Percutaneous spinal decompression.

Symptoms arise when narrowing of the spinal canal, lateral recesses, or intervertebral foramina impinge upon the neural structures that traverse the spinal column. Pathology involving the structures that make up the spine can narrow the relevant spaces. Here a description of percutaneous procedures that address narrowing from disc disease, ligamentum Flavum hypertrophy, and devices that open and stabilize the spine at individual levels are presented.

Tomodensitometric bone anatomy of the intervertebral foramen of the lower cervical spine: measurements and comparison of foraminal volume in healthy individuals and patients suffering from cervicobrachial neuralgia due to foraminal stenosis.

PURPOSE: Degenerative foraminal stenosis of the cervical spine can lead to cervicobrachial neuralgias. Computed tomography (CT)-scan assists in the diagnosis and evaluation of foraminal stenosis. The main objective of this study is to determine the bony dimensions of the cervical intervertebral foramen and to identify which foraminal measurements are most affected by degenerative disorders of the cervical spine. These data could be applied to the surgical treatment of this pathology, helping surgeons to focus on specific areas during decompression procedures. METHODS: A descriptive study was

Importance of neural foraminal narrowing in lumbar spine fractures of low AIS severity.

OBJECTIVE: In recent years, based on injuries predicted using machine learning, there have been efforts to reduce imaging performed on trauma patients. While useful, such efforts do not incorporate results from studies investigating the pathophysiology of traumatic events. The objective of this study was to identify potentially symptomatic vertebral foramen narrowing in the presence of minor to moderate (AIS ≤ 2 levels of severity) thoracolumbar fractures sustained in motor vehicle crashes (MVCs). METHODS: Hospital records and images of patients admitted to a Level One trauma center between t

The Role of the Ligamentum Flavum Area as a Morphological Parameter of Lumbar Central Spinal Stenosis.

BACKGROUND: Hypertrophy of the ligamentum flavum (LF) has been considered as a major cause of lumbar central spinal stenosis (LCSS). Previous studies have found that ligamentum flavum thickness (LFT) is correlated with aging, disc degeneration, and lumbar spinal stenosis. However, hypertrophy is different from thickness. Thus, to evaluate hypertrophy of the whole LF, we devised a new morphological parameter, called the ligamentum flavum area (LFA). OBJECTIVES: We hypothesized that the LFA is a key morphologic parameter in the diagnosis of LCSS. STUDY DESIGN: Retrospective observational study

Morphological changes of the caudal cervical intervertebral foramina due to flexion-extension and compression-traction movements in the canine cervical vertebral column.

BACKGROUND: Previous studies in humans have reported that the dimensions of the intervertebral foramina change significantly with movement of the spine. Cervical spondylomyelopathy (CSM) in dogs is characterized by dynamic and static compressions of the neural components, leading to variable degrees of neurologic deficits and neck pain. Studies suggest that intervertebral foraminal stenosis has implications in the pathogenesis of CSM. The dimensions of the cervical intervertebral foramina may significantly change during neck movements. This could have implication in the pathogenesis of CSM and

Morphological and radiometrical study of the human intervertebral foramina of the cervical spine.

BACKGROUND: Degenerative changes of the cervical spine are an inevitable response to certain occupational status and aging processes. Compression of cervical nerve roots may result from disc degeneration, disc herniation or intervertebral foraminal stenosis. The precise and detailed anatomical knowledge of the intervertebral foramen of the cervical spine is essential for the diagnosis and management of cervical radiculopathy. The significance of the observations and findings of the present study was to elucidate the correlation between the morphology and disorders of the cervical intervertebra

Lumbosacral foraminal ratios and areas using MRI in medium-sized dogs.

OBJECTIVE: To examine conventional magnetic resonance imaging planes of the lumbosacral foramina to obtain objective measurements of foraminal size in medium-sized (20-28 kg) normal dogs. METHOD: Ten canine cadavers were evaluated using magnetic resonance imaging in neutral, flexed and extended position. Foraminal ratios, areas and lumbosacral angles were calculated and their relationship to body weight was evaluated. RESULTS: Foraminal ratios were found to be independent of body weight in medium sized dogs (p >0.42). Foraminal areas were dependent on body weight (p <0.05). Flexion and exten

Morphometric analysis of the lumbar intervertebral foramen in patients with degenerative lumbar scoliosis by multidetector-row computed tomography.

