[MACROSCOPIC HEMATURIA AND LYMPHATIC SWELLING AS INDICATORS FOR SURGERY IN PAPILLARY RENAL CELL CARCINOMA MIMICKING A HEMORRHAGIC CYST: A CASE REPORT].

A 78-year-old man presented with macroscopic hematuria. Non-contrast computed tomography (CT) performed at the initial visit suggested a possible renal malignancy. Subsequent contrast-enhanced dynamic CT revealed a 3-cm non-enhancing renal mass in the left kidney with a shadow defect, raising suspicion of renal pelvic carcinoma localized to the upper calyx. Enlarged lymph nodes were also observed around the left renal vasculature. Retrograde pyelography and urine cytology showed no evidence of urothelial carcinoma. Magnetic resonance imaging (MRI) demonstrated a well- capsulated non-enhancing

Short- and long-term outcomes and complications following feline perineal urethrostomy.

BACKGROUND: Perineal urethrostomy (PU) is a surgical procedure performed in male cats with recurrent urethral obstructions or irreversible urethral damage, creating a permanent urinary stoma. Although PU effectively relieves obstruction, it can lead to postoperative complications that affect feline quality of life. CASE DESCRIPTION: The present study assessed the short and long-term outcomes and complications following PU in 10 male cats treated at the Small Animal Hospital, Faculty of Veterinary Medicine, University of Tehran. A retrospective review of the medical records was conducted for m

Inguinoscrotal Bladder Herniation With Mery's Sign: Individualized Timing and Primary Repair in the Setting of Suspected Urinary Infection.

Urinary bladder involvement in inguinal hernias is an uncommon condition, predominantly affecting older men and potentially causing urinary obstruction, infection, and renal dysfunction. We present the case of a 79-year-old man with a six-month history of progressive right inguinoscrotal enlargement, lower urinary tract symptoms, hematuria, and a positive Mery's sign. Laboratory studies revealed renal dysfunction and urinary abnormalities in the setting of suspected urinary tract infection. Computed tomography demonstrated a right inguinoscrotal hernia containing the urinary bladder, with asso

Long-term outcomes of initial tonsillectomy and steroid pulse therapy in children with IgA nephropathy and heavy proteinuria: a single-center retrospective study.

BACKGROUND: Clinical practice guidelines do not recommend tonsillectomy combined with steroid pulse therapy (TSP) as the initial treatment of severe IgA nephropathy (IgAN) in children. Therefore, the long-term prognosis following TSP is unknown in IgAN children with heavy proteinuria. METHODS: This retrospective study aimed to determine the long-term outcome of children with IgAN and heavy proteinuria (urinary protein-to-creatinine ratio, ≥ 1.0 g/g) following TSP as the initial treatment. The primary endpoint was the probability of achieving treatment-free clinical remission (CR: disappearanc

Hematological Malignancies With Multiple Primary Cancers: A Rare Case Presentation.

BACKGROUND: Two or more primary cancers that arise in two different patients are referred to as multiple primary cancers. We record those cases because optimal therapy requires interdisciplinary cooperation. CASE 1: A 53-year-old male presented with intermittent hematuria for one year, fever, burning micturition, appetite loss, and a 3 kg weight loss over 2 months. His CBC showed 81% atypical cells, and bone marrow aspiration and flow cytometry indicated Precursor B-ALL. He started on the BFM-2002-protocol but had persistent hematuria. The USG of the whole abdomen revealed a urinary bladder m

The Micro-hole Zone Technology as a new era in urinary drainage: a narrative review from bench to bedside.

INTRODUCTION: Intermittent catheterization (IC) remains the standard of care for patients who are unable to empty their bladder. However, incomplete bladder emptying and urinary tract infections (UTIs) can persist, leading to complications such as recurrent infections, episodes of hospitalizations, and decreased quality of life. The Micro-hole Zone Technology (MHZT) catheter (Luja™) represents an innovative catheter design that aims to address known mechanical risk factors for UTI by minimizing flow-stops, mucosal microtrauma, and residual urine, all key contributors to infection risk. METHOD:

Stenting for symptomatic left renal vein compression in patients with complex pelvic venous disease is safe on long-term follow-up.

OBJECTIVE: The ideal treatment for symptomatic left renal vein (LRV) compression continues to be debated, with several available open and endovascular options. Concerns about percutaneous stenting have been raised due to risk of stent migration, fracture, and erosion. This study aims to assess the long-term safety, patency, and effectiveness of LRV stenting. METHODS: A retrospective institutional chart review of all patients undergoing LRV stenting for symptomatic LRV compression between 2010 and 2024 was conducted. Baseline characteristics, operative details, complications, reinterventions,

Rare Inflammatory Myofibroblastic Tumor of the Urinary Bladder: A Case Report and Review of the Literature.

Background: An inflammatory myofibroblastic tumor (IMT) is a rare mesenchymal tumor, sometimes with urinary bladder involvement (though this is extremely uncommon). Due to its rarity, the exact etiology and optimal treatment strategy remain unclear. Methods: A review of the existing literature on IMT of the urinary bladder was performed. Results: We report a case of a 32-year-old female presenting with frequent urination, hematuria with clots, and lower abdominal pain for one month. Initially misdiagnosed as acute cystitis, the symptoms persisted despite antibiotic therapy. Laboratory findings

Persistent follicular cystitis following intravesical gemcitabine: a case series and literature review.

BACKGROUND: Intravesical gemcitabine is widely used for non-muscle-invasive bladder cancer (NMIBC) due to its favorable safety profile. While transient irritative symptoms are common, persistent inflammatory cystitis remains poorly characterized. METHODS: We conducted a retrospective observational study at a tertiary referral center. Patients receiving intravesical gemcitabine following transurethral resection of bladder tumor (TURBT) between 2022 and 2024 were screened using a stringent operational definition: (1) irritative symptoms lasting ≥ 8 weeks; (2) persistent sterile pyuria with nega

A case report of concomitant anti-glomerular basement membrane disease, IgA nephropathy, and membranous nephropathy.

