Extension injury to the arthritic spine of elderly individuals involves rupture of the anterior longitudinal ligaments and disks at several levels, but no bony injury can be observed on X-ray. There are many small focal haemorrhages in the central portion of the spinal cord and the injury is often accompanied by incomplete paresis. As the injury is a stable injury with the posterior ligament complex being intact, it is only necessary to immobilise the cervical spine conservatively. Laminectomy is not indicated. In hyperextension injury of the rigid cervical spine and in momentary posterior dis

The assessment of stability is one of the most important problems in the analysis of vertebral trauma. There are numerous indirect signs of instability. The "three column model" provides a concept towards a better understanding of instability. Compared with the conventional complex classifications, the new system based on the "three column model" provides certain advantages.

In the treatment of spine fractures and fracture-dislocations, stability of the spine is one of the major objectives. In the craniocervical joint, the alar and transverse ligaments provide much of the stability of the healthy spine. Because the anatomy appears well described, the contribution of each of these structures so far has received little attention. The alar ligament restrains rotation of the upper cervical spine, whereas the transverse ligament restricts flexion as well as anterior displacement of the atlas. A lesion in one or both structures can produce damage to the neural structure

A new and simple method for the operative treatment of atlanto-axial instability in children is described. The technique involves a metal rectangle wired in place between the occiput and cervical spine to act as an internal fixation device. Onlay bone grafts subsequently produce bony fusion. A synchronous decompression of the cervical canal can be performed and there is no need for complex or prolonged external fixation.

The inflammatory involvement of cervical spine ligaments in patients with rheumatoid arthritis (RA) with atlantoaxial subluxation was studied. Biopsy specimens were obtained from the ligament structures between the posterior arch of the atlas and the spinous process of C2 during atlantoaxis stabilizing operations. Inflammatory cells in cryostat sections were stained in situ using monoclonal hybridoma antibodies applied in a sensitive avidin-biotin-peroxidase complex (ABC) method. In 4 of 8 ligament specimens, focal inflammatory cell infiltrates consisting mainly of T11 + T lymphocytes (77 +/-

Three-dimensional CT (3-DCT) surface reconstructions were generated in 157 patients with various disorders of the musculoskeletal system. Whereas generating the 3-D display of ligaments, tendons, intervertebral discs and nerve roots was difficult and time-consuming, the comprehension of complex abnormalities of bony structures was easier than with two-dimensional CT. 3-DCT was particularly useful in the preoperative planning of hip and spine surgeries while it also simplified the intraoperative orientation. 3-DCT proved to be a useful diagnostic method supplementing axial computed tomography.

A 33-year-old female dialysis patient suffered from osteomyelitis and luxation of the dens axis with cervical myelopathy. In the past she had had three attacks of anaphylaxis after treatment with dialyzers that had been sterilized with ethylene oxide. IgE-type antibodies directed against human serum albumin-ethylene oxide complexes could be demonstrated in the patient's serum by radioallergosorbent techniques. Immediately after an operation in which acrylic bone cement (Palacos-R) sterilized with ethylene oxide was implanted for stabilization of the cervical spine, the patient developed massiv

Trauma to the upper cervical spine results in injuries to the C1-C2 complex that may differ significantly from those in the lower cervical spine. Five major types of injuries occur, and their treatment is based on an understanding of the mechanisms of injury.

32 patients with traumatic injuries of the cervical spine were investigated by CT. All patients were initially examined by plain films. CT was definitely superior to conventional radiology in compressive fractures of vertebral bodies. In flexion-extension injuries CT was useful showing fractures of the posterior elements in great detail. Conventional tomography was superior to CT in patients with complex dislocations of vertebral bodies. In the absence of fracture CT failed to detect disruptions of ligaments, which were readily shown by functional examination.

An evaluation of musculoskeletal disorders in 202 patients using three-dimensional CT displays has revealed the usefulness of the technique, especially in patients with fractures of skeletal areas with complex anatomy, articular disorders of the hip, and spinal stenosis. In some cases, plastic models of diseased areas have been created from the CT data and are reasonably accurate, providing a graphic representation of the disease and the ability to perform rehearsal surgery. A preliminary investigation of three-dimensional displays of MR images indicates that the technique is feasible, althoug

Twenty patients with C1-C2 vertebral trauma underwent computed tomographic (CT) scans to evaluate prospectively its efficiency for diagnosis and treatment. It is suggested that if standard roentgenograms are inconclusive, CT scan is recommended for further evaluation of C1 fractures and C1-C2 rotatory subluxation in preference to tomography and cineradiography. CT scans do not assess dynamic stability and may miss minimally displaced odontoid fractures. It was also concluded that they add little in the treatment of hangman's fractures, although scanning did show that the pathology was often mo

