The safety of rituximab in pediatric nephrotic syndrome patients: a pharmacovigilance study of the FAERS database.

This study aims to systematically evaluate the safety of rituximab (RTX) in pediatric nephrotic syndrome (NS) using real-world data from the US Food and Drug Administration Adverse Event Reporting System (FAERS). We analyzed FAERS data from Q1 2013 to Q4 2024, focusing on individual case safety reports (ICSRs) involving NS patients under 18 years old where RTX was the primary suspected drug. Adverse events (AEs) were coded using preferred terms (PTs) and mapped to system organ classes (SOCs). Four pharmacovigilance data analysis methods-reporting odds ratio (ROR), proportional reporting ratio

Lymphocytic expression and IL-2 levels in calcineurin inhibitor-resistant nephrotic syndrome due to focal segmental glomerulosclerosis: Implications for calcineurin inhibitor response.

BACKGROUND AND OBJECTIVE: Calcineurin inhibitors (CNI) are the first-line treatment for steroid-resistant nephrotic syndrome (SRNS) due to focal segmental glomerulosclerosis (FSGS), though response rates vary. This study analyzed lymphocyte activation through IL-2 receptor expression on T lymphocytes and serum IL-2Rα (soluble CD25) levels in these patients to evaluate their relationship with response to CNI therapy. MATERIALS AND METHODS: A multicenter case-control study was conducted with 39 patients with steroid-resistant FSGS, diagnosed via renal biopsy, and 23 healthy controls. Clinical,

Durvalumab-associated membranous nephropathy: a case report and brief literature review.

Immune checkpoint inhibitors can trigger renal immune-related adverse events, including glomerular disease. We report an 84-year-old man with non-small cell lung cancer who developed nephrotic syndrome after starting durvalumab. Proteinuria rose to 4+ four weeks into durvalumab therapy, and by week 6 serum albumin had fallen to 2.7 g/dL, prompting discontinuation. He was referred in January 2024 with serum albumin 2.0 g/dL and a urine protein-creatinine ratio of 13.57 g/gCr. Kidney biopsy demonstrated membranous nephropathy: capillary-wall thickening with spike formation on PAM, granular capil

Identification of monogenic variants in steroid-resistant and steroid-sensitive nephrotic syndrome.

BACKGROUND: Steroid-resistant nephrotic syndrome (SRNS) constitutes the second most common cause of chronic kidney disease (CKD) in children. A monogenic cause can be identified in approximately 25% of SRNS cases. In contrast, steroid-sensitive or steroid-dependent nephrotic syndrome (SSNS/SDNS) is typically attributed to multifactorial or immunological causes and rarely linked to monogenic etiology. METHODS: We performed exome sequencing (ES) in 237 families, including 183 with SRNS and 54 with SSNS/SDNS. RESULTS: A (likely) pathogenic variant in a known SRNS gene was identified in 29/183 i

Plasma-saliva correlations for total and unbound mycophenolic acid and its glucuronide in children with nephrotic syndrome.

BACKGROUND: For mycophenolic acid (MPA), therapeutic drug monitoring (TDM) is recommended due to its complex pharmacokinetics. A non-invasive TDM approach using saliva is promising due to its simplicity and minimal invasiveness. This study investigated the relationship between MPA and its glucuronide metabolite (MPAG) levels in saliva and plasma in children with nephrotic syndrome (NS). METHODS: Salivary MPA (sMPA) and MPAG (sMPAG), as well as total MPA (tMPA), total MPAG (tMPAG), and free MPA (fMPA) in plasma were determined for 23 children aged 4-17 years with NS. All children were treated

Efficacy of Recombinant Human Growth Hormone on Glucocorticoid-Induced Short Stature in Children: A Retrospective Controlled Study.

INTRODUCTION: Long-term glucocorticoid (GC) therapy is a leading cause of growth retardation in children with chronic renal and rheumatic diseases. While recombinant human growth hormone (rhGH) is used to counteract these effects, its efficacy relative to spontaneous catch-up growth following GC withdrawal remains inadequately quantified. This study aimed to compare the efficacy of rhGH treatment against spontaneous catch-up growth in children with GC-induced short stature. METHODS: This retrospective, non-randomized controlled study was conducted at a single tertiary children's hospital, inc

Pierson Syndrome: An Update.

Pierson syndrome (PS) is a rare autosomal recessive disorder, primarily characterized by (1) congenital nephrotic syndrome, (2) ocular abnormalities, and (3) neurodevelopmental deficits. It is caused by mutations in the LAMB2 gene, which encodes the laminin β2 chain-a protein subunit that is part of a specific group of proteins known as laminins. These proteins are present in the glomerular basement membrane, neuromuscular junctions, and ocular structures. Although PS exhibits a wide spectrum of phenotypic presentations, the prognosis remains poor, with most patients not surviving beyond early

Axillary artery thrombus in a patient with critical bicuspid aortic stenosis.

Severe aortic stenosis typically presents with reduced exercise tolerance, exertional chest pains, or syncope. We report on a case of a young female on therapeutic anticoagulation and a history of nephrotic syndrome, who presented with subacute limb ischaemia resulting from axillary artery thrombus. Urgent echocardiogram demonstrated a bicuspid aortic valve with critical stenosis, and she underwent surgical aortic valve replacement. Her presenting symptoms resolved after three months of warfarin therapy. This case highlights the importance of systemic evaluation of unexplained arterial thrombo

Clarifying the immunoglobulin light chain variable gene usage in Chinese patients with renal AL amyloidosis.

