Direct stimulation of Ephrin-B1 induces calcium-independent podocyte injury through Cdc42-calcineurin activation.

Although a subset of nephrotic syndrome has been reported to be caused by autoantibodies against nephrin, its etiology remains poorly understood. A previous study demonstrated that anti-nephrin antibodies phosphorylate not only nephrin but also Ephrin-B1 at the slit diaphragm of podocytes. However, the pathogenic role of Ephrin-B1 in nephrotic syndrome remains unclear. The present study, using cultured human podocytes, demonstrated that direct stimulation of Ephrin-B1 with soluble EphB2 or an anti-Ephrin-B1 antibody induced phosphorylation of both Ephrin-B1 and nephrin. Neither EphB2 nor the a

Four cases of type III collagen deposition glomerulopathy and summary of the literature.

Collagen type III glomerulopathy is a rare glomerular disorder characterized by the deposition of type III collagen fibers within the glomeruli, predominantly in the mesangial and subendothelial regions. Renal biopsy with histopathological examination remains the gold standard for diagnosis of this disease. Clinically, collagen type III glomerulopathy typically presents with proteinuria and progressive renal dysfunction, with or without nephrotic syndrome. Collagen type III glomerulopathy affects individuals across all age groups, ranging from children to adults, and shows no apparent sex pred

Thromboembolic prophylaxis in childhood idiopathic nephrotic syndrome. From pathophysiology to clinical management.

Thromboembolic complications remain among the most serious complications of childhood idiopathic nephrotic syndrome (INS). Although their incidence is considerably lower than in adults, venous thromboembolism continues to cause substantial morbidity and occasional mortality, including pulmonary embolism, cerebral venous sinus thrombosis, and extensive deep venous thrombosis. Preventive management remains controversial because most affected children may not develop thrombosis, whereas pharmacological prophylaxis exposes many low-risk patients to potential overtreatment. Current recommendations

Advancing genomic medicine in Nigeria: research progress, clinical translation and policy priorities.

BACKGROUND: Genomic Medicine involves the comprehensive analysis of an individual's complete DNA set, the genome, to gain insights into the relationship between genes and how they influence diseases and individual responses to treatment modalities. It has created a solid foundation for advancements in precision medicine and understanding complex biological processes. One of the most groundbreaking achievements is the Human Genome Project (HGP), which was launched in October 1990, to generate a complete sequence of the human genome and identify all the genes present in it. OBJECTIVE: This narr

Nephrotic Syndrome During Ipilimumab Plus Nivolumab Therapy for Sarcomatoid Malignant Pleural Mesothelioma: A Case Report of Nivolumab Rechallenge Without Recurrence of Nephrotic Syndrome.

Immune checkpoint inhibitor (ICI)-associated nephrotic syndrome is a rare immune-related adverse event. When it occurs during combination ICI therapy, identifying the causative agent and determining an appropriate strategy for subsequent ICI rechallenge can be clinically challenging. A 67-year-old man with unresectable sarcomatoid malignant pleural mesothelioma received combination therapy with ipilimumab and nivolumab, resulting in tumor shrinkage. Approximately four months after treatment initiation, he developed nephrotic syndrome characterized by severe proteinuria, hypoalbuminemia, and bi

Seroprevalence of varicella antibodies in unvaccinated children with nephrotic syndrome in disease remission.

Background and objectives Varicella-zoster (VZV) virus infection can be severe in children with nephrotic syndrome. Lack of varicella vaccination through the national immunisation programme in India leaves many vulnerable to infection. We conducted this study to document the seroprevalence of Varicella antibodies in patients with nephrotic syndrome. Methods This cross-sectional study (Oct 2024-May 2025) was conducted at a tertiary-care teaching hospital in India, in children (1-18 y) with nephrotic syndrome (both steroid-sensitive and steroid-resistant) in disease remission. Serum VZV IgG anti

A pilot prospective cohort study: Changes in circulating T-lymphocyte subsets during steroid therapy in patients with nephrotic syndrome.

ObjectiveGlucocorticoids are used to treat nephrotic syndrome; however, some patients develop resistance. T-lymphocyte subsets are involved in immune regulation and may be associated with disease progression and treatment response. The current prospective pilot cohort study explored T-cell immune phenotypes associated with steroid responsiveness in patients with nephrotic syndrome.MethodsEleven patients newly diagnosed with nephrotic syndrome between 2023 and 2024, classified as steroid-sensitive or steroid-resistant based on treatment outcomes, and 14 healthy volunteers were enrolled. The Brazilian registry of kidney biopsies: results from the 2022 national prospective registry.