A review of the literature reveals strong evidence for both the mechanical model of disease production (structural) and the neurobiological model (functional). Outdated models which attempt to describe a scientific basis for chiropractic theory are inadequate and indeed harmful to the progress and acceptance of chiropractic. Pragmatic or empirical arguments that "Chiropractic works and that's what counts" have served a useful purpose, but now must be augmented by extant research findings. The "paradigm shift" is on. Research investigators around the world are focusing on the multiple component

In a group of 105 patients admitted to hospital with injuries of the cervical spine, the incidence of injuries at multiple levels was 24 per cent. Multi-level injuries occurred in 17 out of 54 patients (31 per cent) sustaining a noteworthy neurological injury and in 8 out of 51 when such an injury was absent. This incidence is higher than previously reported, probably due to increasingly elaborate investigations. In the majority of cases, treatment of the dominant injury was unchanged by the demonstration of injuries at other levels, but in a small number, serious errors in treatment arose or

Computed tomography (CT) provides axial slices plane and shows excellent details of bones and different soft tissues, favoring its use in traumatic lesions caused by sporting activities. Complex anatomical structures such as the shoulder, the vertebral column, the pelvis, the knee, the tarsal and carpal bones are often better recognized in detail than by conventional radiography. Fracture lines, localization of bone fragments and involvement of soft tissues are clearly demonstrated. Luxations and bone changes leading to luxations can be shown. CT arthrography provides for the first time a dire

The snapping hip syndrome is a symptom complex characterized by hip pain and an audible snapping of the hip with exercise typically seen in young individuals. "External" and "internal" etiologies have been described, although the "internal" etiology is poorly understood. A clinical, radiographic, and anatomical study of eight patients with this disorder, secondary to an internal etiology, was undertaken to aid in the diagnosis and surgical treatment. Iliopsoas bursography with cineradiography revealed subluxation of the iliopsoas tendon to be an apparent cause of the snapping hip. The anatomy

Based on a review of the literature and experience with over 100 surgically treated thoracolumbar spinal injuries, the following information summarizes present knowledge of the subject. Reduction and internal fixation of the injured spine allows early mobilization of all patients, regardless of neurologic deficit, while protecting the neurologic structures from further injury and enhancing their recovery. The ability of the posterior ligamentous complex and the anterior bony column to withstand physiologic loads must be assessed by the history, physical examination, and radiography; then the i

This article is a presentation of the concept of the three-column spine. The concept evolved from a retrospective review of 412 thoracolumbar spine injuries and observations on spinal instability. The posterior column consists of what Holdsworth described as the posterior ligamentous complex. The middle column includes the posterior longitudinal ligament, posterior annulus fibrosus, and posterior wall of the vertebral body. The anterior column consists of the anterior vertebral body, anterior annulus fibrosus, and anterior longitudinal ligament. Major spinal injuries are classified into four d

The transoral approach to the lower third of the clivus and to the ventral aspect of the upper cervical spine is used in craniospinal malformations with or without dislocation as well as in basilar aneurysms, ventrally situated cranio-spinal tumours, fractures of the odontoid process, and in rheumatoid arthritis compressing the spinal cord. In consideration of the literature and ten personal cases the indications and techniques of the transoral approach in craniospinal malformations are discussed. According to our own experiences and those of other authors it is possible to expose the lower cl

We studied 100 consecutive patients with potentially unstable fractures and fracture-dislocations by multiplane computed tomography. The mechanism of failure of the middle osteoligamentous complex of the spine (posterior longitudinal ligament, posterior part of the vertebral body, and posterior annulus fibrosus) was determined by three-dimensional analysis. Three modes of middle-column failure were used to classify the injuries: axial compression (seventy-three patients), axial distraction (fifteen patients), and translation within the transverse plane (twelve patients). Fifty of eighty-six pa

Biomechanical parameters measured on lateral postural x-rays were correlated with the clinical syndromes of lumbar extension facet subluxation and herniations of the disc nucleus pulposes to determine their value as a differential diagnostic tool. Statistically significant differences in the disc angle of the involved spinal segment were found between the two test groups. Both groups also contrasted significantly with asymptomatic controls. There also appears to be distinct postural complexes associated with these syndromes, although it is not clear if these postural changes were secondary pro
