ROS-responsive cellular vesicles with ferroptosis-targeting siACMSD delivery for acute kidney injury therapy.

Background: Acute kidney injury (AKI) is a severe and prevalent nephrotic syndrome which lack of definitive therapies. Alpha-amino-β-carboxymuconic acid-ε-semialdehyde decarboxylase (ACMSD) is a metabolic enzyme mainly expressed in the kidney which exacerbated AKI injury by promoting TCA cycle and inhibiting nicotinamide adenine dinucleotide (NAD+) production, whereas lack of effective intervention strategies for ACMSD-targeted therapy. Methods: Herein, we knocked out ACMSD in vitro through CRISPR-Cas9 method, and developed a reactive oxygen species (ROS)-responsive neutrophil-derived cellular

Anti-PLA2R antibody reduction as a predictor of treatment response in primary membranous nephropathy: a retrospective study.

INTRODUCTION: Primary membranous nephropathy (PMN) is a leading cause of nephrotic syndrome in adults. Although proteinuria has been the conventional guiding parameter in disease monitoring, its limitations as a non-specific and delayed marker have led to the appeal of anti-phospholipase A2 receptor antibodies (PLA2R) as specific and sensitive indicators of disease activity and response to treatment. METHODS: This retrospective cohort study included 60 patients with biopsy-proven PMN and positive baseline anti-PLA2R antibodies who underwent treatment at the Multan Institute of Kidney Diseases

The landscape of allele-specific expression in human kidneys.

Allele-specific expression (ASE), the preferential expression of one gene copy, is a key mechanism of genomic regulation. However, its role in human kidney disease remains poorly understood. In this study, we generated a high-quality, genome-wide ASE map using paired whole-genome sequencing and RNA-seq from microdissected glomerular (GLOM) and tubulointerstitial (TUBE) compartments of patients with proteinuric kidney disease. We showed that the majority of common ASE events were deterministic and sequence-mediated. We also found that diseased kidneys exhibited significantly more ASE in GLOM th

Activation of the alternative complement pathway and its relevance for sodium retention in experimental nephrotic syndrome.

The complement component C3, factor B (FB) and factor D (FD) belong to the alternative complement pathway and have been identified in urine samples from nephrotic mice. However, it is not yet known whether these factors are involved in mediating sodium retention in nephrotic syndrome (NS). Here we used a genetic mouse model of NS based on an inducible podocin deletion (Nphs2Δipod). These mice were intercrossed with mice deficient for FB, FD or C3, yielding Nphs2Δipod*Cfb-/-, Nphs2Δipod*Cfd-/- or Nphs2Δipod*C3-/- mice, respectively. NS was induced after oral doxycycline treatment for 14 days. C

Minimal Change Disease with Progression to Focal Segmental Glomerulosclerosis: Support for an Immune-Mediated Disease Continuum.

Minimal change disease (MCD) and focal segmental glomerulosclerosis (FSGS) are among the most common causes of primary nephrotic syndrome in adults. A 32-year-old female with diagnosed MCD, characterized by podocyte effacement and immunoglobulin G (IgG) dusting on initial biopsy, developed worsening renal function unresponsive to first-line treatment with corticosteroids. Persistence of symptoms led to repeat biopsy, which identified collapsing FSGS that was effectively treated with plasmapheresis (PLEX). This case explores the hypothesis that MCD and FSGS lie along a continuous spectrum of po

Clinical and genetic diagnosis of a patient with focal segmental glomerulosclerosis due to a novel variant of the ANLN gene.

Focal segmental glomerulosclerosis (FSGS), a common cause of nephrotic syndrome, is diagnosed by renal biopsy showing focal segmental sclerosis with foot-process effacement. It has a poor prognosis and can rapidly progress to end-stage kidney disease (ESKD). While the etiology of FSGS is largely unknown, many genes have been linked to the familial form of the condition. One of its subtypes, FSGS8, is caused by a heterozygous mutation in the ANLN gene on chromosome 7p14. However, reports of disease-causing ANLN variants in the literature are scarce. Here, we report a novel variant of ANLN in a

Diagnosis and treatment of multisystem amyloidosis associated with SGPL1 mutation: A case report and review of the literature.

RATIONALE: Amyloidosis is a rare, clinically heterogeneous disease, which makes its diagnosis difficult. The relationship between amyloidosis and gene mutations is insufficiently understood. We report a case of sphingosine-1-phosphate lyase 1 (SGPL1) mutation-related amyloidosis, and review the related literature. PATIENT CONCERNS: A 53-year-old man was admitted to our hospital with a 5-month history of renal dysfunction and abdominal distension, aggravated since 2 days. Five months ago, the patient was diagnosed with nephrotic syndrome, with liver dysfunction and abdominal distension. His sy

Icariin sensitizes glucocorticoid therapy in doxorubicin-induced fibrotic nephrotic syndrome via the HIF-1α/NF-κB/HDAC2 Axis.