PURPOSE: How the lumbar neural foramina are affected by segmental deformities in patients in whom degenerative lumbar scoliosis (DLS) is unknown. Here, we used multidetector-row computed tomography (MDCT) to measure the morphology of the foramina in three dimensions, which allowed us to elucidate the relationships between foraminal morphology and segmental deformities in DLS. METHODS: In 77 DLS patients (mean age, 69.4) and 19 controls (mean age, 69), the foraminal height (FH), foraminal width (FW), posterior disc height (PDH), interval between the pedicle and superior articular process (P-SA

Transforaminal ligament may play a role in lumbar nerve root compression of foraminal stenosis.

Lumbar foraminal stenosis is a common pathological change, and lumbar nerve root compression in stenotic foramina was recently considered as one of the main causes of low back pain and leg pain. However, the exact mechanism of lumbar nerve root compression in foramina is still not clear. Previous studies indicated that loss of the intervertebral disc height could reduce the cross-sectional area of lumbar foramina, while lumbar nerve root compression by boundaries of foramina has not been observed in experimental reduction of the intervertebral disc height. Given the close anatomic relationship

Intervertebral disc: anatomy-physiology-pathophysiology-treatment.

This review article describes anatomy, physiology, pathophysiology and treatment of intervertebral disc. The intervertebral discs lie between the vertebral bodies, linking them together. The components of the disc are nucleus pulposus, annulus fibrosus and cartilagenous end-plates. The blood supply to the disc is only to the cartilagenous end-plates. The nerve supply is basically through the sinovertebral nerve. Biochemically, the important constituents of the disc are collagen fibers, elastin fibers and aggrecan. As the disc ages, degeneration occurs, osmotic pressure is lost in the nucleus,

Histological changes and temperature distribution studies of a novel bipolar radiofrequency heating system in degenerated and nondegenerated human cadaver lumbar discs.

OBJECTIVE AND STUDY DESIGN: The purpose of this experimentation was to investigate the safety of a novel cooled bipolar radiofrequency system by examining histology and monitoring temperature distribution in the disc, epidural space, and adjacent to the nerve roots. In our study we used two human cadaver lumbar spines, one moderately to severely degenerated and the other mildly degenerated. SETTING AND INTERVENTIONS: Radiofrequency ablation of the disc posterior annulus is a theoretically plausible technique to ablate the nociceptors and to modify collagen of the annulus fibrosus. A novel coo

Lumbar nerve root occupancy in the foramen in achondroplasia: a morphometric analysis.

UNLABELLED: Lumbar stenosis is common in patients with achondroplasia because of narrowing of the neural canal. However, it is unclear what causes stenosis, narrowing of the central canal or foramina. We performed a morphometric analysis of the lumbar nerve roots and intervertebral foramen in 17 patients (170 nerve roots and foramina) with achondroplasia (eight symptomatic, nine asymptomatic) and compared the data with that from 20 (200 nerve roots and foramina) asymptomatic patients without achondroplasia presenting with low back pain without neurologic symptoms. The measurements were made on

Imaging-based planning for spine surgery.

The planning of decompressive and reconstructive spine surgery is based on clinical findings and diagnostic imaging. The evaluation of segmental instability, but also of the risk of destabilization following a needed decompression of the spinal canal and/or neural foramina make complex spine surgery a challenge, bearing in mind the risk of failures in case of an inadequate operation. The insufficient correlation between imaging and clinical symptoms originating from the spine and its nerve roots has been frustrating for some decades. This review focuses on the new upright, dynamic-kinetic, i.e

Lumbar spinal stenosis, cauda equina syndrome, and multiple lumbosacral radiculopathies.

Narrowing of the vertebral canal, the lateral recess, or the neural foramina causes lumbar spinal stenosis. Stenosis results from degenerative changes that usually are superimposed on a congenitally narrowed spinal canal and can result in significant pain and disability, especially in the elderly. Signs and symptoms are related to the compression of neural and vascular elements from the limited canal space. The article reviews the anatomy and pathophysiology, clinical syndrome, diagnostic workup, and natural history of lumbar spinal stenosis to aid in proper diagnosis and treatment.
