Lactate controls glomerular endothelial barrier integrity in lupus nephritis.

Systemic lupus erythematosus (SLE) is a progressive autoimmune disease that affects multiple organs and tissues, with lupus nephritis (LN) as one of its most severe complications. Although LN progression is associated with compromised permeability of human renal glomerular endothelial cells (HRGECs), the underlying mechanisms are not fully defined. Herein, we demonstrate that aberrant glycolysis drives this glomerular endothelial barrier defect by suppressing the transcription of tight junction (TJ) genes. Mechanistically, circulating self-DNA in SLE plasma acts as a ligand that activates the

PPP2R3C serves as a negative regulator associated with reduced T cell hyperactivation and renal protection in lupus.

BACKGROUND: CD4+ T cell hyperactivation is a pivotal driver of systemic autoimmunity in systemic lupus erythematosus (SLE), yet the molecular regulators that set its activation threshold remain poorly defined. METHODS: PPP2R3C expression was measured by qRT-PCR and Western blotting in CD4+ T cells from 45 SLE patients and 37 healthy controls, as well as in pristane-induced lupus (PIL) mice. Jurkat cells with PPP2R3C knockdown or overexpression were generated by lentiviral transduction. Signaling mechanisms were dissected using transcriptome sequencing and calcium flux assays. In vivo, PPP2R3C

Glomerulonephritis in Takayasu arteritis is dominated by mesangial and AA amyloid lesions and often arises during remission: a case-based review.

Takayasu arteritis (TAK) is a large-vessel vasculitis in which glomerulonephritis is a rare complication with a poorly characterized clinical and histopathological profile. Patients with biopsy-proven glomerulonephritis were retrospectively identified from a single-center TAK cohort followed between 2014 and 2022. A systematic review of PubMed/MEDLINE, Scopus, and Web of Science for articles published up to December 2025 was conducted in parallel, excluding studies involving patients younger than 18 years and cases with confounding autoimmune conditions or non-classifiable biopsy findings. Thi

Coexistence of PYCR1-related cutis laxa and bilateral grade V vesicoureteral reflux with reflux nephropathy: A case report.

PYCR1-related cutis laxa (autosomal recessive cutis laxa type IIB) is a rare inherited connective tissue disorder characterized by loose, inelastic skin, and variable multisystem involvement. While systemic and neurological manifestations have been well described in the literature, data on renal and urinary tract findings remain sparse. We report a 6-year-old girl with genetically confirmed PYCR1-related cutis laxa who presented with recurrent febrile urinary tract infections. Workup revealed bilateral grade V vesicoureteral reflux alongside a small, atrophic left kidney with cortical defects

Quantitative Glycoproteomics Reveals Distinctive N-Glycosylation Patterns in Human Kidney Tissues with Minor Glomerular Abnormalities.

Kidney diseases present substantial clinical challenges, with aberrant glycoproteins emerging as key pathogenic drivers. Minor glomerular abnormalities (MGAs), a category of unclassified glomerular lesions defined by subtle structural changes, are commonly detected in patients with persistent, asymptomatic, isolated proteinuria or microhematuria. Still their site-specific N-glycosylation patterns remain unexplored. To address this gap, a laboratory-developed pressure cycling technology-based quantitative glycoproteomics workflow was applied to compare intact N-glycopeptides (IGPs) among distan

Comparative analysis of renal dysfunction, immune remodeling, and gut microbiota profiles in dietary models of metabolic liver disease.

BACKGROUND: Metabolic dysfunction–associated steatotic liver disease (MASLD) is increasingly recognized as a multisystem metabolic disorder accompanied by extrahepatic complications, including renal dysfunction. High-fat diet (HFD) and choline-deficient, L-amino acid–defined high-fat diet (CDAHFD) are widely used experimental models of MASLD; however, their comparative effects on hepatic and renal pathology, immune cell composition, and gut microbial profiles remain incompletely characterized. METHODS: Male C57BL/6 mice were fed with a normal chow diet (NCD), HFD (16 weeks), or CDAHFD (8 weeks

Structural dynamics, pathogenic mechanisms, and therapeutic targeting of the NLRP3 inflammasome in lupus nephritis.

Lupus nephritis (LN) is a severe complication of systemic lupus erythematosus (SLE). The nucleotide-binding oligomerization domain-like receptor family pyrin domain-containing 3 (NLRP3) inflammasome plays a central role in the pathogenesis of LN. Aberrant activation of the NLRP3 inflammasome directly promotes glomerular injury, proteinuria, and tubulointerstitial fibrosis through the induction of pyroptosis and the release of interleukin-1β (IL-1β) and IL-18. The expression level of the NLRP3 inflammasome is positively correlated with disease activity, making it both a key driver of acute rena

Radiological and Functional Outcomes of Joshi External Stabilization System (JESS) in Proximal Tibia Fractures: A Case Series.