Nephrotic syndrome (NS) can progress to tubulointerstitial fibrosis, and a subset of patients exhibits reduced responsiveness to glucocorticoids (GCs). Beyond podocyte injury, the tubulointerstitial microenvironment may critically shape GC efficacy, but the mechanisms remain incompletely defined. We established a doxorubicin (DOX)-induced rat model featuring tubular epithelial injury with epithelial-to-mesenchymal transition (EMT), interstitial fibrosis, and a blunted therapeutic response to prednisone (Pred). We tested whether icariin (ICA) enhances GC efficacy by modulating the HIF-1α/NF-κB/

Prognostic value of the ratio of globally sclerotic glomeruli in patients with idiopathic IgA nephropathy.

IgA nephropathy (IgAN) is the most common primary glomerulonephritis worldwide. We assessed whether the Ratio of Globally Sclerotic Glomeruli (RoGSG) on diagnostic biopsy predicts subsequent kidney outcomes in a nationwide, multi‑center registry. Among 326 adults with idiopathic IgAN (mean age 39.1 ± 12.8 years; 60.1% male), 43 patients (13.2%) met a 5 year composite outcome defined as any of: doubling of serum creatinine or ≥ 50% decline in eGFR from baseline, eGFR < 15 mL/min/1.73 m2, or initiation of kidney replacement therapy. Receiver operating characteristic analysis identified a RoGSG c

B-cell activation underpins Rituximab response in focal segmental glomerulosclerosis.

The pathogenesis of primary focal segmental glomerulosclerosis (FSGS) is incompletely understood, and outcomes remain poor. Some patients respond to Rituximab, especially patients who have hyporesponsive T-cells, but the underlying mechanism is unknown. This study aimed to investigate the association between B-cell activation and T-cell hypo-responsiveness as well as Rituximab response. A cohort of 33 patients with childhood-onset FSGS receiving Rituximab were recruited. T-cell hypo-responsiveness was defined as stimulated T-cell IFNγ <2.5%, and B-cell activation was characterized by CD80 expr

When Minimal Becomes Massive: A Case Report of Childhood-Onset Minimal Change Disease Marching to End Stage Renal Disease Despite Modern Immunotherapy.

We report a rare case of childhood-onset Minimal Change Disease (MCD) in a 31-year-old male, originally diagnosed at age 2.5 with nephrotic syndrome. Repeated biopsies throughout his childhood and adolescence revealed normal glomeruli on light microscopy and diffuse effacement of podocyte foot processes on electron microscopy, confirming pure MCD. Despite multiple relapses, prolonged corticosteroid therapy, and adjunctive immunosuppressants including cyclosporine, tacrolimus, and 2 courses of Rituximab, the patient developed progressive renal decline culminating in end-stage renal disease (ESR

Endothelial cell-released CD93 contributes to podocyte injury in idiopathic nephrotic syndrome.

Idiopathic nephrotic syndrome (INS) is a podocyte disease triggered by immune-derived factors. Endothelial activation occurs in this context, but whether the activated endothelium contributes to podocyte injury is unknown. We tested the hypothesis that CD93, a protein primarily expressed in the endothelium, is a contributory factor of podocyte injury. We studied 460 patients with INS and 150 with other podocytopathies. CD93 was analyzed in kidney tissue, urine, and serum samples. We tested the efficacy of CD93 blockade in vitro and in vivo and investigated the relationship between soluble CD93

Podocyte-derived exosomes instruct dendritic cell-dependent CD8+ T cell activation and proliferation in renal inflammation.

INTRODUCTION: Minimal Change Disease (MCD) represents a prevalent pathological cause of nephrotic syndrome. While podocyte injury is recognized as the direct cause of proteinuria in MCD, aberrant T-cell activation is a key driver of this podocyte damage. However, the absence of T-cell infiltration in renal biopsies and ineffectiveness of immunosuppressive therapy in a subset of patients suggest the existence of undiscovered key mechanisms underlying MCD immunopathogenesis. OBJECTIVES: This study investigates the role of podocyte-derived exosomes (Pdo-Exos) in T cell activation via antigen pre

Dual MPO-ANCA and PLA2R positivity in crescentic glomerulonephritis with MPO-associated membranous nephropathy: a case report with pathogenic insights.

Recent studies have suggested that myeloperoxidase (MPO) may serve as a target antigen in cases of MPO-associated membranous nephropathy (MN) occurring concurrently with MPO-antineutrophil cytoplasmic antibody (ANCA)-associated crescentic glomerulonephritis (GN). We report the case of a 41-year-old Filipino man with MPO-ANCA-positive rapidly progressive glomerulonephritis (RPGN) complicated by the development of nephrotic syndrome. He exhibited no apparent signs of extra-renal vasculitis. Kidney biopsy demonstrated crescentic GN with MPO-associated MN, characterized by active cellular crescent
