A 78-year-old man presented with macroscopic hematuria. Non-contrast computed tomography (CT) performed at the initial visit suggested a possible renal malignancy. Subsequent contrast-enhanced dynamic CT revealed a 3-cm non-enhancing renal mass in the left kidney with a shadow defect, raising suspicion of renal pelvic carcinoma localized to the upper calyx. Enlarged lymph nodes were also observed around the left renal vasculature. Retrograde pyelography and urine cytology showed no evidence of urothelial carcinoma. Magnetic resonance imaging (MRI) demonstrated a well- capsulated non-enhancing

BACKGROUND: Perineal urethrostomy (PU) is a surgical procedure performed in male cats with recurrent urethral obstructions or irreversible urethral damage, creating a permanent urinary stoma. Although PU effectively relieves obstruction, it can lead to postoperative complications that affect feline quality of life.
CASE DESCRIPTION: The present study assessed the short and long-term outcomes and complications following PU in 10 male cats treated at the Small Animal Hospital, Faculty of Veterinary Medicine, University of Tehran. A retrospective review of the medical records was conducted for m

BACKGROUND: Denys-Drash syndrome (DDS) is a rare genetic disorder caused by pathogenic variants in the WT1 gene, presenting with the classical triad of progressive nephropathy, steroid-resistant nephrotic syndrome (SRNS), disorders of sex development (DSD) encompassing a spectrum of genital presentations, and increased predisposition to Wilms tumor. The condition is associated with early-onset nephrotic syndrome, rapid progression to kidney failure, and significant morbidity. Despite numerous case reports, a comprehensive synthesis of clinical characteristics and outcomes remains limited.

P53-related protein kinase (PRPK; TP53RK) is an evolutionarily conserved atypical kinase whose biology reveals an unexpected integration of translational control, cytoskeletal regulation, and cell-type-specific stress responses. As a core component of the TCTC complex (Threonyl-Carbamoyl Transferase Complex), PRPK participates in the universally conserved synthesis of the tRNA modification t6A, an essential process for translational fidelity, proteostasis, and cellular viability. Structural and biochemical studies from yeast to humans highlight a deeply conserved ATPase-driven mechanism in whi

Given the widespread reliance on insulin among diabetic patients, ensuring its stability is an essential task not only for preserving its therapeutic efficacy but also for safeguarding patient health and safety. In addition, insulin storage practices were frequently misunderstood that may compromise drug potency. Moreover, there were discrepancies in the shelf lives of the different insulin products offered in Tunisia. Therefore, two analytical methods for the analysis of insulin using high-performance liquid chromatography coupled to ultraviolet detector (HPLC-UV) and zinc using atomic absorp

Nephrotic syndrome is the leading cause of pediatric chronic kidney disease, particularly the steroid-resistant subtype (SRNS). The current diagnostic paradigm relies on prolonged steroid trials or invasive biopsies, often delaying optimal treatment. This study systematically synthesizes evidence from cohort studies on non-invasive molecular biomarkers predicting SRNS at onset. We searched PubMed, Scopus, ScienceDirect, and MEDLINE Ultimate via EBSCO for cohort studies published between January 2015 and September 2025. The study protocol was registered with PROSPERO (CRD420251147997), and qual

Combined renal vein thrombosis (RVT) and pulmonary embolism (PE) in young adults without nephrotic syndrome, malignancy, or an identified major thrombophilia is uncommon. Severe iron deficiency anemia (IDA) has been associated with increased thrombotic risk, although a causal relationship cannot be established from an individual case. A 21-year-old male patient presented with flank and chest pain. Diagnostic workup confirmed acute left RVT and an acute embolism of the posterior basal segmental artery of the right lower lobe with pulmonary infarction, concurrent with profound microcytic IDA (he

Permanent immunity to varicella zoster virus (VZV) develops following infection in most patients, with reinfections extremely rare. Here, we report a 6-year-old boy with refractory nephrotic syndrome who contracted VZV infection during B-cell depletion after rituximab treatment. Although developing febrile neutropenia, he was promptly treated with acyclovir, which was successful. The VZV immunoglobulin G antibody titer was weakly positive (3.3) by enzyme immunoassay performed 42 days after the initial diagnosis of VZV infection, confirming the presence of VZV, although antibody production was

BACKGROUND AND HYPOTHESIS: Primary membranous nephropathy (pMN) is a common cause of nephrotic syndrome in adults. Rituximab (RTX) is a first-line therapy targeting B cells; however, 30%-40% of MN patients exhibit treatment resistance or relapse. Studies have shown that anti-rituximab antibodies (ARA) may lead to RTX resistance through mechanisms such as drug neutralization. The objective of this study is to explore the relationship between the emergence of ARA and the clinical efficacy of initial RTX treatment in adult MN.
METHODS: Patients with MN who received RTX therapy were included in t

