Deep Learning-Enabled Interpretative SERS Analysis of Key Urinary Metabolites in Nephropathy Using Interfacial Charge-Engineered Silver Nanoparticles.

The definitive diagnosis of kidney disease relies on the invasive gold-standard method of kidney biopsy, but this provides information only about local characteristics. In contrast, urine samples can be collected noninvasively and contain a variety of metabolites associated with kidney disease. However, obtaining high-quality, high-dimensional biological information and identifying specific diagnostic biomarkers in urine also pose significant challenges. This study developed a detection strategy that uses interface charge engineering with patient urine samples to diagnose diabetic nephropathy,

Intussusception as a rare cause of abdominal pain in a child with idiopathic nephrotic syndrome: a case report and review of literature.

BACKGROUND: Intussusception is a rare cause of abdominal pain in children with nephrotic syndrome with only a few reported cases. The best management pathway has still not yet been determined, with two case reports describing the response to intravenous albumin infusion with loop diuretics, and others reporting the need for pneumatic, hydrostatic, or surgical reduction. This case report describes a child with nephrotic relapse with two simultaneous intussusceptions: an ileo-ileal intussusception that responded to intravenous albumin with Furosemide, and a colo-colic intussusception that needed

Pan-neurofascinopathies: a subgroup that mimic other neuroimmune-mediated diseases.

Pan-neurofascinopathies represent a distinct subset of inflammatory neuropathies characterized by autoantibodies that target multiple neurofascin (NF) isoforms, which are essential for nodal and paranodal architecture in peripheral nerves. These antibodies are associated with a broad clinical spectrum, often mimicking Guillain-Barré syndrome (GBS) or chronic inflammatory demyelinating polyradiculoneuropathy (CIDP), and exhibit significant variability depending on the immunoglobulin G (IgG) subclass involved. The IgG1 and 3 subclasses correlate with acute, fulminant presentations and complement

Quantitative Proteomics: Recent Advances in Glomerular Disease and Nephrotic Syndrome.

BACKGROUND: Chronic kidney disease (CKD) affects nearly 10% (∼850 million people) of the global population and stands among leading causes of morbidity, mortality, and healthcare cost. Current clinical biomarkers including serum creatinine, estimated glomerular filtration rate (eGFR), and proteinuria, are all lagging indicators of nephron damage only after glomerular and podocyte injury and are often coincident with tubule damage and fibrosis and indicative of advanced CKD. SUMMARY: This review summarizes recent progress in proteomics applications in CKD and glomerular diseases, emphasizing b

Assessment of Heavy Metal Exposure in Adult Patients with Minimal Change Disease: A Cross-sectional Case-Control Study.

BACKGROUND: Minimal change disease (MCD) is a major cause of nephrotic syndrome and is traditionally considered an immune-mediated podocytopathy. Environmental heavy metals have emerged as potential nephrotoxic factors that may contribute to glomerular injury and disease susceptibility. OBJECTIVES: The objectives of this study were to determine serum concentrations of selected heavy metals and evaluate their association with MCD in adults. MATERIALS AND METHODS: This cross-sectional case-control study was conducted in the Department of Nephrology at a tertiary care teaching hospital between

C3 Glomerulopathy in Autoinflammatory-Pancytopenia Syndrome: A Case Report.

Autoinflammatory-pancytopenia syndrome (AIPCS) is a rare autosomal recessive disease caused by a mutation in the deoxyribonuclease II (DNASE2) gene. It has mainly haematological manifestations and recurrent fevers. The kidney could be involved in the form of proteinuria, membranoproliferative glomerulonephritis (MPGN) or immune complex deposition rather than acute kidney injury (AKI) or C3 glomerulopathy (C3G). A six-year-old male child presented with high-grade fever, AKI, generalized edema and hypertension. He had a history of recurrent hospital admission with fever, arthritis, skin rash and

Safety and efficacy of prednisolone, mizoribine and lisinopril combination therapy for severe childhood IgA nephropathy.

BACKGROUND: Angiotensin-converting enzyme inhibitors (ACEi), such as lisinopril, have long been used for childhood IgA nephropathy (cIgAN). A two-year regimen including prednisolone, mizoribine, warfarin, and dipyridamole was previously shown to be superior to prednisolone and mizoribine alone in achieving proteinuria remission. Because warfarin nephropathy has been reported and the cumulative prednisolone dose used in earlier regimens was substantial, we evaluated a new two-year regimen consisting of prednisolone, mizoribine, and lisinopril. METHODS: Among 567 biopsy‑proven cIgAN cases treat

A Novel LAMA5 Variant in Adult-Onset Focal Segmental Glomerulosclerosis: A Case Report.

