We report the case of a 17-year-old male with an unremarkable medical history, admitted for acute nephritic syndrome associated with acute renal failure. His presentation occurred three weeks after a generalized febrile cutaneous rash that had been diagnosed as measles. Renal biopsy revealed acute glomerulonephritis (AGN) with cellular crescents; the outcome was favorable under treatment. Measles is a highly contagious disease, and a resurgence in its prevalence has been observed worldwide, including in Morocco. Although the measles virus is known to be nephritogenic and may lead to AGN, its a

INTRODUCTION: The number of kidney biopsies performed in patients with diabetes has increased rapidly over the last two decades. However, the overall value of the biopsy has been questioned because any coexisting non-diabetic kidney disease (NDKD) is not identified in many patients. Here, we quantify the frequency of identifying NDKD and examine clinical indications, demographic factors, and histologic parameters that increase the odds of finding NDKD and the impact on developing end stage kidney disease.
METHODS: A retrospective analysis of clinical and pathologic parameters of 49,075 biopsi

Treatment for adult IgA vasculitis (IgAV), particularly severe cases, is not well established, although recent reports have suggested potential efficacy of rituximab (RTX). Here, we report a case highlighting the effectiveness of RTX based on pathophysiological and pathological considerations of IgAV, as well as a treatment protocol that has not been previously described. A 50-year-old man receiving glucocorticoids for cutaneous IgAV developed nephrotic syndrome and acute nephritic syndrome. Renal biopsy showed marked endocapillary and extracapillary proliferation with IgA deposition, consiste

BACKGROUND: Post-streptococcal glomerulonephritis (PSGN) is a common cause of acute nephritic syndrome in children. Rarely, it may result in life-threatening complications, including acute pulmonary edema and critical hyperkalemia.
CASE PRESENTATION: We report a 10-year-old Yemeni girl (25 kg) presenting with severe respiratory distress, irritability, and generalized pitting edema. Laboratory tests confirmed PSGN with markedly reduced complement C3 (42.2 mg/dL) and nephritic urine sediment containing numerous red blood cells and casts. The patient developed critical hyperkalemia (7.0 mmol/L)

Neuromyelitis optica spectrum disorder (NMOSD) is an autoimmune demyelinating disease manifesting with transverse myelitis and optic neuritis. Systemic lupus erythematosus (SLE) also commonly affects kidneys and central nervous system. Here, we describe a case of proven NMOSD for almost 16 years with multiple relapses over a period of time, presented to us with acute nephritic syndrome. Her kidney biopsy revealed class 4 lupus nephritis. She responded to steroid, rituximab, and mycophenolate mofetil and achieved complete remission at 3 months. Subsequent relapse at 6 months was initially treat

Systemic vasculitis is traditionally classified according to the predominant size of affected vessels; however, phenotypes affecting multiple vascular calibers may not be adequately captured by rigid definitions and require integrated clinicopathological interpretation. Here we report the case of a 70-year-old Caucasian man who presented with persistent systemic inflammation initially suggestive of infection. Although inflammatory markers decreased during antibiotic therapy, residual inflammation persisted, and the subsequent clinical course was characterized by myalgia, pulmonary involvement,

Acute post-streptococcal glomerulonephritis (APSGN) is the leading cause of acute glomerulonephritis in children worldwide, owing to the high incidence of nephritogenic strains of group A beta-haemolytic streptococcal throat and skin infections. The incidence of APSGN is declining in developed countries because of better access to clean drinking water, enhanced sanitation and improved hygiene. However, it remains a public health problem and a significant cause of hospitalization in low- and middle-income countries. The pathogenesis of APSGN involves the activation of the alternate complement p

INTRODUCTION: Post-streptococcal acute glomerulonephritis (PSAGN) is a disease in which patients develop hematuria and leg edema following streptococcal infection, which may lead to acute nephritic syndrome and nephrotic syndrome. We present a case involving acute kidney injury and nephrotic syndrome with nonspecific pathological findings for PSAGN, which posed diagnostic challenges.
CASE PRESENTATION: A 25-year-old man presented with leg edema, cough, nephrotic syndrome, severe kidney dysfunction, and hematuria. Blood tests showed elevated antistreptolysin O levels and decreased complement C

BACKGROUND: The combination of hematuria, proteinuria and a reduced glomerular filtration rate (GFR) in patients with no previous diagnosis of chronic kidney disease is widely considered a strong indication for kidney biopsy. This study aimed to compare the frequencies of diseases leading to this symptom constellation, and to explore differences related to age and sex using data from kidney biopsy registries in Europe and Japan.
METHODS: Data were retrieved from national or regional kidney biopsy registries in Japan, Sweden, Norway, Scotland, Austria, Croatia and Turkey from January 1, 2007,

