Abnormal Cervical Facet Orientation and Congenital Stenosis: Case Reports and Biomechanical Implications-Illustrative Cases.

In patients with congenital cervical stenosis, the addition of abnormal cervical facet orientation (horizontal or hypoplastic joints) may predispose to segmental instability, subluxation, and canal compromise. Full assessment of facet anatomy is essential pre-operatively, as selected patients (e.g., those with demonstrable subluxation, widened facet joint spaces, or high risk of adjacent-segment instability) may require instrumented fusion in addition to decompression to maintain stability.

Dorsal C1-2 Spinal Cord Compression After Reduction of Atlantoaxial Posterior Dislocation Associated With Os Odontoideum: A Case Report and Literature Review.

BACKGROUND: Atlantoaxial posterior dislocation associated with os odontoideum is uncommon, and experience with its surgical management remains limited. Posterior reduction and C1-2 fixation can usually restore atlantoaxial alignment and stability. However, neurological safety after reduction depends not only on the quality of realignment but also on the residual space available for the spinal cord. In patients with congenital atlas hypoplasia or posterior arch osteophytes, reduction may unmask or aggravate dorsal C1-2 canal compromise. This mechanism of postoperative neurological deterioration

Anterior Arch Fracture of the Atlas on Postoperative Day 6 Following Unilateral C1 Laminectomy Requiring Occiput-to-C2 Fusion: A Case Report.

C1 laminectomy is a well-established posterior decompression procedure for craniovertebral junction stenosis. However, disruption of the ring structure of the atlas may alter load distribution across the anterior arch and lateral masses, occasionally resulting in atraumatic anterior arch fractures. Although several cases have been reported following bilateral C1 posterior arch resection or foramen magnum decompression, most developed weeks to months after surgery. In contrast, anterior arch fracture occurring during the acute postoperative period after unilateral C1 laminectomy has rarely been

Quantitatively Extreme Sagittal Orientation of the L5-S1 Facet Joints in Adolescent Dysplastic Spondylolisthesis: A Case Report and Mechanistic Considerations.

Background/Objectives: Dysplastic spondylolisthesis is associated with developmental abnormalities of the lumbosacral junction, but the contribution of extremely sagittal L5-S1 facet orientation in an adolescent with an intact pars interarticularis remains incompletely characterized. We aimed to describe this unusual quantitative morphologic phenotype and discuss its possible biomechanical implications. Methods: Clinical records and serial imaging of a 14-year-old girl with symptomatic low-grade L5-S1 dysplastic spondylolisthesis were reviewed. Standing radiographs, flexion-extension radiograp

A new etiology of nontraumatic C0-C1-C2-Complex instability - abnormality of musculus rectus capitis posterior minor: a case report.

PURPOSE: This study presents an abnormality of the musculus rectus capitis posterior minor (RCPmi) as a new etiological factor for nontraumatic sagittal plane instability in the C0-C1-C2-complex, with a focus on identifying the absence or atrophy of RCPmi on both sides. METHODS: A 36-year-old male patient presented with recurring neck pain (VAS 8/10) and tingling paresthesia in the entire left hand over a six-month period, without significant neurological deficits. Radiated arm pain was not reported. Imaging examinations revealed sagittal plane instability in the C0-C1-C2-complex, spinal cana

Prone Position MRI of the Lumbar Spine in Patients With Low Back Pain and/or Radiculopathy Refractory to Treatment.

BACKGROUND: There are patients with limiting low back pain (LBP) with or without radicular pain in whom conventional supine magnetic resonance imaging (MRI) show no causative pathology. Despite the limitations of dynamic axially loaded MRI examinations, these imaging studies have shown a striking ability to diagnose pathology unrecognized by conventional MRI. The difference in findings between supine and prone MRI with patient symptom correlation has not been studied. METHODS: Nineteen patients suffering from chronic moderate-to-severe LBP and/or radicular pain nonresponsive to conventional t

Clinical and Radiographic Features of the Atlantoaxial Dislocation Associated With Kashin-Beck Disease.

OBJECTIVES: Keshin-Beck disease (KBD) is a particular type of osteoarthritis that affects many joints. However, the deformity of atlantoaxial joint has been rarely reported in KBD, and therefore its clinical and radiograph features have not been identified. METHODS: We reviewed data in 14 patients who were diagnosed with atlantoaxial dislocation (AAD) in KBD at our institution. The demographic data, clinical history, imaging data, operative data, and Japanese Orthopaedic Association score were collected for evaluation. RESULTS: The mean age at presentation was 50 ± 1.7 years old. The most co

Seropositive Neuromyelitis Optica in a Case of Undiagnosed Ankylosing Spondylitis: A Neuro-Rheumatological Conundrum.

Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune astrocytopathy against foot processes of aquaporin-4 (AQP4) water channels. Patients with NMOSD tend to have other coexisting autoimmune/connective tissue diseases. However, AQP-4-antibody-positive NMOSD coexisting with ankylosing spondylitis (AS) is rare. AS is an immune-mediated disorder, a subset of axial spondyloarthropathies, which commonly manifests as chronic inflammatory back pain in young people, and it has a strong association with HLA-B27. In this study, a 35-year-old Indian man with an undiagnosed progressive axial spo

The association of lumbosacral transitional vertebral anomalies with acetabular dysplasia in adult patients with hip-spine syndrome : a cross-sectional evaluation of a prospective hip registry cohort.

AIMS: Some patients presenting with hip pain and instability and underlying acetabular dysplasia (AD) do not experience resolution of symptoms after surgical management. Hip-spine syndrome is a possible underlying cause. We hypothesized that there is a higher frequency of radiological spine anomalies in patients with AD. We also assessed the relationship between radiological severity of AD and frequency of spine anomalies. METHODS: In a retrospective analysis of registry data, 122 hips in 122 patients who presented with hip pain and and a final diagnosis of AD were studied. Two observers anal

Traumatic lateral spondyloptosis of L2 with complete neurological deficit: A case report.

UNLABELLED: Traumatic spondyloptosis of the lumbar spine is an uncommon and severe clinical entity, which is defined as complete fracture dislocation and subluxation (>100%) of one vertebral body in the coronal or sagittal plane from its adjacent vertebra. In coronal spondyloptosis the subluxated vertebral bodies lie beside each other, and the condition is lateraloptosis. CLINICAL CASE: A male patient aged 56 years had multiple injures with complete neurological deficit. Computed tomography(CT) revealed as spondyloptosis, which L2 detached from the rest of the spine, spinal canal stenosis, sa

Congenital Unilateral Atlanto-Occipital Rotatory Subluxation: Rare Cause of C1 Neuralgia.

STUDY DESIGN: Case report. OBJECTIVE: We report a rare case of congenital unilateral rotatory atlanto-occipital subluxation that presented with left C1 neuralgia. SUMMARY OF BACKGROUND DATA: Secondary occipital neuralgia is commonly attributed to pathologies of the atlanto-axial joint and C2/C3 nerve involvement. Our case depicts a model of slow creeping atlanto-occipital subluxation due to a rare left C1 superior articular facet dysplasia with C1 foraminal stenosis presenting as C1 neuralgia. We discuss the eitology and patho-anatomy of this rare undescribed presentation. METHODS: A 42-yea

Traumatic Unilateral L3-4 Jumped Facet Treated with Open Reduction and Short Segment Fusion.

BACKGROUND: Facet dislocations, or jumped facets, are part of a spectrum of flexion-distraction spine injuries. Bilateral and unilateral facet dislocations are commonly seen in the cervical spine. Traumatic jumped facets in the lumbar spine are rare injuries, and most involve the lumbosacral junction. Lumbar facet subluxations occur commonly in young patients owing to flexion-distraction forces on the lumbar spine at the time of injury combined with ligamentous laxity that exists in young patients. CASE DESCRIPTION: A 24-year-old man presented with a unilateral right facet dislocation at L3-4

Is the atlas size associated with the pathophysiology of symptomatic spinal canal stenosis at the C1 level?

Narrow cervical spinal canal is an important risk factor for the development of cervical myelopathy. Patients with this disease often present with congenital narrowness of the cervical spinal canal. While there are studies on patients with subaxial spinal canal stenosis (SAS), few examined the coexistence of congenital narrow spinal canal in patients with cervical myelopathy at the C1 level. We investigated the characteristics of patients with C1 stenosis (C1S) with special reference to the size of the atlas. Thirteen patients (8 men, 5 women, mean age 76 years) with C1S were retrospectively a

The impact of stenosis and translation on spinal cord injuries in traumatic cervical facet dislocations.

BACKGROUND CONTEXT: Although facet dislocations account for only 6% of cervical trauma, the consequences are often devastating. Cervical facet dislocations are associated with a disproportionate amount of spinal cord injuries; however, neurologic examination of patients is often difficult, as patients commonly present with reduced levels of consciousness. There are limited studies that have investigated the impact of spinal canal diameter and translation on neurologic injury following facet dislocations. PURPOSE: Review a consecutive series of patients with facet dislocations to assess the im