BACKGROUND: The coexistence of anti-glomerular basement membrane (anti-GBM) disease, IgA nephropathy (IgAN), and membranous nephropathy (MN) represents an exceptionally rare glomerular pathology triad, with only one previously documented case. This study explores the underlying mechanisms linking these distinct entities and reports a novel case associated with persistent elevation of serum amylase and lipase, thereby expanding the clinical spectrum of this complex disease combination. METHODS: A 51-year-old woman presenting with recurrent macroscopic hematuria and rapidly progressive renal dy

Rapid Remission of Steroid-Refractory IgA Nephropathy With Targeted-Release Budesonide: A Case Report.

IgA nephropathy (IgAN) is the most common primary glomerulonephritis worldwide and a major cause of progressive chronic kidney disease. Systemic corticosteroids are commonly used in high-risk patients but are frequently limited by inadequate response and significant adverse effects. We present the case of a 46-year-old woman with biopsy-confirmed IgAN who developed persistent microscopic hematuria and nephritic-range proteinuria. After six months of treatment with high-dose systemic prednisone, renin-angiotensin-aldosterone system blockade, and a sodium-glucose cotransporter-2 inhibitor, there

"Mesh on the Move" Transabdominal Preperitoneal Mesh Erosion into the Urinary Bladder after Laparoscopic Inguinal Hernia Repair.

Mesh erosion into the urinary bladder is an uncommon but potentially serious late complication of laparoscopic inguinal hernia repair, particularly following transabdominal preperitoneal (TAPP) and totally extraperitoneal (TEP) approaches. Proposed mechanisms include inadequate peritoneal coverage, excessive mesh fixation, postoperative infection, and chronic inflammatory responses. Clinical presentation is often delayed and nonspecific, leading to diagnostic challenges and prolonged morbidity. We report the case of a 60-year-old male with recently diagnosed hypertension and diabetes mellitus

Predisposing factors for angioembolization in persistent hematuria after percutaneous nephrolithotomy: a retrospective analysis.

BACKGROUND: Hemorrhage is one of the most significant complications of percutaneous nephrolithotomy (PCNL) and may lead to clinically significant persistent hematuria requiring digital subtraction angiography (DSA) and transcatheter arterial embolization (TAE). Identifying predictors of TAE may improve perioperative planning and risk stratification. OBJECTIVES: To determine predictors of need for TAE in patients evaluated with DSA for persistent post-PCNL hemorrhage. DESIGN: Single-center retrospective observational study. METHODS: From 2021 to 2024, a total of 2947 tubeless PCNL procedures

Persistent hematuria after PCNL: a rare case report of renal pseudoaneurysm managed with endovascular coil embolization in Indonesia.

INTRODUCTION: Percutaneous nephrolithotomy (PCNL) is a standard treatment for renal stones, but delayed hemorrhage from renal artery pseudoaneurysm (RAP) remains a rare yet serious complication. Early diagnosis and intervention are essential to prevent hemodynamic failure and renal loss. We present an uncommon case of a renal pseudoaneurysm developing after PCNL. PRESENTATION OF CASE: A 64-year-old male presented with recurrent gross hematuria 1 month after PCNL. He had prior episodes of clot retention requiring cystoscopy and multiple transfusions. On admission, he exhibited persistent hemat

From Purpura to pulmonary hemorrhage: dermatological clues to systemic vasculitis - a case of granulomatosis with polyangiitis.

INTRODUCTION: Granulomatosis with polyangiitis (GPA) is a rare ANCA-associated vasculitis that can convincingly mimic IgA vasculitis when it presents with palpable purpura and a leukocytoclastic biopsy. Failure to perform direct immunofluorescence (DIF) and ANCA testing at presentation is a recognized cause of diagnostic delay and irreversible organ damage. CASE PRESENTATION: A 47-year-old woman, previously labeled as IgA vasculitis on the basis of a leukocytoclastic skin biopsy without DIF, returned 1 year later with recurrent necrotizing leg ulcers, ENT involvement, and hemoptysis. Strong c

Primary antiphospholipid syndrome presenting with unilateral renal infarction and C4d-positive cortical necrosis: a case report with a pooled analysis of 24 cases.

Primary antiphospholipid syndrome (APS) is an autoimmune thrombophilic disorder characterized by recurrent venous or arterial thrombosis and/or pregnancy morbidity in the presence of persistent antiphospholipid antibodies (aPL). Renal involvement in APS is heterogeneous and may include renal artery thrombosis, renal vein thrombosis, thrombotic microangiopathy, and chronic APS nephropathy. However, unilateral renal infarction as the first and isolated manifestation of primary APS is exceedingly rare. We report a 23-year-old man with no prior medical history who presented with fever, flank pain,

BCGitis or genitourinary tuberculosis? Diagnostic dilemma following intravesical Bacillus Calmette-Guérin therapy: a case report.

INTRODUCTION AND IMPORTANCE: Intravesical Bacillus Calmette-Guérin (iBCG) therapy is a cornerstone in the management of non-muscle-invasive bladder cancer. Although generally safe, it can rarely lead to granulomatous inflammatory complications that closely mimic recurrent malignancy or genitourinary tuberculosis, particularly in tuberculosis-endemic regions. CASE REPORT: A 47-year-old male with type 2 diabetes mellitus and high-grade T1 non-muscle-invasive bladder carcinoma underwent transurethral resection followed by iBCG induction and maintenance therapy.
