A novel homozygous in-frame deletion variant in TPRKB causing Galloway-Mowat syndrome 5.

Biallelic variants in TPRKB (TP53RK-binding protein) are known to cause Galloway–Mowat syndrome 5 (MIM#617731). It is a rare renal-neurologic disease characterized by early-onset nephrotic syndrome, facial dysmorphism, developmental delay, cerebral and cerebellar atrophy, and central nervous system white matter abnormalities. To date, four families with biallelic variants in TPRKB have been reported. We report two individuals from two unrelated families presenting with Galloway–Mowat Syndrome 5. Whole exome sequencing revealed a novel homozygous in-frame deletion, c.92_94del p.(Arg31del) in ex

B-cell maturation antigen-Targeted T-cell Engager Therapy Combined with B-cell Depletion for Treatment of Refractory HLA Sensitization.

INTRODUCTION: Preformed, high-titer anti-human leukocyte antigen (HLA) antibodies remain a major barrier to successful kidney transplantation, increasing the risk of early graft loss and prolonged time on dialysis. Despite advances in desensitization, current therapies are largely ineffective for patients with extremely high antibody levels. METHODS: Here, we report the case of a 37-year-old woman with kidney failure secondary to NPHS2-related nephrotic syndrome who received no suitable transplant offer despite 20 years of active listing, owing to extreme HLA sensitization (calculated Panel R

Initial treatment patterns of primary membranoproliferative glomerulonephritis in Japan (2017-2021): an updated analysis based on nationwide personal clinical records.

BACKGROUND: Primary membranoproliferative glomerulonephritis (MPGN) is a rare progressive kidney disease that often leads to end-stage kidney disease. Our previous nationwide registry study (Report 1, 2015-2018) characterized initial demographics and treatment patterns. In this study (Report 2), we aimed to update these findings, focusing on newly registered cases (2017-2021). METHODS: Personal clinical records of patients with primary MPGN between 2017 and 2021 were obtained from the national registry organized by the Japanese Ministry of Health, Labour and Welfare. Characteristics of primar

Tacrolimus-induced reversible posterior leukoencephalopathy syndrome in a child with nephrotic syndrome: a case report.

BACKGROUND: Tacrolimus is widely used as an immunosuppressant in the management of refractory nephrotic syndrome. Although effective, it may occasionally lead to rare but serious adverse effects such as posterior reversible leukoencephalopathy syndrome (PRES). PRES has traditionally been associated with hypertension and elevated drug concentrations. CASE PRESENTATION: In the present study, we describe the case of a 10-year-old Chinese girl who was diagnosed with steroid-resistant nephrotic syndrome (SRNS) and pathologically confirmed to have minimal change nephropathy. Following 40 days of fu

Injury-induced paracrine effects on the podocyte's transcriptome.

Although injury in glomerular disease might only damage a subset of podocytes in any given glomerulus, the response of the healthy neighboring podocytes to the injured podocytes oftentimes determines the course of the disease. To investigate this relationship, we designed a dual-chamber open microfluidic coculture device to specifically examine paracrine signaling from podocytes undergoing targeted injury by either adriamycin, puromycin aminonucleoside, or a cytopathic antipodocyte antibody to healthy podocytes. Global transcriptomic analysis measured by RNA sequencing revealed shared and uniq

Treatment outcomes and ongoing pediatric trials in steroid-resistant nephrotic syndrome.

Despite an increasing number of therapeutic options for pediatric patients with steroid-resistant nephrotic syndrome (SRNS), treatment resistance and risk of progression to kidney failure are still high. Children with monogenic forms of SRNS and resistance to non-steroidal immunosuppressants are at the highest risk. Advances in the understanding of SRNS pathogenesis have enabled the development of novel therapies that target genetic, immunologic, and metabolic mechanisms of disease development and progression. In this review, we summarize select contemporary cohort study findings, novel therap

Urinary extracellular vesicles levels of n-3 and n-6 polyunsaturated fatty acids in children with steroid-sensitive nephrotic syndrome: a pilot study.

BACKGROUND: Urine extracellular vesicles (uEVs) are a useful source of non-invasive biomarkers for the diagnosis of kidney diseases. This study aims to characterize the fatty acid (FA) profile of uEVs in children with steroid-sensitive nephrotic syndrome (SSNS) during the active phase of the disease and remission, and to evaluate their association with a pro-inflammatory milieu. METHODS: We enrolled 35 SSNS children at different clinical stages, 10 SRNS children with proteinuria, and 14 healthy controls, and collected urine samples. uEVs were isolated by ultracentrifugation and characterized b

A multicenter prospective real-world cohort study of direct oral anticoagulant use for venous thromboembolism prophylaxis in nephrotic syndrome.