BACKGROUND: The association between immunoglobulin light chain variable (IGVL) gene usage and clinicopathological features in renal AL amyloidosis requires further research. This study analyzed IGVL genes and their associations with clinicopathological characteristics, organ involvement and survival outcomes in a Chinese cohort. METHODS: We retrospectively enrolled 273 renal AL amyloidosis patients. Amyloid deposits were microdissected and subjected to mass spectrometry (MS)-based proteomics to identify IGVL genes. Clinicopathological features, organ involvement and survival data were systema

The Pathological Role of LDL in Membranous Nephropathy and Diabetic Nephropathy and the Protective Efficacy of LDL Apheresis: A Narrative Review.

Diabetic nephropathy (DN) is the leading cause of end-stage kidney disease worldwide. One-third of patients with DN develop primary glomerulonephritis, and membranous nephropathy (MN) is the most common concurrent glomerulonephritis. Nephrotic syndrome (NS) due to DN and MN is often refractory to immunosuppressants because increased levels of low-density lipoprotein (LDL) not only accelerates kidney injury but also reduce the bioavailability of cyclosporine, a first-line immunosuppressant for MN. Given the pathological role of LDL, especially oxidized LDL, reducing LDL cholesterol levels can h

Clinical Model Predicting Presence of Diabetic Nephropathy in Renal Biopsy Performed on Patients with Diabetes.

Background: Chronic kidney disease (CKD) affects up to 40% of individuals with diabetes mellitus. Given the fact that CKD in diabetics may result from various non-diabetic renal disorders, kidney biopsy remains essential in cases with atypical clinical presentation. The aim of this study was to assess the prevalence of diabetic nephropathy (DN) and other non-diabetic kidney diseases (NDKD) among diabetic patients who underwent renal biopsy. We also tried to find clinical and laboratory markers predicting the presence of DN in renal tissue. Methods: A retrospective analysis of all native renal

Gastrodin alleviates high fructose-induced podocyte mitochondria-mediated apoptosis by inhibiting NLRP6 to facilitate TRIM7-triggered Bok mRNA degradation.

Mitochondria-mediated apoptosis is the key determinant of glomerular podocyte injury. NOD-like receptor family pyrin domain proteins (NLRPs) are aberrant in clinical kidney diseases, but the role in podocyte mitochondrial dysfunction is unclear. Here, we first observed only NLRP6 expression change in nephrotic syndrome patients with proteinuria. Next, we found that mouse glomerular podocyte NLRP6 expression was increased in high fructose-induced proteinuria with mitochondria-mediated apoptosis. Importantly, Nlrp6 deficiency ameliorated these disturbances in mice. NLRP6 downregulation inhibited

Consanguinity-linked genetic variants in Algerian patients with steroid-resistant nephrotic syndrome: a familial study.

BACKGROUND: Steroid-resistant nephrotic syndrome (SRNS) is a severe glomerular disorder frequently progressing to kidney failure, particularly in children. Data on the genetic landscape of SRNS in North African populations, particularly in Algeria, where consanguinity rates are high, remain scarce. This study aimed to investigate the genetic etiology of familial SRNS in a large cohort of Algerian patients using targeted next-generation sequencing (NGS), to bridge the current diagnostic gap and better understand the molecular basis of SRNS in consanguineous populations. METHODS: We conducted a

Next-generation Bruton's tyrosine kinase inhibitors for chronic lymphocytic leukemia-associated membranoproliferative glomerulonephritis: a case report.

Renal involvement in chronic lymphocytic leukemia (CLL) is uncommon but can lead to significant morbidity. Membranoproliferative glomerulonephritis (MPGN) is among the most frequently reported glomerular lesions associated with CLL and may presents with nephrotic syndrome. Early recognition of the association between renal lesions and CLL is crucial for guiding treatment and improving both renal and hematologic outcomes. We report two biopsy-proven cases of CLL-associated MPGN successfully treated with next-generation Bruton’s tyrosine kinase inhibitors (BTKis). Both patients presented with ne

Analysis of risk factors for hypoproteinemia after total hip arthroplasty and construction of a nomogram: A retrospective study.

Postoperative hypoalbuminemia (POHA) remains one of the major complications following total hip arthroplasty (THA) in patients with end-stasge hip disease. Identifying factors that can reduce the incidence of POHA is crucial for improving clinical outcomes in these patients. Our study aimed to develop and validate a nomogram that can pre-operatively quantify an individual patient's risk of POHA. We retrospectively reviewed patients aged ≥65 years who underwent primary unilateral THA between January 2021 and December 2022. Inclusion criteria were: age ≥ 65 years, primary unilateral THA, pre-ope

Clinical characteristics of pediatric Moraxella bacteremia in a children's hospital.

BACKGROUND: Moraxella species are commensal organisms of the upper respiratory tract that only occasionally cause invasive infection. Pediatric Moraxella bacteremia is extremely rare, and its clinical features and optimal management remain poorly defined. METHODS: We conducted a single-center, retrospective observational study at Tokyo Metropolitan Children's Medical Center. Patients with blood cultures yielding Moraxella spp. between April 2010 and March 2024 were identified. Clinical and microbiological data were extracted from electronic medical records. The primary outcome was all-cause 2