Focal segmental glomerulosclerosis (FSGS) is a common histopathological diagnosis among adult patients presenting with nephrotic syndrome. This pattern is associated with diverse underlying pathophysiological mechanisms, which may influence disease management, therapeutic response, and prognosis. Genetic forms of FSGS are caused by variants in genes encoding proteins expressed in podocytes or the glomerular basement membrane. Multiple genes have been implicated in the pathogenesis of genetic FSGS, and clinical presentation can vary depending on the specific gene involved. We report the case of

[Membranous nephropathy].

Membranous nephropathy (MN) is an autoimmune disease caused by circulating antibodies (ab) directed against an antigen expressed on glomerular epithelial cells (podocytes), which are a main component of the glomerular filtration barrier. This immune complex formation leads to a loss of selectivity of the filtration barrier and loss of large amounts of proteins in the urine. As a consequence, a nephrotic syndrome develops which is the clinical hallmark of MN. In 75% of patients, the pathogenic antibody is directed against the Phospholipase A2 receptor 1 (PLA2R1). PLA2R1-ab are highly specific f

Differences in urinary biomarker profiles in patients with nephrotic-range proteinuria with and without hypoalbuminemia: a single-center cross-sectional study.

BACKGROUND: Patients with chronic kidney disease (CKD) presenting as nephrotic-range proteinuria (NR, ≥3.5g/d) exhibit distinct clinical phenotypes with and without hypoalbuminemia. The underlying mechanisms need further investigation. METHODS: This retrospective study included patients with CKD hospitalized over the past two years with baseline 24-hour urine total protein (24-hUTP) exceeding 3.5g. Based on whether their baseline serum albumin (ALB) levels were ≤30 g/L, patients were assigned to two groups: the nephrotic syndrome (NS) group and the isolated nephropathy-range proteinuria (isol

Musculoskeletal Manifestations of Light Chain Amyloidosis: A Systematic Review.

INTRODUCTION: Diagnosing amyloid arthropathy remains challenging because its manifestations often mimic those of other inflammatory musculoskeletal diseases. This review aimed to describe the clinical, biological, and imaging characteristics of musculoskeletal involvement in light-chain (AL) Amyloidosis. METHODS: We conducted a review in accordance with the PRISMA guidelines. We searched SCOPUS and MEDLINE for case reports and series published between January 2000 and December 2023 that detailed musculoskeletal manifestations of AL amyloidosis. To minimize the risk of omitting relevant articl

Kimura Disease in an Adolescent With Type 1 Diabetes Mellitus: A Rare Co-occurrence of T Helper 2-Driven Eosinophilic Lymphadenopathy and Autoimmune Endocrinopathy.

Kimura disease (KD) is a rare, chronic T helper 2 (Th2)-driven inflammatory disorder characterized by painless head-and-neck lymphadenopathy or subcutaneous masses, peripheral and tissue eosinophilia, and markedly elevated serum immunoglobulin E (IgE). It predominantly affects young men of Asian descent. Although KD is regarded as an allergic or immune-dysregulatory rather than a classically autoimmune condition, it has been described in association with nephrotic syndrome, suggesting an underlying systemic immunologic disturbance. Its co-occurrence with type 1 diabetes mellitus (T1DM), an arc

Complete remission of minimal change disease after antiviral monotherapy in a patient with immune-active chronic hepatitis B: A case report.

RATIONALE: Minimal change disease (MCD) is characterized by diffuse podocyte foot process effacement on electron microscopy, minimal structural changes on light microscopy, and the absence of immune complex deposition on immunofluorescence. In adults, MCD accounts for approximately 10% to 25% of nephrotic syndrome cases and is typically idiopathic, although secondary causes such as infections, medications, malignancies, and immune disorders have been reported. The association between hepatitis B virus (HBV) infection and glomerular disease has been well established in membranous nephropathy an

Monoclonal antibodies associated IgA nephropathy or other nephropathies.

The use of monoclonal antibodies (mAbs) has revolutionized the treatment of autoimmune, inflammatory, and neoplastic diseases. However, accumulating evidence indicates that biologic therapies may be associated with renal adverse events, including various types of glomerulonephritis. The aim of this review was to summarize all reports available in the scientific literature regarding the occurrence of IgA nephropathy after monoclonal antibody treatment. Additionally, the risk of other glomerulonephritis after biologic therapy was discussed. IgA nephropathy (IgAN) is the most commonly reported gl
