Outcomes and predictors of kidney failure in elderly patients with biopsy-proven IgA nephropathy.

The clinical presentation and prognosis of immunoglobulin A nephropathy (IgAN) in older adults are poorly defined. We evaluated kidney and patient outcomes and predictors of progression in patients aged ≥60 years with biopsy-proven IgAN. We conducted a retrospective observational study in elderly patients diagnosed with primary IgAN between 2010 and 2024 at a tertiary center. Baseline clinical, laboratory, and histopathologic data were collected, and kidney biopsies were scored using the Oxford MEST-C (Mesangial hypercellularity, Endocapillary hypercellularity, Segmental glomerulosclerosis, Tu

Characteristics of patients with undiagnosed stage 3 chronic kidney disease: results from an observational study (REVEAL-CKD) in China.

BACKGROUND: Early diagnosis of chronic kidney disease (CKD) is crucial for timely intervention to delay disease progression and improve patient outcomes. However, data for clinical characteristics of Chinese patients with undiagnosed, early-stage CKD are lacking. METHODS: REVEAL-CKD is a multinational, observational study using real-world data in selected countries to describe factors associated with undiagnosed stage 3 CKD, time to diagnosis, and CKD management post diagnosis. We analysed patient data from 20 hospitals in the China Renal Data System. Adult patients with two consecutive estim

Origin of poorly galactosylated IgA1 other than mucosa: a viewpoint from a report on patient with IgA vasculitis.

Patients presenting monoclonal gammopathy of renal significance (MGRS) and IgA vasculitis collectively have rarely been reported. This study reports one patient with monoclonal IgA and λ, and with IgA positive-mesangial proliferative glomerulonephritis. The patient manifested slight chronic nephritic syndrome, and his serums tested positive for M protein (monoclonal IgA and λ). As a result of bone wear, the plasma cell ratio of these patients was confirmed to be mildly increased in peripheral blood smears. Just like typical IgA nephropathy or IgA vasculitis patients, serum poorly galactosylate

Classical complement activation in light and heavy chain deposition disease with acquired cutis laxa and bronchiolitis obliterans: a case report of monoclonal gammopathy of clinical significance.

Light and heavy chain deposition disease (LHCDD) is a clonal plasma cell or monoclonal B-cell dyscrasia characterized by deposition of monoclonal immunoglobulin light and heavy chains. LHCDD mainly belongs to monoclonal gammopathy of renal significance (MGRS), including a spectrum of kidney disorders caused by a monoclonal protein (M-protein) secreted by a small plasma cell clone or other B-cell clones in patients who do not meet the diagnostic criteria for multiple myeloma or other B-cell malignancies. It may also occur as a renal complication of overt multiple myeloma. We report a 27-year-ol

Clinicopathological discordance in biopsy-proven nephrosclerosis: a nationwide cross-sectional study of the Japan Renal Biopsy Registry (J-RBR).

BACKGROUND: Patients with nephrosclerosis display heterogenous clinical phenotypes, often leading to a clinical diagnosis discordant with pathological nephrosclerosis diagnosis. However, little is known about clinical factors associated with clinicopathological discordance of biopsy-proven nephrosclerosis. METHODS: In a cross-sectional study of 891 patients with biopsy-proven nephrosclerosis registered in the Japan Renal Biopsy Registry (J-RBR) between July 2007 and June 2016, we examined clinical characteristics associated with a pre-biopsy clinical diagnosis discordant with pathological nep

Membranoproliferative glomerulonephritis with deposition of monoclonal IgG evolved from polyclonal IgG: a case report with two consecutive renal biopsies.

BACKGROUND: Proliferative glomerulonephritis with monoclonal Immunoglobulin (G) deposits (PGNMID) is a rare kind of MGRS with intact monoclonal IgG deposition. Seventy percent of PGNMID patients were negative for M-spike. CASE PRESENTATION: A 51-year-old Chinese woman presented with 16-month history of chronic nephritic syndrome. Her first biopsy showed a MPGN pattern, and the IF showed polyclonal IgG deposition but with IgG3λ dominance, MGRS was highly suspected. But the serum/urine IFE and bone marrow examination was negative for monoclonal gammopathy. She was treated with RAS inhibitors, a
