Craniovertebral abnormalities in Down's syndrome.

The ligamentous laxity associated with atlantoaxial subluxation has been assumed as a normal occurrence in 15-20% of Down's syndrome patients. The occipitoatlantoaxial instability that accompanies this entity has been poorly recognized. The medical records and radiographic findings of 18 symptomatic patients with Down's syndrome and cervicomedullary compromise were reviewed (1979-1991). All patients were evaluated prospectively under protocol for abnormalities of the cranial vertebral junction. There were 11 males and 7 females (age 3-42). 'Fixed' atlantoaxial luxation was seen in 8 (5 develop

Craniocervical junction malformation treated by transoral approach. A survey of 25 cases with emphasis on postoperative instability and outcome.

An experience with 25 consecutive cases of craniocervical junction (CCJ) malformations operated upon via the transoral route is reported. Twenty-two patients also underwent posterior occipito-cervical stabilization with alloplastic material and in only one patient was transoral odontoidectomy and fusion with bone autograph performed. Indication for the transoral route consisted of an irreducible ventral compression of the cervicobulbar junction by the abnormal bone complex. Two patients died during the early postoperative period and the remaining 23 survivors were followed for an average of 3.

Lateral cervical spine dislocation and vertebral artery injury.

Although anterior and posterior traumatic displacement of cervical vertebrae are commonly noted, and the devastating neurological deficits associated with these injuries have been amply defined, lateral displacement with fractures has been rarely recognized, and the clinical significance of this injury has been overlooked. This report describes five cases of cervical spine fractures with lateral dislocation. All patients had lateral and anteroposterior cervical spine radiographs as well as cervical angiography or postmortem study demonstrating either complete occlusion or significant impairmen

Evaluation of cervical vertebral injuries.

The use of imaging studies on patients with suspected cervical vertebral injury should be restricted to those patients who fall into the high-risk category for injury. Once a decision is made to obtain radiographs, a minimum of five views is required to adequately rule in or rule out injury. Complex imaging studies such as CT, polydirectional tomography, and MRI may be performed to confirm the initial impression based on plain radiographic findings. The diagnosis of cervical injuries may be facilitated by following a logical pattern of analysis searching for abnormalities of alignment and anat

The role and limitations of computed tomographic scanning in the evaluation of cervical trauma.

We retrospectively reviewed the medical records, plain films, CT scans and complex-motion tomographic studies (TOMOS) of 216 consecutive patients with cervical injuries to determine the uses and limitations of CT in the evaluation of cervical trauma and the indications, if any, for the continued use of TOMOS in evaluating cervical trauma. There were 453 fractures and 104 subluxations or dislocations of the cervical spine in the 216 patients. Plain films detected 58% (262 of 453) of the fractures and 93% (97 of 104) of the subluxations and dislocations; and 94% (202 of 216) of the patients with

Safety-belt injuries in children with lap-belt ecchymosis: CT findings in 61 patients.

We have noted a complex of common injuries in children wearing lap-styled safety belts during vehicular accidents. Sixty-one children who were restrained passengers in motor vehicle crashes had linear ecchymosis across the abdomen and had CT for abdominal trauma. Thirteen children (21%) had a lumbar spine injury, and 14 children (23%) injured a hollow viscus (bowel, 12; bladder, two); five children (8%) had both spine and hollow viscus injuries. Abnormal findings on abdominal CT were recognized retrospectively in three of 13 children with lumbar spinal injury. Lateral radiographs of the spine

Superior facet fractures of the axis vertebra.

Nine cases of superior facet fractures of the axis vertebra are reported. In seven cases, there were associated odontoid fractures. These fractures can occur in a coronal or sagittal direction, shearing off the anterior or lateral plateau of the facet. In addition, the lateral mass of the atlas may sublux into the depressed facet fracture. The fracture complex should be well documented with conventional radiography and tomography. Computerized tomographic scanning has been found to be particularly helpful in diagnosing these fractures and other injuries about the axis or atlas. Patients with u

Sagittally oriented fractures of the lateral masses of the cervical vertebrae.

Lateral flexion of the cervical spine may cause a sagittally oriented fracture of the lateral mass with shearing of all or part of the lateral mass away from the vertebral body. We report 22 patients with 24 sagittal fractures of the cervical lateral masses. Cross-table lateral roentgenograms suggested the presence of a sagittal lateral mass fracture in two thirds of the cases based upon malalignment or widening of the facet joints, displaced fracture fragments, subluxation of the spine, or rotation of the spine above the level of abnormality. However, the cross-table lateral roentgenogram was

[Nerve disorders of micturition in the dog. A review of five cases].

This investigation gives an overview of the etiology and treatment of neural micturition disorders, using five clinical cases. Micturition is only feasible when there is an unimpeded collaboration between superior control mechanisms and peripheral reflex mechanisms. Due to the complex physiology of bladder innervation, the loss of individual nerve paths leads to disorders in the chronological events of urine emission. The purpose of the present work is to demonstrate the symptoms and physiopathological principles which were found to be present in specially selected and typical clinical cases.

A retrospective clinical and neuroimmunohistochemical study of rheumatoid arthritic patients with atlanto-axial subluxation.

