Technical improvements and the use of intrathecal Metrizamide have made computer tomography of the spine a valuable diagnostic method. The value of CT in demonstrating morphological changes in the spine was investigated in 77 examinations of congenital malformation or inflammatory and degenerative disease. Measurements of the spinal canal provide more accurate information than conventional radiology and these are discussed. Catheters were inserted into the extradural space under direct vision at the time of surgery for prolapsed intervertebral disc or lumbar canal stenosis. In the post-operative period, diamorphine (3 mg in 5 ml water) was injected through the catheter when patients requested analgesia. Cross-sectional studies of the lumbar spinal canal were performed by computerised tomography in asymptomatic subjects and in patients with myelographic and clinical findings suggestive of lumbar stenosis. In most cases stenosis was found to be developmental or combined in nature. Computerised tomography is a useful adjunct to myelography in the evaluation of lumbar stenosis as it enables an accurate assessment to be made of the type, degree and extent of narrowing of the vertebral canal.

In a group of 400 patients operated upon at the Department of Neurology, Institute of Rheumatology in Warsaw in the years 1971-1978 for lumbar intervertebral disc prolapse in 20 cases (5%) canal stenosis was found as the cause of vertebral canal narrowing. Three clinical syndromes were isolated in this group: cauda equina intermittent claudication (4 cases), flaccid paraparesis of lower extremities (3 cases) and ischialgia with paralysis of an extremity (13 cases). The use of the water-soluble radiocontrast substance Dimer-X in lumbosacral myelography in 22 patients with a stable pain syndrome in the lumbosacral region showed that this method makes it possible to disclose the cause of the disease exactly and to reveal or rule out any neurosurgical pathology of this region. The authors describe two cases of obliteration of the terminal cistema as a complication developing late after removal of two intervertebral disc prolapse at the levels L4/L5 and L5/S1. Intensification of pains and neurological defects with development of sphincter disturbances developed 5 and 17 years after the operation. Repeated surgical intervention brought improvement. The authors stress that compression of the dural sac by a ring-like scar had contributed to disturbances in cerebrospinal fluid circulation in the terminal cistern and was probably the cause of its obliteration.

Observations on a series of 38 cases (35 Doberman Pinschers and three Great Danes) of the canine wobbler syndrome are described. Radiographic examinations suggested that the primary lesion is an intervertebral instability at C6/7, but with time, secondary changes of the disc degeneration and prolapse and vertebral body malformation causing stenosis of the vertebral canal occur. The operative results in 37 consecutive patients suffering from developmental stenosis of the lumbar spinal canal, compared with those in spondylosis and disc herniations, are discussed. The diagnostic certainty of stenosis, suspected on the bases of clinical and radiographic data, is reached only at the operating table. The authors evaluated 60 epidurographies performed in patients with suspected compressing lesions in the vertebral canal. Computed tomography (CT) and transverse axial tomography (TAT) were used to study the lumbar spines of 164 patients with persistent or recurrent low back pain and/or radiculopathy. Of those patients with previous spinal fusion and of those with previous disectomy, 43% and 28%, respectively, demonstrated bony stenosis of the lumbar spinal canal. Of the patients who underwent surgery for this narrowed canal, 91% showed clinical improvement.
