Gastrointestinal features of pediatric iga vasculitis and their association with renal complications: an observational study.

UNLABELLED: Immunoglobulin A (IgA) vasculitis is the most common systemic vasculitis in childhood, primarily affecting the skin, gastrointestinal system (GIS), joints, and kidneys. This study aimed to evaluate the clinical and laboratory characteristics of mild and severe GIS involvement in pediatric patients with IgA vasculitis and to investigate its association with renal involvement. A retrospective review was conducted on 794 pediatric patients diagnosed with IgA vasculitis between 1997 and 2024. Demographic data, clinical findings, and laboratory parameters were collected from patient rec

Opioid Withdrawal Misdiagnosed as a Crohn's Flare.

INTRODUCTION: There is an increased incidence of opioid use disorder (OUD) originating from prescription misuse. Diagnosing OUD remains challenging, particularly in patients with multiple medical comorbidities where other potential etiologies may cause overlapping symptoms. CASE PRESENTATION: This report highlights a 28-year-old male with Crohn's disease (CD) with recurrent hospitalizations of abdominal pain, hematuria, nausea, vomiting, and diarrhea. An extensive workup was negative for a Crohn's flare; however, the patient's symptoms improved within 48 h of admission, coinciding with opioid

Recurrent Thrombotic Thrombocytopenic Purpura Associated With Helicobacter pylori: More Than a Gut Feeling.

Thrombotic thrombocytopenic purpura (TTP) is a rare, life-threatening hematologic disorder characterized by severe ADAMTS13 deficiency, leading to uncontrolled platelet aggregation, microvascular thrombosis, and multi-organ dysfunction. Although the pathogenesis of immune-mediated TTP (iTTP) is primarily autoimmune, infectious agents, including Helicobacter pylori (H. pylori), have been implicated as potential triggers. However, the association between H. pylori and iTTP remains poorly defined. We describe a case of a 33-year-old male presenting with hematuria, thrombocytopenia, and laboratory

Refractory Bleeding From an Ileal Conduit in a Patient With Antiphospholipid Syndrome: A Multidisciplinary Management Challenge.

Bleeding from an ileal conduit is a rare but potentially life-threatening complication, typically associated with portal hypertension and stomal varices. However, intra-conduit hemorrhage in benign settings is exceedingly rare. We report a case of a 54-year-old woman with a complex history including antiphospholipid syndrome (APS), who presented with persistent bleeding from an ileal conduit, ultimately requiring multidisciplinary intervention after failed conservative management. This case illustrates the challenges of balancing anticoagulation in high thrombotic risk patients and the importa

Immunoglobulin A Nephropathy: Molecular Pathogenesis and Targeted Therapy.

Immunoglobulin A nephropathy (IgAN), the most prevalent primary glomerulonephritis globally, is characterized by mesangial IgA deposition and heterogeneous clinical trajectories. Historically, management relied on renin-angiotensin system inhibition and empirical immunosuppression, yet high lifetime kidney failure risk persists despite optimized care. This review synthesizes advances in molecular pathogenesis, highlighting how the traditional multi-hit hypothesis-while foundational for targeted therapy development-fails to capture IgAN's recurrent, self-amplifying nature. We introduce the "spi

When contraception goes rogue: Bladder migration of an intrauterine device presenting as refractory cystitis.

Intrauterine device (IUD) migration into the urinary bladder is a rare but significant complication that may mimic chronic cystitis or recurrent urinary tract infections due to nonspecific symptoms such as dysuria, hematuria, suprapubic pain. A 31-year-old multiparous woman presented with persistent lower abdominal pain, fatigue, and urinary symptoms. Imaging and cystoscopy revealed a migrated IUD embedded in the bladder wall. Transurethral removal failed due to fibrosis, but laparoscopic removal was successful, with complete symptom resolution. This case underscores the need to consider IUD m

Beyond bladder clots: Unmasking nutcracker syndrome in refractory hematuria: A case report.

Nutcracker syndrome (NCS) is a rare cause of unexplained hematuria and flank pain. We report a 28-year-old woman with recurrent gross hematuria misdiagnosed as a urinary tract infection. Imaging revealed left renal vein compression from a narrow aortomesenteric angle. Stenting failed due to complications, and she improved with conservative management. This case highlights the importance of vascular imaging in cases of persistent hematuria and underscores the role of nonsurgical management when intervention is not feasible.