INTRODUCTION: AA amyloidosis is an uncommon complication of solid malignancies and is usually associated with chronic inflammatory conditions. Its association with gastrointestinal stromal tumors (GISTs) is exceptionally rare, with only a few cases reported in the literature.
CASE REPORT: We describe a 64-year-old woman with metastatic duodenal GIST and peritoneal carcinomatosis. Four months after diagnosis, she developed severe nephrotic syndrome, acute kidney injury, and rapidly progressive renal failure requiring hemodialysis. Kidney biopsy revealed glomerular and vascular AA amyloid depos

Von Hippel-Lindau (VHL) syndrome is a rare hereditary disorder characterised by the development of multiple benign and malignant tumours across various organs. Caused by mutations in the VHL tumour suppressor gene, it presents a wide range of clinical manifestations, including retinal haemangioblastomas, central nervous system tumours, renal cell carcinoma, pancreatic lesions, and phaeochromocytomas. This report describes the case of a 35-year-old woman who presented with visual impairment and abdominal masses. Imaging revealed typical VHL manifestations, including bilateral renal lesions and

BACKGROUND: Transplantation of adult-sized kidneys into very small pediatric recipients can be limited by pronounced donor-recipient size mismatch. Although intraperitoneal placement is often considered, extraperitoneal implantation may reduce abdominal morbidity, provided that the graft can be accommodated safely.
CASE PRESENTATION: We report transplantation of a left kidney from a 46-year-old deceased adult donor into a 15-month-old boy weighing 8.5 kg with congenital nephrotic syndrome and previous bilateral nephrectomy. The 14-cm graft had three arteries of comparable caliber. Because the

RATIONALE: The diagnosis of Fabry disease (FD) in heterozygous females remains clinically challenging, as random X-chromosome inactivation frequently leads to preserved α-galactosidase A (α-Gal A) activity and absence of classic clinical stigmata. While concurrent FD and IgA nephropathy (IgAN) is a rare comorbidity predominantly reported in East Asian females, its overlap with systemic autoimmune conditions such as undifferentiated connective tissue disease (UCTD) has not been documented. The shared immunopathogenic mechanisms linking lysosomal sphingolipid storage, immune-mediated glomerular

Urinary bladder involvement in inguinal hernias is an uncommon condition, predominantly affecting older men and potentially causing urinary obstruction, infection, and renal dysfunction. We present the case of a 79-year-old man with a six-month history of progressive right inguinoscrotal enlargement, lower urinary tract symptoms, hematuria, and a positive Mery's sign. Laboratory studies revealed renal dysfunction and urinary abnormalities in the setting of suspected urinary tract infection. Computed tomography demonstrated a right inguinoscrotal hernia containing the urinary bladder, with asso

We report a 55-year-old man who developed nephrotic syndrome 52 months after allogeneic hematopoietic stem cell transplantation for acute lymphoblastic leukemia. He had chronic graft-versus-host disease controlled with low-dose prednisolone and tacrolimus. Asymptomatic proteinuria was detected five months before referral, followed by progressive edema, massive pleural effusion, and ascites. At presentation, laboratory testing showed profound hypoalbuminemia, mildly reduced kidney function, and nephrotic-range proteinuria. Kidney biopsy demonstrated membranous nephropathy, with periodic acid-me

BACKGROUND: Posterior Reversible Encephalopathy Syndrome (PRES) is an acute neurological syndrome characterized by clinical and radiologic features that can occur in children under specific circumstances. Solid Organ Transplantation (SOT) and Hematopoietic Stem Cell Transplantation (HSCT) are the most commonly reported underlying conditions in children, although they have been studied separately and in small series.
OBJECTIVE: This study aimed to compare the characteristics of PRES in children following HSCT and SOT.
METHODS AND SETTINGS: We conducted a retrospective single-center study of c

INTRODUCTION: IgA nephropathy (IgAN) with nephrotic-range proteinuria (NRP) comprises two distinct presentations - with and without hypoalbuminemia, and comparative outcome data are limited, particularly for patients with preserved serum albumin.
METHODS: From a multicenter cohort of patients with IgAN and NRP, 127 patients were divided into a nephrotic syndrome (NS) group (NRP with hypoalbuminemia) and an NRP with normal serum albumin (NRP-NA) group. We compared baseline features, one-year outcomes, and progression to a composite endpoint of a ≥ 30% decline in estimated glomerular filtration
