The three column spine and its significance in the classification of acute thoracolumbar spinal injuries.

From a retrospective study of 412 thoracolumbar injuries, the author introduces the concept of middle column or middle osteoligamentous complex between the traditionally recognized posterior ligamentous complex and the anterior longitudinal ligament. This middle column is formed by the posterior wall of the vertebral body, the posterior longitudinal ligament and posterior annulus fibrosus. The third column appears crucial, as the mode of its failure correlates both with the type of spinal fracture and with its neurological injury. Spinal injuries were subdivided into minor and major. Minor inj

Lumbar facet syndrome: spinographic assessment of treatment by spinal manipulative therapy.

Spinographic parameters of a group of patients with extension facet subluxations and a group with herniated disc nucleus pulposus of the lumbar spine were evaluated pre- and post-manipulative treatment. A statistically significant decrease in the mean disc angle at the involved level in the extension facet subluxation group was seen with the mean post-treatment disc angle more closely resembling that of the controls. Likewise, a statistically significant increase occurred in the mean disc angle at the involved level in the disc herniation group post-treatment to one more closely resembling tha

Combined anterior and posterior fusion in seventy-three spinally deformed patients: indications, results and complications.

Seventy-three patients with complex spinal deformities undergoing combined staged anterior and posterior fusion were analyzed. The indications for the combined approach were (1) severe kyphosis with or without neural compromise; (2) major thoracolumbar or lumbar curves with trunk imbalance or marked pelvic obliquity; (3) severe congenital scoliotic curves associated with hemivertebrae or anterior unsegmented bars; (4) absent posterior elements, e.g., myelodysplasia, post laminectomy; and (5) failed anterior fusion. In 35 patients with kyphosis, the deformity was reduced on the average from 78

Spinal fractures and dislocations in children and adolescents.

During the years 1962 through 1978 a total of 42 children under the age of 15 years with fractures or dislocations of the spine were admitted to the Royal Brisbane Hospital Complex. Sixty-three percent of these children were males. The predominate causes of injury were fall/jump (40%), road traffic accidents (29%), and water-related injuries (19%). Half the injuries occurred to the cervical spine, while injuries at more than one level occurred in 35% of patients. Cord injuries occurred in 14% of patients, while skeletal and head injuries occurred in over half of the spinal injuries associated

[Roentgenological diagnosis and differential diagnosis of injuries of the upper cervical vertebral column (author's transl)].

Accurate differentiation of traumatic, inflammatory and congenital changes in the upper cervical vertebral column requires specific knowledge of the complex anatomic conditions in this segment of the spine, as well as of its embryological specialities and its regional pathology. This article deals with the analysis of the most important traumas of the upper cervical vertebral column and the relevant differential diagnosis.

Halo immobilization of cervical spine fractures. Indications and results.

Thirty-three patients with a spectrum of cervical spine fractures or subluxations were treated with immobilization by a halo apparatus. All spines were assumed to be unstable because of the nature of the fracture or because of a subluxation noted on spine films. Treatment consisted of immobilization and fracture reduction followed by application of a halo plaster cast or molded halo plastic vest. Patient acceptance was high. Complications were few and minor. No patient experienced neurological deterioration during treatment. Reduction was well maintained during an average halo immobilization p

A comparison of radiographic findings in fusion and nonfusion patients ten or more years following lumbar disc surgery.

Ninety-six patients who had undergone disc excision and midline spinal fusion and 36 patients who had had simple disc excision had spinal radiographs made 10 or more years postoperatively. Claw spurs were found most commonly at the L2-3 and L3-4 levels in fusion patients, particularly male laborers. Traction spurs with segmental hypermobility were found more commonly at the L4-5 level in patients whose spines were not fused, particularly women. Total lumbar flexion-extension was greater in nonfusion than in fusion patients, but the L1-3 mobility was greater in those who had undergone fusion, s

Injuries of the hindfoot.

Although the long term prognosis for many injuries of the hind foot is poor, prompt diagnosis and treatment can lead to a satisfactory result. Failure to treat these injuries promptly can lead to circulatory problems and skin necrosis. Stiffness and post-traumatic arthritis may occur despite adequate treatment due to the complex anatomy of the region. With the exception of certain rare fractures of the os calcis, displaced fractures of the talar neck and body and fracture-dislocations, injuries in this region of the foot heal best after closed treatment and early mobilization of the ankle and

Jefferson fracture with disruption of the transverse ligament. A case report.

A 20-year-old male incurred a vertex injury of the skull, disruption of the transverse ligament of the atlas, and a Jefferson fracture--lateral displacement of the lateral masses of the atlas in relation to the lateral margins of the second cervical vertebra. The combination of Jefferson fracture with transverse ligament disruption was not appreciated initially (or even late) because of the head injury. The hospital course was complicated by the patient's initial unconsciousness and restlessness caused by associated intracranial injury. Roentgenograms in all views of the cervical spine were es

Fractures of the arch of the lower cervical spine.

In a series of 285 cases of injuries of the lower cervical spine, fractures of the vertebral arch constituted 12.6%. 27 cases treated conservatively were analysed in view of their subtyping and clinical properties and with reference to the choice of treatment. Five different main subtypes of fractures of the vertebral arch of the lower cervical spine can be distinguished: 1) Bilateral fracture of lamina, without displacement of the vertebra or with subluxation. The assessment of potential instability is discussed, and criteria for its evaluation are presented; 2) Bilateral fracture of lamina c

[Several problems in the rehabilitation of patients with injuries to the lumbosacral portion of the spine and spinal cord].

Observation over clinical features distinguishing the course of traumas involving the sacro-lumbar vertebral column and spinal cord in 251 patients permitted it to recognize a number of points determining the orientation of a complex restorative treatment. Thanks to the use of a set of medico-rehabilitating measures it was possible to achieve positive results in the treatment of 222 (89 per cent) of the patients and 178 (75 per cent) of them could move without any outside help.