Case Report: A rare case of complex Behçet's disease complicated with acute tubular necrosis and IgA nephropathy, coexists with myelodysplastic syndrome, trisomy 8 and intestinal involvement.

Behçet's disease (BD) is a rare systemic disease in which small-vessel vasculitis impacts multiple bodily organs. It is typically marked by recurrent oral and genital ulcers, uveitis, and cutaneous lesions. However, peripheral vessels, cardiovascular structures, central nervous system, gastrointestinal tract, joints, lungs, or kidneys may be affected as well. Renal involvement, although uncommon, may manifest as proteinuria, hematuria, and varying degrees of renal insufficiency. Herein, we describe a 35-year-old man with longstanding BD, myelodysplastic syndrome (MDS), and trisomy 8. He presen

Prostatic artery from an extrapelvic obturator artery: a rare common femoral artery variant with clinical implications.

BACKGROUND: Prostate artery embolization (PAE) is an established treatment option for benign prostatic hyperplasia (BPH). Variations in prostatic artery (PA) origins can present significant technical challenges. CASE PRESENTATION: An 86-year-old male with recurrent bladder cancer and persistent gross hematuria post-TURBT presented for PAE. Intra-procedural angiography revealed a prostatic artery branching from an aberrant obturator artery that originated from a trifurcation of the common femoral artery. PAE was successfully performed with contralateral access and particle embolization. The pa

Prognosis and predictive factors in pediatric IgA nephropathy.

BACKGROUND: IgA nephropathy (IgAN) is a common glomerular disease in children that may progress to chronic kidney disease (CKD) and kidney failure. Identifying reliable prognostic markers is important for guiding clinical management. This study investigated independent predictors of poor prognosis in pediatric IgAN and their impact on eGFR slope, as well as the dynamic patterns of complete kidney remission and relapse, predictive factors associated with remission, and the influence of remission on poor prognosis. METHODS: A retrospective cohort study of 224 children (aged 3-18 years) with bio

Breaking the Nutcracker: a case-based review of diagnosis and surgical treatment.

Nutcracker syndrome is a rare condition caused by compression of the left renal vein between the abdominal aorta and the superior mesenteric artery. This results in increased venous pressure, leading to haematuria, flank pain, and pelvic discomfort, particularly in individuals with low body mass index. Diagnosis is often challenging due to nonspecific symptoms and overlap with other pelvic disorders. We report a case of a 49-year-old woman with persistent left flank pain, dysmenorrhea, and recurrent vaginal bleeding. Imaging confirmed compression of the left renal vein. A conservative approach

Calyceal Diverticulum: A Cystic Lesion Mimicking a Urinary Tract Infection.

Calyceal diverticulum (CD) is an uncommon renal anomaly in children and often mimics other cystic lesions, making diagnosis challenging. We report the case of a nine-year-old boy evaluated during follow-up after an episode of acute pyelonephritis. He remained asymptomatic, with normal examination findings and appropriate growth. Renal ultrasound revealed a non-vascularized cystic lesion with heterogeneous content in the upper pole of the right kidney. Subsequent magnetic resonance imaging demonstrated communication with the collecting system, confirming the diagnosis of CD. While typically asy

Early Experience With Iptacopan for Recurrent IgA Nephropathy After Kidney Transplantation.

Immunoglobulin A (IgA) nephropathy is a common cause of kidney failure and can recur after kidney transplantation, increasing the risk of allograft loss. Effective treatments for recurrent IgA nephropathy in kidney transplant recipients are urgently needed. Iptacopan is a complement factor B inhibitor that received accelerated approval by the US Food and Drug Administration in August 2024 for the treatment of high-risk native IgA nephropathy based on trials that excluded transplant recipients. In this case series, we report our early experience with iptacopan in three individuals with biopsy-c

A presentation of systemic lupus erythematosus manifesting as abdominal pain: a case report.

BACKGROUND: Systemic lupus erythematosus is a multisystem inflammatory disease with a broad range of clinical and serologic presentations. The heterogeneity of presentation poses diagnostic challenges for the clinician, and a high index of suspicion is required. Classification systems exist for both clinical and immunologic criteria; however, they may lack sensitivity in assisting with diagnosis of atypical presentations. We present a case of an initial presentation of systemic lupus erythematosus consisting of nonspecific gastrointestinal symptoms with clinical and immunologic findings that f