In a retrospective study of 87 RA patients with radiographically documented anterior atlanto-axial subluxation (AAS) in flexion-extension radiographs, 40 had been studied radiographically before they developed AAS. Of these 40 patients, 34 had had occipitocervical pain already before the subluxation. This shows that pain early in the course of the rheumatoid cervical spine is not caused by or associated with AAS itself. In a separate operatively treated group of 5 patients, ligament neuroanatomy in AAS was more closely studied, using specific heteroantisera to cytoskeletal neurofilaments and v

[Long-term regional infusion of drugs into the arteries of the spine and spinal cord. 2. Experimental and clinical studies].

The present paper is the result of the next stage of the studies devoted to prolonged intraarterial introduction of drugs into the tissues of the thoracic spine. For the solution of the problems of surgical provision of this task the authors have worked out the operative approaches to the secondary departments of the posterior intercostal arteries and the technique of their retrograde catheterization on 14 cadavers in 31 experimental operations. This method has been used in the complex treatment of 12 patients with acute and inveterate injuries of the thoracic and the thoracolumbar departments

[Surgically stabilizing "complex therapy" of unstable injury of the lower cervical spine].

Numerous assessments of the stability of screws within the cervical vertebral bodies after strong mechanical loading showed that spondylodesis can be considered as a stable fixation whether it is performed as a ventral, dorsal of combined procedure. Luxation fractures of the cervical spine, however, often do not only show lesions of the osteo-ligamentary structures, but are also accompanied by lesions of the spinal marrow. In addition to surgical measures, the self-destructing fermentative processes occurring in the mechanically injured spinal marrow have to be and can be eliminated or at leas

On the basis of 14 experiments with simulation of the lower cervical vertebra dislocations on cadavers and human cervical vertebra blocks, combined with stage discography, the authors singled out 3 groups of +disk and ligament structure damages: damage of capsular--ligamentous apparatus of the rear support complex; damage of ligamentous--capsular apparatus and disk; damage of ligamentous-capsular apparatus, frontal longitudinal ligament and total rupture of a disk. Differentiation is made according to the degree of dislocation of vertebra bodies and articular processes. The data are confirmed

Radiology of the cervical spine in trauma patients: practice pitfalls and recommendations for improving efficiency and communication.

Trauma constitutes a significant portion of emergency department practice. Such patients often have suspected cervical spine injury necessitating cervical spine radiographs. The importance of detecting cervical spine injury is obvious because failure to do so can lead to tragic consequences for patient and physician alike. Although most cervical spine radiographs are justified, poorly indicated and unnecessary examinations are unfortunately commonplace. Indiscriminate ordering of cervical spine examinations can easily exceed radiology resources assigned to the emergency department. Rational or

Imaging of the facet joints.

Facet joints have a variable anatomic orientation, and their imaging must be adapted to this variation. They have an important role in the stability and mobility of the spine at all levels. They are the sites of microtrauma leading to degenerative changes. Trauma may cause subluxation, dislocation, and fracture. As synovial joints, they manifest inflammatory processes and can present as part of a systemic disease. The facet joints are a component of a trijoint complex at each motion segment and cannot be separated biomechanically. Their visualization and analysis must be integrated with the ad

Magnetic resonance imaging of the shoulder.

MR imaging has proved to be of great value in the assessment of musculoskeletal imaging. Its superiority in evaluating knee and spine pathology is well established. Recent studies also have established an important role for MR imaging in the assessment of shoulder disorders. Multiplanar capabilities coupled with excellent contrast resolution that allows simultaneous delineation of osseous and soft-tissue structures make MR imaging a useful method for evaluating the complex anatomy of the shoulder.

Ligamentous laxity across C0-C1-C2 complex. Axial torque-rotation characteristics until failure.

The axial torque until failure of the ligamentous occipito-atlanto-axial complex (C0-C1-C2) subjected to axial angular rotation (theta) was characterized using a biaxial MTS system. A special fixture and gearbox that permitted right axial rotation of the specimen until failure without imposing any additional constraints were designed to obtain the data. The average values for the axial rotation and torque at the point of maximum resistance were, respectively, 68.1 degrees and 13.6 N-m. The specimens offered minimal resistance (approximately 0.5 N-m), up to an average axial rotation of 21 degre

Subaxial injuries.

Injuries to the subaxial cervical spine must be suspected in any patient who suffers a head injury or complains of neck pain or neurologic symptoms of the arms or legs following an accident, particularly a motor vehicle or diving accident. Careful neurologic examination and lateral roentgenograms are indicated in all patients with suspected injury. If there is any neurologic deficit, fracture, or dislocation seen on roentgenogram, skull-traction tongs should be applied to provide stability and prevent further damage. If the neurologic examination and roentgenograms are normal, a stretch-test r

Traumatic lesions of the occipitoatlantoaxial complex.

Injuries of the occipitoatlantoaxial complex are relatively rare but form a group of ligamentous and bony lesions that may unnecessarily confuse the clinician. Since fractures of the dens and traumatic spondylolisthesis of the axis have been well described in a number of large series, this article concentrates on the lesions less frequently seen and described. The ligamentous injuries are occipitoatlantal dislocation, rupture of the transverse ligament, and atlantoaxial rotatory fixation. The bony injuries are fractures of the occipital condyles, atlas, and the lateral masses of C2. The incide
