Twenty-two patients (15 women and seven men) with Klippel-Feil syndrome were reviewed at an average age of 35 years (range, 26-57 years). The average age at diagnosis was 9 years (range, birth-32 years). All patients had a clinical neurologic and orthopaedic examination. Ten patients (45%) had abnormal findings on clinical examination, and 15 patients (68%) had at least one complaint that could be related to the syndrome. The roentgenograms were unremarkable in all patients, except for the typical findings of congenital fusion; no vertebral subluxation or stenosis was noted in any patient. The

STUDY DESIGN: The terminal event in disc space narrowing and facet subluxation of the lumbar spine can produce clinical symptoms of spinal stenosis. Reversal of this process via distraction was performed on ten cadaveric motion segments with documented stenosis.
METHODS: Computerized tomography and caliperic methods for the measurement of canal and foraminal areas were calculated in each segment after 5 and 10 mm of symmetrical distraction. Measurements were done in a blinded manner verified with orthographic software and statistical analysis.
RESULTS: Decompression of foraminal space was st

Neurologic outcomes were correlated with the cervical canal diameter for 33 patients who sustained unilateral or bilateral facet fractures, dislocations, subluxations, or perch injuries during a 9-year period. Lateral roentgenograms (target distance, 72 in.) were used to measure the canal and calculate the canal-body ratio (Torg's ratio) at the level of the vertebral injury. Frankel's classification system was employed to compare initial and final neurologic function. The most important factors that determined the long-term functional results after unilateral or bilateral injuries in the cervi

The acute traumatic central cord syndrome (ATCCS) is commonly stated to result from an injury which affects primarily the center of the spinal cord and is frequently hemorrhagic. To test the validity of this widely disseminated hypothesis, the magnetic resonance images [MRI] of 11 consecutive cases of ATCCS caused by closed injury to the spine were analyzed and correlated with the gross pathological and histological features of 3 cervical spinal cords obtained at post mortem from patients with ATCCS, including 2 of patients studied by MRI. The MRI studies were performed acutely (18 h to 2 days

Posterior internal fixation associated with bone fusion is indicated in those traumatic cervical spines in which an additional posterior direct action is required to achieve reduction and/or decompression. The internal fixation device is represented by screw plates allowing strong anchorage, short fixation, and light postoperative immobilization. Twenty-three cases of posterior internal fixation in traumatic cervical spines are presented. In selected cases, the results can be estimated as very good with respect to instability, spinal balance involvement, canal stenosis, and reversible neurolog

Pre- and postoperative CT scans of 35 patients with fractures of the thoracolumbar and lumbar spine (20 men and 15 women, with a mean age of 35 years), were evaluated with reference to the extent and shape of spinal stenosis. There were 24 patients with wedge-shaped fragments compressing the canal, the wedge being intact in 14 and broken in 10; 3 had fragmented wedges; and in 5, pedicular or arch fragmentation was combined with the dorsal vertebral fractures mentioned. Spinal stenosis was due to flexion-distraction injuries with angulation 3 patients, 1 of whom showed signs of dislocation and

The pathomechanism of injuries of the spinal cord due to injuries of the cervical spine in 158 patients treated during the last ten years has been presented. It has been determined that total and irreversible injuries of the spine occurred most often after fractures of the spine with displacement from hyperflexion (29.7 per cent) crushed fractures (24.7 per cent), and dislocation of the spine (24.7 per cent). In the majority of the patients, the spinal cord was irreversibly damaged already at injury, and in 29.2 per cent, the symptoms of deformity of the lumen of the vertebral canal and compre

The introduction of new techniques to the magnetic resonance imaging (MRI) armamentarium is beginning to provide an MRI examination that overcomes many of the disadvantages noted in earlier reports. An analysis of the various advantages and disadvantages of MRI, plain film myelography, and computed tomographic myelography points to a potential revision of the sequence of diagnostic studies and the workup of cervical degenerative disease. MRI might now be the appropriate first test for the evaluation of the cervical spine in a patient with symptoms referable to degenerative disease when therape

The author describes some particularities seen in the abnormal or pathological image of the cervical spine: the osteolysis of the cortical bone in the spinous processes, the "Y" shaped course of the corporeal veins, the notch in interspinous bursitis, and the main forms of constitutional stenosis of the cervical canal.

