fusion is performed between the artificial atlas and the C-2 lamina, interposing a strut bone graft. Since the compression force on tightening the wire is vertical,

Free vascularized osteocutaneous bone grafts are an unusual but effective method to reconstruct complex and extensive defects of skin and bone of the extremities due to trauma, tumor, or infections. Many recent publications support their utility in these situations. In distinction, little literature exists to document the use of free vascularized osteocutaneous flaps in the spine. This is likely due to the infrequent occurrence of concomitant skin and osseous defects that are not manageable by routine spinal and plastic surgical approaches. We recently managed a particularly challenging patien

Biomechanical analysis using nondestructive and destructive investigative methods was performed to evaluate the mechanisms of cervical compression injuries. These injuries produce two basic modes of failure: 1) anterior dislocation; and 2) rupture of the anterior ligamentous complex of the vertebral body. Distribution of these two failure patterns was determined by the initial cervical spine position; translational alignment did not have a significant effect. Different results were observed between spines positioned in flexion and extension, indicating that the most important factor determinin

Six patients were managed with gradual reduction of a deformity of the cervical spine, before operative stabilization, with use of a modified halo cast with adjustable distraction components that allowed the patient to sit and walk while the deformity was being corrected. The distraction components were constructed from the distractors, hinges, and connectors of an Ilizarov apparatus. The diagnoses were atlanto-axial subluxation secondary to rheumatoid arthritis, atlanto-axial rotatory subluxation secondary to juvenile rheumatoid arthritis, post-traumatic atlanto-axial rotatory subluxation, an
