Dizziness is a non-specific and common condition in which the afflicted individual experiences abnormal sensations such as lightheadedness, imbalance, or a false sense of spinning (vertigo). The experience of "dizziness" can result from a wide spectrum of abnormal physiological states, including exhaustion, hypotension, and hypoglycemia, but could also indicate a serious underlying health issue. Since it has many potential generating causes, accurate identification of the underlying etiology of dizziness can present a challenge to clinicians, often resulting in ineffective treatments. OBJECTIVES: Traumatic cervical spine injuries in pediatric populations are uncommon, accounting for 1% to 10% of spinal trauma cases. Cervical facet dislocations are particularly rare and usually result from high-energy trauma. This study presents a rare instance of a C5/C6 facet dislocation caused by a low-energy mechanism during judo and discusses management strategies based on a literature review.
METHODS: This work includes a case report of a 15-year-old male with a C5/C6 facet dislocation sustained during judo practice. Imaging confirmed vertebral malalignment and a herniated disc withou

Open fracture-dislocations with significant malleolar bone and soft tissue loss, along with ligament injuries, are complex and often require extensive treatment and staged surgeries. The aim of this report is to highlight treatment strategies and key considerations, including surgical reconstruction with allografts or autografts and various flap techniques as viable options. Additionally, deltoid ligament repair or reconstruction is crucial for surgical success. We report the case of a 29-year-old woman with no significant medical history who sustained an open fracture-dislocation of the left

INTRODUCTION: Facet fracture dislocations of the subaxial spine pose notable risks of neurologic injury and spinal instability. The optimal surgical approach-whether anterior-alone, posterior-alone, or combined anterior-posterior-remains debated. The aim of this study was to evaluate the effectiveness, safety, and long-term outcomes of these surgical approaches.
METHODS: A retrospective analysis of patients presenting with cervical facet fractures at a single level I trauma center was conducted. They were divided into anterior-alone, posterior-alone, and combined AP surgical groups. Primary o

OBJECTIVE: Traumatic posterior atlantoaxial dislocation (TPAD) is uncommon, and related research is very limited. By analyzing the imaging characteristics of the anterior arch-odontoid process-transverse ligament complex in patients, the authors classify the outcomes of TPAD and elucidate its mechanisms of injury.
METHODS: This is a retrospective review of CT and MRI data on patients with TPAD treated at two clinical centers from June 2014 to March 2024. Through analysis and statistics on injuries to the anterior arch, odontoid process, and transverse ligament complex, the authors proposed a

AIM: To investigate whether atlantoaxial intra-articular fusion (AIF) can maintain sagittal balance stability in the cervical spine during follow-up.
MATERIAL AND METHODS: The data of 39 patients with anterior atlantoaxial dislocation who underwent AIF and 21 patients who underwent structural bone grafting (SBG) fusion were retrospectively reviewed. Radiographic variables, including T1 slope (T1S), C1?C2 angle, C2?C7 angle, C2?C7 sagittal vertical axis (SVA), and lateral atlantoaxial joint space height (LAAJSH), were measured preoperatively, postoperatively, and at the last follow-up. This case report describes a traumatic C2-C3 subluxation caused by a motorcycle accident following cervical spinal surgery. Traumatic C2-C3 subluxation is an uncommon but serious injury frequently accompanied by considerable neurological deficits. The coexistence of vascular injuries, particularly bilateral carotid artery dissection, complicates the management of such cases. We report a 48-year-old male patient who experienced a C2-C3 subluxation concurrent with bilateral carotid artery dissection and left vertebral artery occlusion as a result of a high-velocity motorcycle accident. Properly applied animal chiropractic (AC) can be safe and effective. Only licensed health care professionals who have completed extensive postgraduate training should offer AC. By providing AC, veterinarians can help their patients achieve the highest level of function, independence, and quality of life possible. In this article, we have described AC as a valuable modality that, by improving afferent input and positive modulation of the ventral horn cells and their efferent motor neurons, can positively affect patient strength, stability, and mobility. Therefore, improved conditioning, perform