The pathomorphology of normal and degenerated human cervical spines that had been subjected to trauma was studied in detail by surface-cryoplaning of frozen autopsy specimens. Four cervical spines that had been surgically fused were also sectioned after removal of the metal. In young individuals, disc ruptures occurred that resulted in compromise of the vertebral canal. In degenerated spines, vertebral endplate ridges were frequently fractured. These injuries resulted in encroachment on the spinal cord as well as on the nerve roots in the foramen. Osteophytes from the uncinate processes also c

No firm statistical evidence exists establishing the superiority of the anterior or the posterior approach in the management of spondylostenosis, although some sense of order is evolving. In general the consensus suggests that in spondylostenosis, the anterior approach may be preferred for disc or segmental osteophyte intrusions limited to one or two levels. Laminectomy is the preferred procedure in patients with a narrowed canal and multiple level involvement. The surgeon's personal preference and experience remains the dominant factor. Patients with congenital stenosis involving all of the m

The purpose of this study was to define as a distinct clinical entity the syndrome of neurapraxia of the cervical spinal cord with transient quadriplegia. The sensory changes include burning pain, numbness, tingling, and loss of sensation, while the motor changes range from weakness to complete paralysis. The episodes are transient and complete recovery usually occurs in ten to fifteen minutes, although in some patients gradual resolution occurs over a period of thirty-six to forty-eight hours. Except for burning paresthesia, pain in the neck is not present at the time of injury and there is c

A 17-year-old white male patient sustained a cervical hyperextension injury while body surfing. Plain cervical radiographs, tomography, and CAT scan showed neither fracture nor subluxation, but congenital narrowing of the spinal canal and fusion of C2-C3 (Klippel-Feil). Clinically, he had a central cord syndrome, characterized by a motor dominant myelopathy. The conservative management of this patient with a central cord injury in the presence of spinal stenosis and a Klippel-Feil syndrome resulted in almost full recovery although he was quadriplegic initially. This constellation of findings r

During the last three years we have experienced 20 cases of atlanto-axial dislocation (AAD) of various types. The series consists of 10 cases of traumatic anterior pure AAD, 2 traumatic anterior AAD with dens fracture, 2 os odontoideum, 1 rheumatic AAD, 1 Jefferson's fracture, and 3 "rotatory dislocation" (1 pure traumatic rotatory dislocation and 2 rotation deformity after Wackenheim). All patients were studied by plain X-ray, tomography, myelography with water soluble contrast media and computed tomography (CT) scanning. All of the operated patients and a part of the non-operated patients we

The biologic cost-effectiveness of computed tomography (CT) versus myelography is so favorable to CT that it is now the method of choice for evaluating patients with less clear-cut clinical findings. CT is now used to detect lesions formerly difficult to diagnose, such as subluxation, arthrosis, facet osteophytes, and stenosis of the vertebral canal, as well as herniated disks and lateral disks. The findings in over 1,000 patients examined by CT for lumbosacral spinal pathology are documented. Associated or multiple abnormalities were present in about 60% of cases, with bulging or herniated di

The degree of lordosis and mobility of the cervical spine, the "formation of steps" at the top end of the cervical spine, the Atlas-Dens distance, the diameter of the bony cervical spine canal and the breadth of the retropharyngeal and retrotracheal space were measured on 360 lateral radiograms of the cervical spine in the median position, at maximum flexion and maximum extension. The subjects were 120 children aged three to 14 years. The discussion of the results draws attention to their practical value and to the importance of functional radiographical examination of the cervical spine durin

Computed tomography was applied in 29 patients with cervical spinal stenosis. In 8 cases there was a congenital narrowed spinal canal. In 18 cases we found dorsal spondylotic ridges of the vertebral bodies and in three cases an atlanto-dental dislocation. The complaints showed either radicular character or in case of myelopathy came out as para- and quadriplegia. In 25 cases the spinal sagittal diameter was a lot below a critical borderline of about 13 mm. The kind and localisation of the underlying process can be demonstrated very excellent by computed tomography.

Pseudospondylolisthesis is a subluxation of the lumbar vertebrae due to incompetent facet joints. The resulting stenosis of the lumbar spinal canal may impinge on the nerve roots of the cauda equina and induce neurogenic claudication. This syndrome is difficult to distinguish clinically from lower extremity claudication of vascular etiology. Accurate diagnosis requires radiographic examination of the spine.

Clinical, biological, histopathological and laboratory data are reported on a 13-year-old male with atlanto-axial, atlanto-occipital hypermobilities, cervical canal stenosis, hypoplastic atlas and hypoplastic first rib in the Ehlers-Danlos syndrome. He developed frequent attacks of sudden tetraplegia or tetraparesis and numbness below C2 due to trivial neck injuries. Examination revealed marked hypermobilities of all joints the cutis elastica (Fig. 1), spastic tetraparesis and fasciculation of the tongue. Occipito-atlantal hypermobility measured by Wiesel and Rhothmans' method revealed 5 mm ho

A comprehensive review of 188 spinal computed tomography (CT) scans done over a 9-month period was performed to establish both the shortcomings and applications of the technique. The value of intrathecal enhancement was reviewed in an attempt to establish indications for its use. The data base comprised 51 abnormal scans including cases of spinal trauma, dysraphism, intra- and extraspinal neoplasm, and spinal stenosis. Results indicate that: (1) the introduction of intrathecal contrast media greatly extends the morphologic capabilities of spinal CT in practically all cases where altered anatom