INTRODUCTION: Nephrotic syndrome (NS), especially when associated with severe hypoalbuminia, confers a high risk of venous thromboembolism (VTE). While low-molecular-weight heparin and warfarin are recommended by some guidelines for thromboprophylaxis in high risk patients, data supporting the use of direct oral anticoagulants for primary prophylaxis in NS remain limited and largely observational. METHODS: We conducted a prospective multicentric cohort study across eight tertiary centers in India, enrolling adults (≥18 years) with NS and severe hypoalbuminia (albumin <2.4 g/dL for Membranous n

Characteristics of the gut mycobiome in patients with membranous nephropathy.

Membranous nephropathy (MN), the most common causes of nephrotic syndrome in adults, is known to be associated with a disordered gut microbiota. However, the relationship between the gut mycobiome and MN remains largely unclear. This study aimed to reveal the characteristics of gut mycobiome and explore the potential discrimination ability of intestinal fungi in patients with MN. We collected 154 fecal samples from the First Affiliated Hospital of Zhengzhou University. Internal transcribed spacer (ITS) sequencing was used to analyze the diversity and composition of gut mycobiome. Optimum opera

Novel immune-related susceptibility loci associated with pediatric steroid-sensitive nephrotic syndrome identified by a transethnic genome-wide association study.

BACKGROUND: Steroid-sensitive nephrotic syndrome (SSNS) is a relatively rare glomerular disease with considerable clinical and genetic heterogeneity. Although efforts have been made to explore the genetic determinants of SSNS, the underlying genetic architecture has still been hampered by limited sample size and remains to be elucidated. METHODS: We conducted a genome-wide association study involving 510 sporadic pediatrics of SSNS and 1062 controls with Chinese ancestry, followed by a transethnic meta-analysis involving 2942 cases and 37 041 controls. We evaluated the colocalization probabil

Voclosporin ameliorates proteinuria in a model of non-inflammatory glomerular disease.

BACKGROUND: Idiopathic nephrotic syndrome (INS) is among the most common glomerular diseases in children with standard first line treatment of glucocorticoids. However, there has been no definitive treatment that can completely cure disease with few side effects for steroid resistant INS patients. Voclosporin (VCS) is a second generation calcineurin inhibitor (CNI) approved in multiple countries for the treatment of adults with active lupus nephritis. VCS does not require therapeutic drug monitoring due to an improved pharmacokinetic profile compared to other CNIs. Based on this, we assessed t

When haematuria is more than warfarin: lambda-predominant IgA nephropathy.

A woman in her 20s with a mechanical mitral valve replacement on long-term warfarin therapy presented with frank haematuria and acute kidney injury (AKI). The initial clinical impression was anticoagulant-related nephropathy. However, due to the persistence of urinary abnormalities, further evaluation was undertaken. Renal biopsy revealed immunoglobulin A nephropathy with lambda light chain predominance, raising concern for an underlying monoclonal gammopathy. The patient was managed conservatively with antiproteinuric therapy alone, without immunosuppression, and demonstrated favourable renal

A Case of Pulmonary Thromboembolism in a Child With Steroid-Resistant Nephrotic Syndrome.

A seven-year-old girl with steroid-resistant nephrotic syndrome (SRNS) presented to the Emergency Department with chest pain, palpitations, dyspnea, and generalized edema. She had been receiving immunosuppressants but remained nephrotic. During evaluation, she was found to have an acute occlusive pulmonary thromboembolism (PTE), confirmed by CT pulmonary angiography. Anticoagulation was initiated promptly with heparin, followed by enoxaparin, a low-molecular-weight, long-acting heparin. This case highlights the increased risk of thromboembolic events in children with SRNS due to their hypercoa

Polygenic risk scores and HLA class II variants are biomarkers of corticosteroid response in childhood nephrotic syndrome.

INTRODUCTION: Nephrotic syndrome (NS), a common glomerular disease in children, is classified based on response to corticosteroid therapy as either steroid-sensitive nephrotic syndrome (SSNS), or steroid-resistant nephrotic syndrome (SRNS). However, there are no current reliable predictors of therapy response at initial clinical presentation. METHODS: To evaluate predictors, we conducted genome-wide association studies, developed polygenic risk scores (PRS) for therapy response and analyzed classical HLA alleles in 1,997 children (994 discovery and 1,003 replication/validation cohorts) previo

Concurrence of renal amyloidosis and membranous nephropathy: a case series and literature review.

BACKGROUND: Amyloidosis is a group of heterogeneous diseases characterized by the deposition of amyloid fibrils in various organs and tissues. Membranous nephropathy (MN) is one of the most common causes of nephrotic syndrome in adults. CASE PRESENTATION: We report three cases of early-stage MN occurring concurrently with distinct types of renal amyloidosis: apolipoprotein A-I (AApoA-I) amyloidosis, leukocyte chemotactic factor 2 (ALECT2) amyloidosis, and monoclonal immunoglobulin light-chain (AL) amyloidosis. Each case represents a different pathogenic mechanism, therapeutic approach, and cl