Pediatric craniovertebral junction (CVJ) abnormalities comprise a heterogeneous and multifaceted collection of congenital and CVJ malformations that involve the occipital bone, C1, and C2, as well as associated ligamentous structures. They have the potential to severely impair neurologic function through cervicomedullary junction compression, instability, or vascular insufficiency. Early diagnosis and urgent surgical intervention are crucial to optimizing neurological outcomes in children with this condition, but the anatomical and biomechanical features of the pediatric spine present special

Instability of the occipitocervical junction may compress neural elements, resulting in progressive disability. After the Kickstand Rod technique was developed to correct for thoracolumbar scoliosis, the Candy Cane construct was developed for chin-on-chest deformity at the cervicothoracic junction as a similar three-rod approach. Demonstrated is a four-rod iteration utilized to stabilize the occipitocervical junction and correct condylar instability, termed the Catcher's Mitt technique. PURPOSE: In patients presenting with the sandwich fusion, characterized by C1 occipitalization and C2-C3 non-segmentation, leading to stress concentration at the atlantoaxial joint, there is an increased likelihood of atlantoaxial dislocation (AAD). The decision to proceed with transoral surgery is contingent upon the outcomes of intraoperative traction assessment. The complexity of intraoperative decision-making introduces a degree of uncertainty in preoperative planning, complicating both the surgical preparation and doctor-patient communication. This study aims to investigate the factors in

BACKGROUND: Atlanto-occipital dislocation (AOD) is a rare but devastating craniocervical injury typically seen in victims of high-energy trauma. Improvements in pre-hospital care have increased the number of patients presenting with AOD, thus early diagnosis and management are critical for survival. Despite this, there are no established diagnostic pathways, and the role of MRI is not well-characterized. This case series aims to highlight CT, MR, and clinical features of patients with AOD with the goal of evaluating and refining current paradigms on the pathology.

BACKGROUND/AIM: Rheumatoid arthritis (RA) is a chronic autoimmune disease affecting synovial joints, often causing to joint destruction and systemic comorbidities. Cervical spine involvement, especially atlantoaxial subluxation (AAS), can lead to spinal cord compression and neurological deficits. While disease-modifying antirheumatic drugs (DMARDs) are standard therapy, intolerance to agents like methotrexate (MTX) in elderly or comorbid patients limits options. Molecular hydrogen, with antioxidant and anti-inflammatory properties, has emerged as a potential adjuvant in autoimmune diseases. Th

INTRODUCTION: Anomalous anterolateral meniscofemoral ligaments (ALMFLs) are rare and underreported anatomical variations that are crucial for knee stability and joint biomechanics. This case report presents two instances of ALMFL, one associated with an anterior cruciate ligament (ACL) tear and the other with congenital ACL absence. These cases are significant as they contribute to the limited body of literature on ALMFL and its management, highlighting the importance of recognizing this anomaly during knee surgeries.
CASE REPORT: The first case involves a 28-year-old female with a torn ACL f

BACKGROUND: Atlantodental subluxation (ADS) is a serious condition that can result in sudden death. Measuring the anterior atlantodental interval (AADI method) is the gold standard for diagnosis but the complex anatomy of this region can make diagnosis difficult, especially for beginners. Therefore, we would like to use a simpler method, the Swischuk line method, to diagnose ADS. The purpose of our study was to evaluate the diagnostic performance of the Swischuk line method for ADS on lateral cervical spine radiographs compared to the AADI method.
METHODS: A retrospective study was conducted

The etiology and pathological morphology of atlantoaxial dislocation (AAD) are complex. Based on the pathological anatomical characteristics of AAD, combined with clinical techniques, it is essential to formulate reasonable classification criteria and develop corresponding treatment strategies for different types of AAD. The satisfactory outcome of surgical treatment for AAD can only be achieved through the comprehensive application of various atlantoaxial reduction techniques, tension band releasing techniques, internal fixation and fusion techniques. This article discusses the latest advance

A 42-year-old man presented with neck pain after a fall from a tree. Spine computed tomography (CT) illustrated the right C5 superior articular process fracture without displacement. Magnetic resonance imaging (MRI) confirmed the fracture and injury of the posterior ligament complex. Initially he was managed conservatively with a neck brace as there were no signs of instability or vertebral body misalignment. However, three days after discharge, right shoulder weakness and numbness of the right upper arm became prominent. X-rays and CT showed anterior slippage of the C4 vertebral body and lock