BACKGROUND: Atypical hemolytic uremic syndrome (aHUS) is a rare thrombotic microangiopathy (TMA) caused by complement dysregulation, leading to microangiopathic anemia, thrombocytopenia, and acute kidney injury (AKI). Complement over activation typically results from genetic mutations in alternative pathway proteins. The genetic profile varies regionally and influences clinical phenotype and outcomes.
METHODS: We conducted a retrospective observational study in all 27 patients (12 children, 15 adults) diagnosed with aHUS, at "Fundeni" Clinical Institute, between January 2017 and January 2025.

Proliferative glomerulonephritis with monoclonal immunoglobulin deposition disease (PGNMIDD) is a rare entity. We evaluated the clinicopathological features of PGNMIDD and the effectiveness of different treatment regimens in 13 cases diagnosed using kidney biopsy. Most had chronic kidney disease followed by acute nephritic syndrome, rapidly progressive glomerulonephritis, and nephrotic syndrome. Membranoproliferative glomerulonephritis was the most common pattern of renal injury. Three patients had abnormal bone marrow studies. Different treatment regimens were deployed; >60% had partial remis

Changes over time in prevalence of glomerulopathies (GP) have been reported worldwide, but given the scarcity of data from Romania, we assessed the frequency of biopsy-proven GP over a 10-year period. This single-centre retrospective study enrolled 1 254 adults with GP on native kidneys diagnosed between 01.01.2008 - 31.12.2017, whose cases were extracted from the kidney biopsy (KB) registry of the hospital. Those with repeated KB and insufficient tissue sample or missing data were all excluded. Demographic, clinical, laboratory and histological data were analyzed and compared between subjects

INTRODUCTION: Monoclonal gammopathy of renal significance (MGRS) is a rare entity describing patients with renal impairment related to the secretion of immunoglobulins without hematological criteria for treatment of a specific disease. We present 3 cases of MGRS identified at our center that were either rare or difficult to diagnose. Case Presentations. The first patient presented with monoclonal membranoproliferative glomerulonephritis in the context of known chronic lymphocytic leukemia (CLL), diagnosed about 10 years prior. She presented with nephritic syndrome with serum protein electropho

Renal involvement is a common occurrence in patients with immuno-rheumatological diseases (IRDs). Several instances of glomerulonephritis (GN) occur in the setting of IRD and complicate the clinical course of an underlying condition. The aim of this study was to observe the spectrum of nephropathies according to age, kidney function, history of IRD at the time of biopsy, and histopathological kidney diagnosis. We evaluated data relating to 699 consecutive kidney native biopsies (female 52.1%) with a median age of 48 years (IQR 34-62) performed in adult patients collected over 15 years. The stu

The patterns of infection-related glomerulonephritis (IRGN) are rapidly changing in terms of age at presentation and sources of infection. The existing literature on the use of steroids in IRGN is inconsistent. A diabetic male in his sixties presented with features of anasarca, bilateral flank pain, and acute pulmonary edema. He had a non-healing ulcer over his right leg, with pus culture showing growth of methicillin-resistant Staphylococcus aureus (MRSA). Computed tomography (CT) of the kidneys, ureter, and bladder (KUB) showed features of bilateral pyelonephritis. The patient went on to dev

BACKGROUND: Germline heterozygous gain-of-function (GOF) mutations in the PIK3CD gene lead to a rare primary immunodeficiency disease known as activated phosphoinositide 3-kinase (PI3K) δ syndrome type 1(APDS1). Affected patients present a spectrum of clinical manifestations, particularly recurrent respiratory infections and lymphoproliferation, increased levels of serum immunoglobulin (Ig) M, Epstein-Barr virus (EBV) and cytomegalovirus (CMV) viremia. Due to highly heterogeneous phenotypes of APDS1, it is very likely that suspected cases may be misdiagnosed.
METHODS: Herein we reported three

BACKGROUND/AIM: Simple inflammatory biomarkers, such as neutrophil to lymphocyte ratio (NLR), could serve as prognosis indicators in patients with Coronavirus disease 2019 (COVID-19). The utility of on-admission inflammatory biomarkers in predicting outcomes was investigated in patients suffering from severe COVID-19 infection.
METHODS: We performed a retrospective study to assess the role of white blood count (WBC), neutrophils (N), lymphocyte (L), platelets (PLTs), C-reactive protein (CRP), reverse transcription polymerase chain reaction (RT-PCR), NLR (N/L), PLR (P/L), dv (derived variation