INTRODUCTION: Kidney biopsy plays a central role in the etiological diagnosis and management of kidney diseases. The Brazilian Registry of Kidney Biopsies (BRKB) was established by the Brazilian Society of Nephrology to characterize biopsy-proven kidney diseases nationwide. The aim of this manuscript is to present the BRKB data for 2022. METHODS: We analyzed data from adult patients who underwent native kidney biopsy and were prospectively included in the BRKB between January and December 2022. Demographic, clinical, laboratory, and histopathological variables were evaluated. RESULTS: A tota

Distinct Complement Activation Profiles Between NELL1- and PLA2R-Associated Membranous Nephropathy: A Clinicopathological and Proteomic Study.

AIM: Primary membranous nephropathy (MN) is classified into antigen-defined subtypes, most commonly M-type phospholipase A2 receptor (PLA2R)-associated MN and neural epidermal growth factor-like 1 (NELL1)-associated MN. Whether complement activation differs between these subtypes remains unclear. This study compared their clinicopathological features and complement-related protein expression. METHODS: We retrospectively compared PLA2R-associated MN (n = 17) and NELL1-associated MN (n = 9), including five additional NELL1-associated MN cases from an external institution. Dual Roles of p53-Related Protein Kinase in Cytoskeletal Remodeling and Translational Control.

P53-related protein kinase (PRPK; TP53RK) is an evolutionarily conserved atypical kinase whose biology reveals an unexpected integration of translational control, cytoskeletal regulation, and cell-type-specific stress responses. As a core component of the TCTC complex (Threonyl-Carbamoyl Transferase Complex), PRPK participates in the universally conserved synthesis of the tRNA modification t6A, an essential process for translational fidelity, proteostasis, and cellular viability. Structural and biochemical studies from yeast to humans highlight a deeply conserved ATPase-driven mechanism in whi

Predictive Molecular Biomarkers in Pediatric Steroid-Resistant Nephrotic Syndrome: A Systematic Review.

Nephrotic syndrome is the leading cause of pediatric chronic kidney disease, particularly the steroid-resistant subtype (SRNS). The current diagnostic paradigm relies on prolonged steroid trials or invasive biopsies, often delaying optimal treatment. This study systematically synthesizes evidence from cohort studies on non-invasive molecular biomarkers predicting SRNS at onset. We searched PubMed, Scopus, ScienceDirect, and MEDLINE Ultimate via EBSCO for cohort studies published between January 2015 and September 2025. The study protocol was registered with PROSPERO (CRD420251147997), and qual

Complete Resolution of Unprovoked Left Renal Vein Thrombosis and Pulmonary Embolism Associated With Severe Iron Deficiency Anemia: A Five-Year Follow-Up Case Report and Dual-Energy CT Appraisal.

Combined renal vein thrombosis (RVT) and pulmonary embolism (PE) in young adults without nephrotic syndrome, malignancy, or an identified major thrombophilia is uncommon. Severe iron deficiency anemia (IDA) has been associated with increased thrombotic risk, although a causal relationship cannot be established from an individual case. A 21-year-old male patient presented with flank and chest pain. Diagnostic workup confirmed acute left RVT and an acute embolism of the posterior basal segmental artery of the right lower lobe with pulmonary infarction, concurrent with profound microcytic IDA (he

Varicella zoster virus reinfection in a patient with refractory nephrotic syndrome: a case report.

Permanent immunity to varicella zoster virus (VZV) develops following infection in most patients, with reinfections extremely rare. Here, we report a 6-year-old boy with refractory nephrotic syndrome who contracted VZV infection during B-cell depletion after rituximab treatment. Although developing febrile neutropenia, he was promptly treated with acyclovir, which was successful. The VZV immunoglobulin G antibody titer was weakly positive (3.3) by enzyme immunoassay performed 42 days after the initial diagnosis of VZV infection, confirming the presence of VZV, although antibody production was

Association between anti-rituximab antibodies and treatment efficacy of rituximab in membranous nephropathy.

BACKGROUND AND HYPOTHESIS: Primary membranous nephropathy (pMN) is a common cause of nephrotic syndrome in adults. Rituximab (RTX) is a first-line therapy targeting B cells; however, 30%-40% of MN patients exhibit treatment resistance or relapse. Studies have shown that anti-rituximab antibodies (ARA) may lead to RTX resistance through mechanisms such as drug neutralization. The objective of this study is to explore the relationship between the emergence of ARA and the clinical efficacy of initial RTX treatment in adult MN. METHODS: Patients with MN who received RTX therapy were included in t

Back-Table Reduction of a Deceased Adult Donor Kidney for Transplantation Into a Small Child: A Six-Year Follow Up.

BACKGROUND: Transplantation of adult-sized kidneys into very small pediatric recipients can be limited by pronounced donor-recipient size mismatch. Although intraperitoneal placement is often considered, extraperitoneal implantation may reduce abdominal morbidity, provided that the graft can be accommodated safely. CASE PRESENTATION: We report transplantation of a left kidney from a 46-year-old deceased adult donor into a 15-month-old boy weighing 8.5 kg with congenital nephrotic syndrome and previous bilateral nephrectomy. The 14-cm graft had three arteries of comparable caliber. Because the