INTRODUCTION: External fixators have a limited role in the management of tibial plateau fractures (TPF). They are typically used as an initial management strategy. The Joshi External Stabilisation System (JESS) is a Kirschner wire-based fixator and has been used in TPF where plating may not be feasible due to poor skin conditions or delayed due to other soft tissue complications. This retrospective case series aimed to evaluate the radiological and functional outcomes of TPF managed using the JESS fixator. MATERIALS AND METHODS: Records of patients operated on with the JESS fixator for TPF be

Glomerulus-targeted nanotherapy via collagen IV-α3 binding enhances renal immunoregulation in lupus nephritis.

Lupus nephritis (LN) requires long-term immunosuppressive therapy, which is often associated with severe systemic side effects. Therefore, new therapeutic strategies that maintain high efficacy while minimizing adverse effects are essential. Although nanomedicine has advanced systemic and kidney-targeted drug delivery, a reliable method for glomerulus-specific delivery is lacking. Collagen IV (Col4)-alpha 3, located in the glomerular basement membrane at the blood-tissue interface through fenestrated capillary endothelium, represents an ideal target for glomeruli delivery. Herein, we developed

Inhibition of STING modulates dendritic cell responses and ameliorates podocyte injury in SLE.

BACKGROUND: SLE features aberrant type I interferon production, with the stimulator of interferon genes (STING) pathway acting as a key upstream driver. However, its expression patterns in patients with SLE and contributions to clinical phenotypes and tissue injury remain insufficiently defined. METHODS: The proportion of STING-positive cells in peripheral blood mononuclear cells (PBMCs) from 28 patients with SLE and 15 healthy controls was analysed alongside clinical and serological features. Glomerular STING expression and podocyte injury were assessed in kidney biopsies from 39 patients wi

Hydroxychloroquine-induced renal phospholipidosis manifesting as proximal tubulopathy in systemic lupus erythematosus.

BACKGROUND: Hydroxychloroquine (HCQ)-induced renal phospholipidosis typically manifests as glomerular "zebra bodies", "myeloid bodies", and "curvilinear bodies" and is generally considered a benign histological mimic of Fabry disease. We report a case of HCQ-induced renal phospholipidosis with proximal tubulopathy presenting as slowly progressive kidney dysfunction and Fanconi syndrome, challenging the notion that renal phospholipidosis is clinically silent. CASE PRESENTATION: A 36-year-old woman with systemic lupus erythematosus (SLE) treated with HCQ for 18 months presented with slowly prog

MPO-ANCA-associated glomerulonephritis with immune complex deposition and membranous features in a patient with Sjögren's syndrome: a case report.

Antineutrophil cytoplasmic antibody (ANCA)-associated vasculitis (AAV) is classically characterized by pauci-immune necrotizing crescentic glomerulonephritis; however, immune complex deposition is increasingly recognized in a subset of patients, and its clinical significance remains incompletely defined. Coexistence with membranous nephropathy (MN)-like features is uncommon and may complicate diagnosis and treatment. We report a 37-year-old woman with persistent urinary abnormalities detected during routine health screening. Laboratory evaluation showed proteinuria (1.83 g/gCr), microscopic he

Tiny warriors, big impact: targeted immunotherapy in kidney diseases.

Recent advances in immunotherapy targeting B cells have ushered in a new era for disease treatment. The effectiveness of targeted approaches such as T-cell engagers or B cell-directed chimeric antigen receptor (CAR)-T-cell therapy in treating autoimmune diseases has been demonstrated so far only in uncontrolled, investigator-initiated trials. Several case series in patients with therapy-resistant systemic lupus erythematosus showed efficacy and safety of CD19+ CAR-T cells, showing depletion of autoreactive B cells and full clinical remission. Autoreactive B cells not only mediate pathogenesis

Advances in neoadjuvant therapy for locally advanced head and neck squamous cell carcinoma (Review).

PURPOSE: Identifying factors that affect recovery or restoration of neurological function is a key goal of rehabilitation in neurology and ophthalmology. One such factor can be prolonged mental stress, which may be not only the consequence of nervous system damage but also a major risk factor, or cause, of neural inactivation. Using the visual system as a model of neural injury, we wished to study how patients' stress and personality profiles correlate with vision recovery as induced by therapy with alternating current stimulation (ACS) in patients with optic nerve damage.
